Lanark Meridian, ID 83642 ~S`~li Re: Permit Application No. 3-11-135; Construction of Right Turn/Deceleration Lanes within the Highway Right of Way, Plan Approval for Construction SH55 (Eagle Road), Milepost 12.427-12.591, Sta. 80+80.40-89+18.38 Dear Mr. Johnson, This letter is to recap our meeting Tuesday, November 01, 2011. Discussion covered the construction plan approval, permit expiration and extension, and the median construction costs. Included with this letter are the Idaho Transportation Department approved construction plans. Please include these plans and make them a part of the permit document. The permit will not be complete without the inclusion of these plans or copies of these plans. The above referenced permit application was approved on November 16, 2010. This permit is to expire on November 16, 2011. Per IDAPA 39.03.42, this letter grants a onetime six (6) month extension, the new expiration date is May 16, 2012. The original permit conditions requiring the construction of a raised median has been reevaluated. The Idaho Transportation Department has determined the required median construction specified as a condition of the permit is more feasible to be constructed with our upcoming project. As we have discussed a formal agreement is to be drafted by your staff. This agreement is to define the applicants' burden of cost for the required median within the SH55 frontage. When the Idaho Transportation Departments project is awarded for construction, the contract bid pricing is to be used to determine the cost for this work. At that time a check is to be remitted, payable to the Idaho Transportation Department, for the construction cost of the permit required median construction. This agreement shall be made a condition of the permit and be fully executed prior to the final acceptance of the permit work (3-11-135). Please take note that at all times when operations are ongoing that a complete copy of the approved permit is required to be on site. Please see the General Provisions item number fifteen (15).
