DESCRIPTION OF APPLICANT'S REQUEST
The applicant, Tim Miller, requests Certificate of Zoning Compliance (CZC) and Design Review (DES)
approval of a new 29,987 square -foot building for an ambulatory surgery center in an L -O zoning district.
This type of use is classified in the Unified Development Code (UDC) as a healthcare service and is listed
as a principal permitted use in the L -O zoning district.
The site is located at 520 S. Eagle Road.
DECISION
The applicant's request for CZC is approved with the conditions listed in this report.
Note: This is not a building permit. Please contact Building Services at (208) 887-2211 to verify if you
need a building permit and/or inspection. If you do need a building permit, you must complete that
process before you commence the use or construction. As part of the application submittal, you will
need to provide one set of the final, stamped "approved" Planning Department plans, stamped and
signed by the architect and/or civil engineer as applicable. You must provide these plans in PDF
format and hardcopy as specified in the Building Services "Plan Intake Checklist" and include them
with your building submittal set. Please contact Building Services for additional details about building
permits and inspections.
Site Specific Conditions of Approval
1. Business hours of operation within the L -O and C -N districts shall be limited from 6 am to 10 pm
as set forth in UDC 11 -2B -3A4.
2. Construct the multi -use pathway, lighting, and landscaping as depicted on the approved site plan
(see process condition number 11 and the analysis section below).
Process Conditions of Approval
1. No signs are approved with this application. Prior to installing any signs on the property, the
applicant shall submit a sign permit application consistent with the standards in UDC Chapter 3
Article D and receive approval for such signs.
2. The applicant shall complete all improvements related to public life, safety, and health as set
forth in UDC 11 -5C -3B. A surety agreement may be accepted for other improvements in
accord with UDC 11 -5C -3C.
3. Upon installation of the landscaping and prior to inspection by Planning Department staff, the
applicant shall provide a written certificate of completion as set forth in UDC 11 -3B -14A.

4. The site plan prepared by CTA, Inc., dated December 2012, labeled C1, is approved (stamped
"approved" on January 4, 2013 by the City of Meridian Planning Department) with the
following changes:
• The proposed compact stalls shall be designated on site (see redlines on plan).
5. The landscape plan prepared by CTA, Inc., on December 27, 2012, labeled 1,101 and L201, is
approved (stamped "approved" on January 4, 2013 by the City of Meridian Planning
Department) with the following changes:
• Plant one tree in each interior planter per UDC 11-3B-8C2d (see redlines on plan).
6. The elevations prepared by CTA, Inc., on December 12, 2012, labeled A201, are approved
(stamped "approved" on January 4, 2013 by the City of Meridian Planning Department) with
no changes.
7. The approved site plan, landscape plan and/or elevations may not be altered without prior
written approval of the City of Meridian Planning Department.
8. The applicant shall pay any applicable impact fees prior to the issuance of a building permit.
9. If any changes must be made to the site plan to accommodate ACRD requirements, the
applicant shall submit a new site plan to the City of Meridian Planning Department for
approval prior to issuance of the building permit.
10. The applicant shall complete all required improvements prior to issuance of a Certificate of
Occupancy. It is unlawful to use or occupy any building or structure until the Building Official
has issued a Certificate of Occupancy.
11. As required by UDC 11-31-140, the applicant shall construct and install a multi -use pathway,
lighting, and landscaping consistent with the Eagle Road corridor study.
12. Prior to Certificate of Occupancy, the applicant shall record a public access easement for the
multi -use pathway along Eagle Road and submit copy of the subject easement to the Planning
Department.
Ongoing Conditions of Approval
1. The applicant and/or assigns shall have the continuing obligation to provide irrigation that
meets the standards as set forth in UDC 11-313-6 and to install and maintain all landscaping as
set forth in UDC 11-3B-5, UDC 11-313-13 and UDC 11-3B-14.
2. The project is subject to all current City of Meridian ordinances and previous conditions of
approval associated with this site.
3. The issuance of this CZC does not release the applicant from any previous requirements of the
other permits issued for the site.
4. The applicant and/or property owner shall have an ongoing obligation to prune all trees to a
minimum height of six feet above the ground or sidewalk surface to afford greater visibility of
the area.
5. The applicant shall have an ongoing obligation to maintain all pathways.
6. The applicant has a continuing obligation to comply with the outdoor lighting provisions as set
forth in UDC 11-3A-11.
7. The applicant and/or property owner shall have an ongoing obligation to maintain all
landscaping and constructed features within the clear vision triangle consistent with the
standards in UDC 11-3A-3.

ANALYSIS OF UDC REQUIREMENTS RELATED TO SITE SPECIFIC CONDITIONS:
The Director's Determination for this application is that the improvements proposed by the
applicant meet the intent of 11-31-1-40 in providing a multiuse pathway, lighting, and
landscaping for a section of the Eagle Road frontage proportional to the increment of
development proposed. It is understood that as the rest of the St. Luke's campus continues to
develop the remainder of the Eagle Road frontage will also be improved per the UDC.
CITY COUNCIL REVIEW
The applicant or a party of record may request City Council review of a decision of the Director. All
requests for review shall be fled in writing with the Planning Department on or before January 21,
2013, within fifteen (15) days after the written decision is issued, and contain the information listed in
UDC 11 -5A -6B.
If City Council review of the decision is not requested, the action of the Director represents a final
decision on a land use application. You have the right to request a regulatory taking analysis under
Idaho Code 67-8003.
EXPIRATION
Certificates of Zoning Compliance issued in conjunction with a proposed use shall expire if said use has
not commenced within one year of the date of issuance of the Certificate of Zoning Compliance.
Certificates of Zoning Compliance issued in conjunction with construction or alteration of a structure
shall expire if said construction or alteration has not commenced within one year of the date of issuance
of the Certificate of Zoning Compliance.
In accord with the above provisions, the subject Certificate of Zoning Compliance is valid until January
4, 2014.

CTA Architects is hereby submitting CZC & Design Review documents for a New 30,000 sq. ft. Ambulatory
Surgery Center for the Meridian St Lukes Campus located at 500 S Eagle Rd, Meridian, Idaho.
The project site is currently zoned L -O and the proposed improvements are an allowable use. The
property is well suited for the proposed facility.
1. The architectural character of the proposed building follows the existing campus architecture style
that has been established by St Luke's as a brand recognition standard for their facilities. Please
see the following:
a. Facades:
The exterior of the proposed building is clad using the blended red brick found on all St Luke's
buildings as well as Stucco banding and accent caps. The building has pilasters located on all four
facades to break up the brick walls that are capped with a stucco cornices to mimic precast
concrete and stone elements. This pattern is widely used on the existing hospital and medical
office building and is repeated on the proposed building. The building has brick at the base, with
stucco accents in an earth toned color at the higher elements on the building to give the building a
proper proportion to the pedestrian and observer exuding a sense of connective aesthetic with the
ground.
b. Primary Entrances:
The building has a large entrance canopy to drop off and pick up medical patients coming to this
facility. Additionally there is a smaller canopy used for the discharge of patients. Both of these
canopies are located on the west elevations of the building. The service entrances and other
required exits are located on the north and south elevations. The south exit door is recessed and
set back from the main brick elevation of the building to provide some shelter to this exit. The
northern fagade has the stair tower exit and utility room access doors that will be painted to blend
with and complement with the brick exterior.

The building has several edges creating step backs to the elevations and the general height of the
building. The first floor footprint has a parapet height of 18'-0". Most of the north, south and west
first floor elevations have this parapet height. To distinguish the main entrance, the roof lines
along a 42 foot wide section of the west elevation reach up to 34'-6" feet above the ground level.
Other occupied spaces on the second floor create parapet heights of 34'-6" along the East elevation
and the NE corner of the building. The mechanical screening and mechanical penthouse areas on
the roof reach a height of 40'-10" and 42'-10" heights. These areas are mostly stepping back from
the first and second floor roof parapet with the exception of the roof lines at the west main
entrance area of the building. This tower element is designed to emphasize the entrance to the
building and give hierarchy to the building mass similar to what has been done on the St Luke's
campus previously.
d. Pattern Variations:
The use of the brick and stucco elements on the exterior create pattern variations by the use of 1",
2" and 4" setbacks in the brick and solider coursing. The reveals in the stucco also create shadow
lines and articulate the buildings mass as a stacked tectonic aesthetic to the mass of the building.
The color pallet is limited to be simple and non disruptive to the design aesthetic of the existing
campus. Brick pilasters at the four main corners of the building extend out 4" beyond the main
walls of the building to create interest at the corners of the building. The pilasters are accented
with a stacked running bond brick reveals at the center of the 4'-0" wide pilasters. In all there are
32 pilasters designed to break up the brick patterned base of the building.
The stucco vertical elements on the second story elevations of the building and for the mechanical
parapet screens will have the same rhythm and patterning as the brick pilasters. They are detailed
with reveals to break up the mass of the second story exterior walls working together with the
brick below to form a rhythm that follows the look of all St Luke's buildings.
e. Mechanical equipment:
The mechanical equipment on the roof will be screened on the North, West and South elevations
with a 7'-0" screening wall that is integrated with the second floor parapets. The mechanical
equipment is set back far enough from the Eastern second floor roof parapets, that the screening is
not needed for the east areas of the mechanical roof units because of this set back and the height
of the second floor roof parapets.
2. Materials:
The new facility and site improvements are designed to complement and follow the design
aesthetic of established hospital campus. Please refer to the building elevations and rendered
image of the building included with the CZC application packet for indication of the building exterior
materials
3. Parking Lots:
The site plans indicate the expansion of the parking lot to west of the proposed facility.
4. Pedestrian Walkways and Facilities:
A 10'-0" pedestrian pathway will replace the existing 5'-0" wide/gutter connected sidewalk along
Eagle road for approximately 412 feet of the existing ITD developed street frontage. This item was
discussed in detail with the City of Meridian planning staff on Friday Dec. 21St, 2012. The proposed
development of the 10' wide pedestrian pathway is an effort to meet the current and future
pedestrian access needs to and from this site. Existing sidewalks running along the St Luke's

parking lot entrance area are to remain and be expanded to abut the parking stalls in the adjoining
parking areas. A new pedestrian sidewalk will connect the new parking lot to the west with the
new parking and drop off driveway at the west entrance of the building.
