DESCRIPTION OF APPLICANT'S REQUEST
The applicant, Luke Randall requests certificate of zoning compliance (CZC) and design review (DES)
approval to construct a new 2,385 square foot office building located at 1540 W. Cayuse Creek Drive.
The proposed healthcare and social service use is a principal permitted use in the L -O zoning district, per
UDC Table 11-2B-2.
DECISION
The applicant's request for CZC and DES is approved with the conditions listed in this report.
Note: This is not a building permit. Please contact Building Services at (208) 887-2211 to verify if you
need a building permit and/or inspection. If you need a building permit, you must complete that
process before you commence the use or construction. As part of the application submittal, you must
provide these plans in PDF format and hardcopy as specified in the Building Services "Plan Intake
Checklist". Please contact Building Services for additional details about building permits and
inspections.
Site Specific Conditions of Approval
1. All ground level mechanical equipment shall be screened to the height of the unit as viewed
from the property line in accord with UDC 11-3A-19A.1e.
Process Conditions of Approval
1. No signs are approved with this application. Prior to installing any signs on the property, the
applicant shall submit a sign permit application consistent with the standards in UDC Chapter 3
Article D and receive approval for such signs.
2. The applicant shall complete all improvements related to public life, safety, and health as set forth
in UDC 11 -5C -3B. A surety agreement may be accepted for other improvements in accord with
UDC 11 -5C -3C.
3. Upon installation of the landscaping and prior to inspection by Planning Department staff, the
applicant shall provide a written certificate of completion as set forth in UDC 11 -3B -14A.
4. The site plan prepared by Habitec Architects LLC, dated 04/04/12, labeled Al is approved
(stamped "approved" on 04/20/12 by the City of Meridian Planning Department) with no
changes.
5. The landscape plan prepared by The Land Group, dated 12/13/05, labeled L1.00 is approved
(stamped "approved" on 04/20/12 by the City of Meridian Planning Department) with the

construction on the site.
6. The building elevations prepared by Habitec Architects LLC, dated 03/16/12, labeled A3.1 and
A3.2 are approved (stamped "approved" on 04/20/12 by the City of Meridian Planning
Department) with no changes.
7. The approved site plan, landscape plan and/or building elevations shall not be altered without
prior written approval of the City of Meridian Planning Department.
8. The applicant shall pay any applicable impact fees prior to the issuance of a building permit.
9. If any changes must be made to the site plan to accommodate ACHD requirements, the applicant
shall submit a new site plan to the City of Meridian Planning Department for approval prior to
issuance of the building permit.
10. The applicant shall complete all required improvements prior to issuance of a Certificate of
Occupancy. It is unlawful to use or occupy any building or structure until the Building Official
has issued a Certificate of Occupancy.
Ongoing Conditions of Approval
1. The applicant and/or assigns shall have the continuing obligation to provide irrigation that meets
the standards as set forth in UDC 11-3B-6 and to install and maintain all landscaping as set forth
in UDC 11-3B-5, UDC 11-3B-13 and UDC 11-3B-14.
2. The project is subject to all current City of Meridian ordinances and previous conditions of
approval associated with this site (CUP -03-008 and CUP -05-013, PP -05-011, FP -05-062, and DA
#103137116 and amended DA #107145935).
3. The issuance of this CZC does not release the applicant from any previous requirements of the
other permits issued for the site.
4. The applicant and/or property owner shall have an ongoing obligation to prune all trees to a
minimum height of six feet above the ground or sidewalk surface to afford greater visibility of the
area.
5. The applicant has a continuing obligation to comply with the outdoor lighting provisions as set
forth in UDC 11-3A-11.
6. The applicant and/or property owner shall have an ongoing obligation to maintain all landscaping
and constructed features within the clear vision triangle consistent with the standards in UDC 11-
3A-3.
CITY COUNCIL REVIEW
The applicant or a party of record may request City Council review of a decision of the Director. All
requests for review shall be filed in writing with the Planning Department on or before 05/4/2012, within
fifteen (15) days after the written decision is issued, and contain the information listed in UDC 11 -5A -6B.
If City Council review of the decision is not requested, the action of the Director represents a final
decision on a land use application. You have the right to request a regulatory taking analysis under Idaho
Code 67-8003.

EXPIRATION
Certificates of Zoning Compliance issued in conjunction with construction or alteration of a structure
shall expire if said construction or alteration has not commenced within one year of the date of issuance
of the Certificate of Zoning Compliance.
The administrative design review shall be exercised with the approval period of the underlying permit
(CZC) or the approval shall expire.
In accord with the above provisions, the subject Certificate of Zoning Compliance and Design
Review is valid until April 20, 2013.

Please find enclosed our applications for Certificate of Zoning Compliance and Design Review for one new 2,385 sq. ft.
professional optometry practice to be constructed at 1540 W. Cayuse Creek Drive, on Lot 8, a 0.167 acre, pad ready,
site with existing parking, sidewalk, lighting, utility services, and trash enclosure. This will be the 70, building of the 12
building complex.
This building will be a wood frame and truss structure, with stucco exterior and manufactured stone and recycled
stained wood accents with architectural shingle gable roofs and covered shed roof entry element. All exterior materials
and design elements will match in color and style, the six existing buildings. Site and building layout is consistent with
the previous Plat ((File No. PP -05-011) and CUP (File No. CUP -05-013) as well as previously approved CZC File No.
06-086 that was never acted upon.
Please note that all site lighting is existing, and no photometric analysis is being provided at this time. Existing site
lighting fixtures do not exceed 1,800 lumens each and do not project light over any of the perimeter property lines of
the development. Also note that the trash enclosure is an existing structure and as such no details are included.

Please find enclosed our applications for Design Review for one new 2,385 sq. ft. professional optometry practice to
be constructed at 1540 W. Cayuse Creek Drive, on Lot 8, a 0.167 acre, pad ready, site with existing parking, sidewalk,

Section 1.0 Site Character:
• With the site being in an established subdivision with the aforementioned amenities already existing the
majority of the responses are either Yes (existing conditions) or N/A. Site and building layout is consistent
with the previous Plat ((File No. PP -05-011) and CUP (File No. CUP -05-013) as well as previously approved
CZC File No. 06-086 that was never acted upon.
• Parking is provided on one side only and contained between buildings with the narrow dimension of the lot
facing Linder Road.
• All perimeter walks and landscaping are existing.
• The site takes access from a local street that then intersects with the arterial street.
Section 2.0 Architectural Character:
• Again with 6 existing buildings, this new structure utilizes all of the same materials and elements to create the
desired cohesive design statement expressed and repeated throughout the development.
• The building is wood frame with truss roof. The primary exterior material is a tan stucco. Painted Hardi

shingle siding accents the upper portion of the two gable ends along with decorative truss elements fabricated
from recycled full size lumber.
• Windows are vinyl exterior/wood clad interior with stained recycled wood trim and built up sill & corbel
supports.
• The entrance is covered by a shed roof element, at the gable end, supported by two manufactured stone
columns.
• The Linder road side has had a gable element added to the center of the elevation which provides interest in
the street side elevation, provides a consistent element for proposed building signage and is similar in size and
appearance to the entrances of two of the smaller existing buildings on the site.
• Roof slopes are consistent with the existing buildings at 7/12 and 12/12 at the Linder road accent.
• The smaller buildings (which this is) are much simpler in footprint and roof plan than the larger end anchor
buildings, promoting a variation in massing, while maintaining harmony of materials and forms.

No, you do not need to submit a TIS to ACRD. Yes, after you receive approval from the City of Meridian
you will need to submit plans to ACHD for impact fee assessment. The submittal info is in the City of
Meridian packet for building permit. Impact fees will need to be paid prior to issuance of a building
permit. This use will be classified as "Medical" for impact fee purposes.
