Dear Mr. Rich:
Central District Health Department has reviewed and does approve the. final plat for this
subdivision for central water and central sewer facilities. Final approval was given -
Apri124, 2013.
Sanitary restrictions as required by Idaho .Code, Title 50, Chapter 13 have been satisfied based
on the State of Idaho, Department of Environmental Quality (DEQ) approval of the design
plans and specifications and the conditions imposed on the developer for continued
satisfaction of the sanitary restrictions. Buyer is cautioned that of the time of this approval, no
drinking water extensions or sewer extensions were constructed. Building construction can be
allowed with appropriate building permits if drinking water extensions or sewer extensions.

If the developer fails to construct facilities then sanitary restrictions may be reimposed, in
accordance with Section 50-1326, Idaho Code, by the issuance of a certificate of disapproval,

Nampa & Meridian Irrigation District (NMID) has a Land Use Change Application on file and is
in the process of reviewing this proposed project.
Plans show for irrigation water to be supplied through NMID's pressurized irrigation system.
NMID's Kennedy Lateral courses through this project with an easement of fifty-five feet (55');
twenty feet (20') left and thirty-five feet (35') right of centerline facing downstream.
Once NMID has finished its review, an acceptance letter will be issued and a License
Agreement drafted.

The Idaho Transportation Department has reviewed the referenced final plat application for the
Larkspur No. 5 Subdivision near Meridian Road. ITD has the following comments:
1) ITD has no objection to the final plat application and requires no mitigation.
2) The Edmonds Court intersection with SH-69 is fully improved and SH-69 has a center two-
way left turn lane. ITD has no plans for any current plans change to SH-69. Increasing
traffic volumes may necessitate an improved level of access control in the future and may
include turn prohibitions. Future subdivision plans should include roadway
interconnections to the adjacent subdivisions to ensure access to the regional roadway
network.
