DESCRIPTION OF APPLICANT'S REQUEST
The applicant, Gregory Kent, requests certificate of zoning compliance (CZC) and administrative design
review (DES) approval of the following:
1) renovate the east facade of the existing 12,753 square foot office building. The exterior alteration
includes the enclosure of an existing garage door and man door with stucco siding to match the existing
siding material.
2) change the use of the property from office and showroom to health and social services. Per UDC Table
11-2B-2, a health and social service facility is a principally permitted use in the L-O zoning district.
The subject property is located at 2620 S. Eagle Road.
DECISION
The applicant's request for CZC and DES is approved with the conditions listed in this report.
Note: This is not a building permit. Please contact Building Services at (208) 887-2211 to verify if you
need a building permit and/or inspection. If you do need a building permit, you must complete that
process before you commence the use or construction. As part of the application submittal, you will
need to provide two sets of the final, stamped "approved" Planning Department plans. You must
provide these plans in PDFjormat and hardcopy as specified in the Building Services "Plan Intake
Checklist" and include them with your building submittal set. Please contact Building Services for
additional details about building permits and inspections.
Process Conditions of Approval
1. No signs are approved with this application. Prior to installing any signs on the property, the
applicant shall submit a sign permit application consistent with the standards in UDC Chapter 3
Article D and receive approval for such signs.
2. The exterior building alteration proposed by Larson Architects dated October 24, 2011 is
approved (stamped "approved" on November 2, 2011 by the City of Meridian Planning
Department) with no changes.
3. No exterior site and landscape alterations are approved with the subject application unless prior
written approval is obtained from City of Meridian Planning Department.
4. The applicant shall pay any applicable impact fees prior to the issuance of a building permit.
5. The applicant shall complete all required improvements prior to issuance of a Certificate of
Occupancy. It is unlawful to use or occupy any building or structure until the Building Official
has issued a Certificate of Occupancy.

Ongoing Conditions of Approval
1. The applicant and/or assigns shall have the continuing obligation to provide irrigation that meets
the standards as set forth in UDC 11-3B-6 and to install and maintain all landscaping as set forth
in UDC 11-3B-5, UDC 11-3B-13 and UDC 11-3B-14.
2. The project is subject to all current City of Meridian ordinances and previous conditions of
approval associated with this site (CZC-04-069).
3. The issuance of this CZC does not release the applicant from any previous requirements of the
other permits issued for the site.
4. The applicant and/or property owner shall have an ongoing obligation to prune all trees to a
minimum height of six feet above the ground or sidewalk surface to afford greater visibility of the
area.
5. The applicant has a continuing obligation to comply with the outdoor lighting provisions as set
forth in UDC 11-3A-11.
6. The applicant and/or property owner shall have an ongoing obligation to maintain all landscaping
and constructed features within the clear vision triangle consistent with the standards in UDC 11-
3A-3.
CITY COUNCIL REVIEW
The applicant or a party of record may request City Council review of a decision of the Director. All
requests for review shall be filed in writing with the Planning Department on or before November 17,
2011, within fifteen (15) days after the written decision is issued, and contain the information listed in
UDC 11-SA-6B.
If City Council review of the decision is not requested, the action of the Director represents a final
decision on a land use application. You have the right to request a regulatory taking analysis under Idaho
Code 67-8003.
EXPIItATION
Certificates of Zoning Compliance issued in conjunction with a proposed use shall expire if said use has
not commenced within one year of the date of issuance of the Certificate of Zoning Compliance.
Certificates of Zoning Compliance issued in conjunction with construction or alteration of a structure
shall expire if said construction or alteration has not commenced within one year of the date of issuance
of the Certificate of Zoning Compliance.
In accord with the above provisions, the subject Certificate of Zoning Compliance is valid until
November 2, 2012.

CZC Project Narrative
October 27, 2011
This project is a tenant improvement to the existing Sight and Sound by design building.
2620 S. Eagle Road.
The new building Owner and occupant is Dr. Gregory J Kent who will operate "The Eye
Associates". This business is a medical office conducting general Ophthalmology and Lasik.
This use is categorized under health care and social services. The previous use of the building
was noted as office and showroom.
The existing first floor square footage is 8,601 - no change fo square footage is proposed.
There is an area of 915 sq. ft in the north east corner of the building which will remain for future
use. This use will remain as existing (office) to avoid having additional ACRD impact fees
imposed.
The 4,152 sq. ft basement is currently noted as storage use. This use will remain. No changes
are proposed.
Exterior modifications will be limited. The only changes proposed are infilling an existing
overhead door and a man door on the east side. See attached drawing sheet CZC-DR.
The infill will match the adjacent wall; Stucco and color to match.
The interior TI will leave approximately 50% of the existing spaces with no work being done.
There is no structural work in the building
All parking, site amenities and landscaping will remain as is. No changes are proposed.

Since we are going to be receiving a CZC application from the City, we are going to hold off on doing a
formal review of this site, in case there is more than just impact fees. You can include this a-mail with
your CZC application though as verification that you have contacted ACRD. There will bean impact fee
assessed for the change in use. The prior impact fee paid was for General Office. For the 7,664 s.f.
there will bean impact fee assessed for Medical Office, with a deduction for the prior use paid of
General Office. We will not assess impact fees on the other portions of the building, per your a-mail
below. After the City transmits the CZC application to us we will complete our review and contact you.

Hello Cristy:
Last week we submitted this TI to ACHD for a number and whatever we need so Meridian P&Z will accept
it for the CZC verification intake.
One thing I had noted on the plans but wasn't clear on the application was that 937 sq.ft of the main floor
is to remain vacant and unoccupied. The tenant wants to rent this out -hopefully as the existing use, ie
office & showroom. This shown on the attached plan.
The actual sq. footage of the ground floor which will be changed from office/showroom to medical office is
8,601-937 or 7,664 sq. ft.
The basement area will remain the same - as storage. 4,152 sf.
