Roger Llewellyn (Golden West Advertising) on behalf of Hopkins Financial, submitted a Revision to the Planned
Sign Program Application for Fairview Lakes on 4/13/04. This letter outlines my review and approval notes of the
Planned Sign Program. The revision included allowing letters on wall signs to be a maximum of 5-feet in height
which is reflected below.
The number and types of approved signage are as follows:
Center /Commercial Signs (the 21arge tenant signs on Fairview Ave.)
• Total # of allowed Center/Commercial type signs = 2
• Golden West Advertising is applying for both (2) of these signs with the Planned Sign Permit and both are
approved by P&Z.
Single Building Signs (the smallersipns that identify each individual building in the protect)
• One free-standing sign is allowed per building within the center (sign may not exceed a height of 10-feet and
background area of 70 square feet).
• Golden West is applying for (2) of these signs at this time for Pad 1 & Pad 2 located at the western entrance of
the subdivision. These are approved at this time.
• The rest of the individual free-standing signs will require separate sign permit applications.
• Each of the future signs must be in conformance with the design approved under this Planned Sign Program.
Wall Signs
• No specific Wall Signs were proposed under this application. Each of these future signs will require separate
sign permits.
• Sign shall not exceed 18% of wall area or 9% if the property also has afree-standing sign.
• Signs shall be individual letters mounted to a continuous raceway not to exceed 5-feet in height or allowed
square footage.
Sign Setbacks
The setbacks for the Center Sign(s) are approved as shown on the Site Plan. The Fairview Lakes Center Sign(s)
meets the minimum one foot setback on the front property line and five-foot setback on the side property line.

I just wanted to let you know I have approved the two monument signs located in the roundabout. However, to do
this two of the buildings had to forfeit their monument signs for their individual buildings. Per your application the
buildings located at 1830 E. Carol and 1879 N. Lakes Drive will NOT be eligible nor approved for monument
signs. I have detailed this in the PSP file for future reference and to clear up any confusion in the future.

Fairview Lakes Center Sign Standards
This criteria has been established for the mutual benefit for all tenants. Conformance will be
strictly enforced and any non-conforming or unapproved signs must be removed or brought into
conformance at expense of tenant,
A. GENERAL APPROVAL REQUIEMENTS:
1. Tenant shall submit or cause to be submitted to the landlord for approval before fabrication, at
least two copies of detailed colored drawings indicating location, layout, design and color of
the proposed signs, including all lettering and/ or graphics with complete specifications and
dimensions drawn to scale.
2, All permits for Tenants' signs and their installation shall be obtained by the Tenant or his
representative and shall conform to all local building and electrical codes as well as the
criteria set forth hereafter.
3. All signs shall be designed, constructed and installed at Tenant's expense.
4. Tenant shall be required to fulfill all the requirements of these criteria.
B. NU1viBER, LOCATION, DESIGN AND CONSTRUCTION OF SIGNS:
1. Tenant shall be permitted a maximum square footage calculated as 9% of that elevations wall
area. VVaI[ area shall be calculated at the height times the length of that elevation.
2. One tenant space on Pylon sign shall be permitted if available. In addition one pad
identification sign may be allowed per building if available.
3. All Tenant's identification designs, logos, copy and shapes shall require landlord's approval.
4. No exposed conduit, brackets, fasteners or wiring of any type will be permitted.
5. Exterior signs shall pan channel letters mounted on wall and not exceed allowed square
footage.

amp load and UL sticker in legible view from ground,
6. Total sign length shall not exceed 80% of tenant's linear feet of leased frontage.
7. Property described as Fairview Lakes' subdivision Lot 3 flock 2 shall be removed fmm this
plan program and shall not be advertised on Fairview Lakes' Pylon suns.
C. l~i1SCELANEOUS REQUIREMENTS:
1. Tenant shall be responsible for the operations of Tenants sign contractors. Tenant and sign
contractor will be liable for any actions cotzunitted during installation or sen~ice of signage.
2. Tenant oc his representative shall be responsible for cost of maintenance, service, repair or
replacement of Tenant's sign, time clock or electrical wiring.
b. Tenant shall be required to keep his sign in working order and all surfaces shall be
maintained in good condition. Tenant shall also be responsible for any repair or painting for

(Proposed revisions in italics)
SECTION 5.29 Signs.
(a) Except as provided insub-section (b) hereof, no free-standing pylon signs or
monument signs shall be allowed with the exception of the pylon signs and
monument signs to be constructed/installed on the lots which abut Fairview
Avenue (U.S. Highway 30), unless otherwise approved by the Grantor or the
ACC. The location, design and size of the pylon signs and the monument
signs within the Property shall be approved by the ACC. The costs for the
maintenance, operation, repair and replacement of the pylon signs and the
monument signs which identify the Property or specific Portions of the
Property shall be paid by the Association, and included in the Regular
Assessments levied on the Lots benefitted thereby, as determined by the Board
of the Association.
Except as provided in sub-section (b) hereof, there shall be no signs, except
directional signs and signs on Buildings, in the Property. All exterior
Building signs on Lots shall be restricted to identification of the businesses or
services located or provided thereon. No exterior Building sign shall be
placed on penthouse walls, extend above the Building roof or be painted on
the exterior building surface. No exterior building or free-standing sign
(except a pylon sign located on a Lot abutting Fairview Avenue) shall utilize
flashing, moving or audible lights or appurtenances. No commercial billboard
or advertising shall be displayed to the public view on or form any Lot. One
(1) sign not exceeding four square feet (4 sq. ft.) in size advertising the name
of the builder and one (1) sign not exceeding four square feet (4 sq. ft.) in size
advertising the name of the institution providing financing therefore maybe
displayed on a Lot during construction of the Improvements. Directional
signs may be used to give directions to traffic or pedestrians or give special
instructions. Any directional or identification sign within the Property shall
be permitted, provided the same is approved by the ACC prior to installation.
Notwithstanding the foregoing, the ACC shall have the right to adopt ACC
Design Standards with respect to signs allowed within the Property, which
ACC Design Standards, if adopted, shall regulate signs within the Property
and shall control over the specific provisions of this Section.
(b) Lot 1, Block 3 of Devon Park No. 1, ("Banner Bank Lot") shall be
permitted to have two free standing signs thereon, the location, size and

design of which have been approved by the Grantor, and such additional
sign(s) located on said Lot as are approved by Grantor or the ACC prior to
installation. Any proposed modification of the location, size or design of
either or both free standing signs shall be subject to ACC approval prior to
modification. The owner and/or occupant of the Banner Bank Lot shall not
be permitted any sign which is located outside the boundaries of the Banner
Bank Lot, including, but not limited to, the use of any portion of any
monument or pylon sign serving the Property which is located outside the
boundaries of the Banner Bank Site.

Subject: Banner Bank sign at Fairview Lakes on Fairview
Banner Bank did not receive approval for their sign that is located at the entrance (N. Lakes Ave.) into Fairview
Lakes and the copy area (panel) on the sign must be removed. This sign was originally shown on the site plan
submitted with the Planned Sign Program for Fairview Lakes as a subdivision identification sign although details
were never submitted for approval as such. Banner Bank did receive approval for their other electronic
readerboard sign located on Fairview; however, only 1 sign is allowed per building, so the sign at the entranceway
needs to removed.
Doug--If Fairview Lakes wishes to use this as a subdivision identification sign, a sign permit will need to be applied
for.

The DMV sign permit application calls for a cabinet style sign. This sign does
not follow the exact specification of the Planned Sign Program for Fairview
Lakes. Thus this sign was presented to be reviewed by the Developer, Board of
Directors, and ACC of Fairview Lakes and has been approved.
This is an exception to our plan and will only be allowed on a case-by-case
basis. If you have any questions or concerns please let me know,

(R~se Revised Sign Program for Fairview Lakes
Roger Llewellyn (Golden West Advertising) on behalf of Hopkins Financial; submitted a Revision to the Planned
Sign Program Application for Fairview Lakes on 4/13/04. This letter outlines my review and approval notes of the
Planned Sign Program. The revision incuded aNowing letters on wall signs to be a maximum of 5-feet in height
which is reflected below.
The number and types of approved signage areas follows:

• Total # of allowed Center/Commeroial type signs = 2
• Golden West Advertising is applying for both (2) of these signs with the Planned Sign Permit and both are
approved by P8Z.

• One free-standing sin is allowed per building within the center (sign may not exceed a height of 10-feet and
background area of 70 square feet).
• Golden West is applying for (2) of these signs at this time for Pad 1 & Pfd 2 located at the western entrance of
the subdivision. These are approved at this time.
• The rest of the individual free-standing signs will require separate sign permit applications.
• t=ech of the future signs must be in conformance with the design approved under this Planned Sign Program.
Wins
• No specific Wall Signs were proposed under this application. Each of these future signs will require separate
sign permits.
• Sign shall not exceed 1896 of wall area or 996 if the property also has afree-standing sign.
• Signs shall be individual letters mounted to a continuous rac~nray not to exceed 5-feet in height or allowed
square footage.
Sion Setbacks
The setbacks for the Center Sign(s) are approved as shown on the Site Plan. The Fairview Lakes Center Sign(s)
meets the minimum one foot setback on the front property line and five-foot setback on the side property fine.

Re: DMV sign and Fairview Lakes Planned Sign Program
Dear Kristi,
The DMV sign permit application calls for a cabinet style sign. This sign does
not follow the exact specification of the Planned Sinn Program for Fairview
Lakes. Thus this sign was presented to be reviewed by the Developer, Board of
Directors, and ACC of Fairview Lakes and has been approved.
This is an exception to our plan and will only be allowed on a case-by-case
basis. If you have any questions or concerns please let me know,
