:Dear Mr. Rich:
Central District Health Department has reviewed and does approve the final plat for this
subdivision for central water and centralsewer facilities. Final approval was given
May 23, 2012.
Sanitary restrictions as required by Idaho Code, Title 50, Chapter 13 have been satisfied based
on a review by a Qualified .Licensed Professional .Engineer. (QLPE) representing the City. of
Meridian and the QLPE approval of the design plans and specifications and the conditions
imposed on the developer for continued satisfaction of the sanitary restrictions. Buyer is
cautioned that atthe time of this approval, no drinking water extensions or sewer extensions
were., constructed. Building construction can be allowed with appropriate building permits if
drinking water extensions or sewer extensions have since been constructed or if the developer
is simultaneously constructing those facilities. If the developer fails to construct facilities then
sanitary restrictions may be reimposed, in accordance with Section 50-1326; Idaho Code, by
the issuance. of a certificate of disapproval,-and no construction of any building or shelter
requiring drinking water or sewer/septic facilities shall be allowed.

After review of the Final Plat of the above-mentioned application, Settlers Irrigation District
requests the following:
1. .All irrigation/drainage facilities along with their easements must be protected and
continue to function, The facility involved is the Settlers North Side Canal (50'
.easement).
2. A Land Use Change Application. must be on file prior to any approvals.
3. A license agreement MUST be signed and recorded prior to canstruction of any SID
facilities, orwithin its easements.
4. Any changes to the existing irrigation system such as relocation, water delivery, tiling,
and: landscaping must be approved by Settlers Irrigation District's Board of Directors..
5. All storm drainage must be retained on-site.
ti, All irrigation ponds combined shall not exceed a certain amount of stored water equal to
the allocated water right, of this property, over a 24 hour period.
7. A pressure irrigation system must be provided to service all lots within the above-
mentioned subdivision from the current surface irrigation water delivery point.

Re: FP12-004 Final Plat for the Zebulon Heights No. 5 Subdivision
The Idaho Transportation Department has reviewed the referenced final plat application for the
Zebulon Heights No. 5 Subdivision southwest of the SH-55/McMillan Road intersection.
ITD has the following comments:
1) ITD has no objection to the requested final plat. The project does not generate any more
trips than the original application and the overall project has already improved the
Wainwright intersection with SH-55 (Eagle Road).
2) ITD is constructing raised medians in Eagle Road. The new medians will not restrict the
SH-55/Wainwright intersection.

Nampa & Meridian Irrigation District has no comment on the above-referenced application as
it lies outside of our district boundaries. Please contact Nathan Draper with Settlers Irrigation
at 344-2471 or at PO Box 7571, Boise, ID 83707.
All laterals and waste ways must be protected. All municipal surface drainage must be
retained on-site. If any surface drainage leaves the site, Nampa & Meridian Irrigation District
must review drainage plans. The Developer must comply with Idaho Code 31-3805.
