Primary Healthj CERTIFICATE OF ZONING COMPLIANCE REPORT DATE: August 23, 2013 TO: Matthew Witt FROM: Kristy Vigil, Assistant City Planner SUBJECT: Primary Health— CZC-13-106 and DES -13-101 OWNER: PH Gramercy, LLC DESCRIPTION OF APPLICANT'S REQUEST The applicant, Matthew Witt, requests Certificate of Zoning Compliance (CZC) and Design Review (DES) approval of a 5,618 square -foot healthcare building on .73 acres of land in the C- G zoning district. The site is located at 1623 S. Wells Avenue. DECISION The applicant's request for Certificate of Zoning Compliance and Design Review is approved with the conditions listed in this report. Note: This is not a building permit. Please contact Building Services at (208) 887-2211 to verify if you need a building permit and/or inspection. If you do need a building permit, you must complete that process before you commence the use or construction. As part of tine application submittal, you will need to provide one set of the final, stamped "approved" Planning Deparimentplans, stamped and signed by the architect and/or civil engineer as 'applicable. You must provide these plans in PDF format and hardcopy as specified in the Building Services "Plan Intake Checklist" and include them with your building submittal set. Please contact Building Services for additional details about building permits and inspections. Process Conditions of Approval 1. No signs are approved with this application. Prior to installing any signs on the property, the applicant shall submit a sign permit application consistent with the standards in UDC Chapter 3 Article D and receive approval for such signs. 2. The applicant shall complete all improvements related to public life, safety, and health as set forth in UDC 11 -5C -3B. A surety agreement may be accepted for other improvements in accord with UDC 11 -5C -3C. 3. Upon installation of the landscaping and prior to inspection by Planning Department staff, the applicant shall provide a written certificate of completion as set forth in UDC 11 -3B -14A. 4. The site plan prepared by Rudeen Architects on August 19, 2013, labeled A1.01, is approved (stamped "approved" on August 23, 2013 by the City of Meridian Planning Department) with no changes). Conditions Document 1 Primary Health— CZC-13-106 and DES -13-101 5. The landscape plan prepared by Mychal Gordon Design on July 31, 2013, labeled L1.0, is approved (stamped "approved" on August 23, 2013 by the City of Meridian Planning Department) with the following changes: 1) Install one tree in the south perimeter landscape buffer as redlined on the plan. 6. The approved site plan, landscape plan and/or elevations may not be altered without prior written approval of the City of Meridian Planning Department. 7. The elevations prepared by Rudeen Architects on August 1, 2013, labeled A3.01 & A3.02, are approved (stamped "approved" on August 23, 2013 by the City of Meridian Planning Department) with no changes. 8. The applicant shall pay any applicable impact fees prior to the issuance of a building permit. 9. If any changes must be made to the site plan to accommodate ACED requirements, the applicant shall submit a new site plan to the City of Meridian Planning Department for approval prior to issuance of the building permit. 10. The applicant shall complete all required improvements prior to issuance of a Certificate of Occupancy. It is unlawful to use or occupy any building or structure until the Building Official has issued a Certificate of Occupancy. Ongoing Conditions of Approval 1. The applicant and/or assigns shall have the continuing obligation to provide irrigation that meets the standards as set forth in UDC 11-313-6 and to install and maintain all landscaping as set forth in UDC 11-313-5, UDC 11-3B-13 and UDC 11-313-14. 2. The project is subject to all current City of Meridian ordinances and previous conditions of approval associated with this site (AZ -06-021, PP -06-019, FP -06-048, and DA #106141056). 3. The issuance of this CZC does not release the applicant from any previous requirements of the other permits issued for the site. 4. The applicant and/or property owner shall have an ongoing obligation to prune all trees to a minimum height of six feet above the ground or sidewalk surface to afford greater visibility of the area. 5. The applicant has a continuing obligation to comply with the outdoor lighting provisions as set forth in UDC 11-3A-11. 6. The applicant and/or property owner shall have an ongoing obligation to maintain all landscaping and constructed features within the clear vision triangle consistent with the standards in UDC 11-3A-3. CITY COUNCIL REVIEW The applicant or a party of record may request City Council review of a decision of the Director. All requests for review shall be filed in writing with the Planning Department on or before September 9, 2013, within fifteen (15) days after the written decision is issued, and contain the information listed in UDC 11 -5A -6B. Conditions Document 2 Primary Health — CZC-13-106 and DES -13-101 If City Council review of the decision is not requested, the action of the Director represents a final decision on a land use application. You have the right to request a regulatory taking analysis under Idaho Code 67-8003. EXPIRATION Certificates of Zoning Compliance issued in conjunction with a proposed use shall expire if the use has not commenced within one year of the date of issuance of the Certificate of Zoning Compliance. Certificates of Zoning Compliance issued in conjunction with construction or alteration of a structure shall expire if the construction or alteration has not commenced within one year of the date of issuance of the Certificate of Zoning Compliance. Wells Avenue. REVISED AUGUST 21, 2013 To Whom It May Concern: The attached Design Review and Certificate of Zoning Compliance documents are for the proposed construction of the Meridian Primary Health Clinic at 1623 S. Wells Ave. in Meridian, Idaho. The building, as proposed, is 5,618 s.f, and will contain ten exam rooms, two procedure rooms, and an X-ray suite, as well as support spaces for the primary use areas. The site is .73 acres and the work will include paving, concrete slabs and sidewalks, drainage swales, and landscaping. Site Access — The site is located at the corner of Overland Road and Wells Avenue. Vehicular ingress and egress of the site will be via a shared access lane connecting to Wells Avenue and located at the southeast corner of the property. Pedestrian Walks- Sidewalks currently exist along Overland Road and Wells Avenue right-of- ways. The main entrance of the facility will connect to the existing sidewalk at Wells Ave. via a 7'-0" wide sidewalk at the south side of the building. Building Orientation and Layout- The building as proposed, is positioned just outside the 35' landscape buffer Overland Road and 20' from the east property line at Wells Ave. This location positions the main build facade as close Overland Road as possible, and provides additional screening of the parking area to the south from the main road. Parking- The lot has 27 parking spaces, which include two accessible spaces adjacent to the main entry and ten compact stalls. The lot is located at the south portion of the site and is screened from view by the building. Parking spaces are broken up with landscape planters and landscape buffers, including 5' wide buffer along south access drive lane for Gramercy Development. A trash enclosure is located in the south east portion of the lot, and is integrated into the lot and the surrounding landscape area. Landscaping — The proposed site will have approximately 13,000 s./f of landscaping. It will be a mix of lawn areas and planters, with trees throughout the landscape areas. Additional landscaping planters are proposed in the parking lot as well as along the building fapade. See attached Landscape Plan. Architectural Character — The building will be wood -frame constructed with stucco and stone veneer finishes. The overall design of the building features multiple architectural components rendered in a variety of textures and colors, which will provide visual interest and architectural character, while minimizing flat surfaces and large featureless masses. Copper finished metal roofing and awnings delineate the main entrance of the building and provide a premium accent material that will be visible from Overland. The various building masses feature stone -veneer pilasters and complimentary stucco walls. All windows and doors are anodized aluminum frame storefront system. Primary Entrance- The primary public entrance is at the west elevation of the structure, and is visible from Overland Road. The entrance is delineated by a large copper -clad barrel vault roof as well as copper finished awnings above the front windows. Roof Lines- The building has a flat roof with parapet walls. The height of the parapet walls varies, as shown on elevations. Color & Texture Variations- See attached exterior building elevations and Color Board Mechanical Equipment- The project will have five roof top mechanical units. Each will be screened by 24" — 32" parapet walls around the perimeter of the building. Mechanical units are placed close to the center of the roof, completely screened from view. Color and Materials- The structure is proposed to be a mix of colored stucco, stone veneer and copper accents. The structure will also have a barrel -vault standing seam roof element at the main entry. Signs and Lighting- The building as proposed will have one main sign above the covered entry, and two smaller signs at the front fapade. See attached elevation for locations and approximate sizes. The area adjacent to the building will be lit with wall mounted and recessed (at covered sidewalk area) exterior florescent fixtures.
