Landscaping_ The Landscape Plan (Sheet L-1.0) is approved per the plan prepared by South
Landscape Architecture, PC, on 2-4-OS and stamped "approved" on 3-16-OS by the Meridian
Planning & Zoning Department. The approved landscape plan is not to be altered without prior
written approval of the Planning & Zoning Department. No field changes to the landscape plan

Irri ag tion: An underground, pressurized irrigation system must be installed to all landscape areas per

Curbing may be cut to allow for storm water runoff.
Parkin: All parking stalls shall be as depicted on the site plan. The project engineer/architect
shall certify that the number and size ofhandicap-accessible spaces conforms to the Americans
with Disabilities Act (ADA). Handicap accessible stalls must have signage in accordance per ADA.
The applicant shall be responsible for meeting the requirements of ACHD as they pertain to this
development.
Sidewalks: All sidewalks shall be constructed as submitted and in accordance with MCC 12-5-2.K.
Sidewalks shall be constructed prior to occupancy.
Drainage: Storm water drainage swales shall not have a slope steeper than 3:1, shall be fully
vegetated, and shall be designed in compliance with the MCC.
Lighting: Lighting shall not cause glare or impact the traveling public or neighboring
development. Down-shield all lighting so it does not spill onto adjacent properties.
Signage: No signs are approved with this CZC. All signs require a separate sign permit in
compliance with the sign ordinance.
Trash Enclosure: Any dumpster(s) must be screened in accordance with MCC 11-12-1.C. Trash
enclosures must be built in the location and to the size approved by SSC.
Handicap-Accessibility: The structure, site improvements and parking must be in compliance
with all federal handicap-accessibility requirements.
ACHD Acceptance: All impact fees, if any, shall be paid prior to the issuance of a building permit.
If any changes must be made to the site plan to accommodate the ACRD requirements, a new site
plan shall be submitted to the City of Meridian Planning and Zoning staff for approval prior to the
issuance of a building permit.
Certificate of Occupancy: All required improvements must be complete prior to obtaining a
Certificate of Occupancy. A Temporary Certificate of Occupancy may be obtained by providing
surety to the City in the form of a Letter of Credit or cash in the amount of 110% of the cost of the
remaining improvements. A bid must accompany any request for Temporary Occupancy.
Plan Modifications: The approved "CZC Compliance Site Plan", stamped "approved" on 3-16-05, is
not to be altered without prior written approval of the Planning and Zoning Department. No
significant field changes to the site or landscape plans are permitted; prior written approval of all
changes is required.

To Whom It May Concern:
Attached, please find a completed Certificate of Zoning Compliance and
supporting documentation as required by the Meridian Planning and
Zoning Ordinance. The proposed project, an approximately 11,000 square
foot medical office building will be located within the Meadowlake
Village / Touchmark development on an existing individual parcel
adjacent to the proposed Meadowlake PUD. A lot-line adjustment hs been
submitted to Planning and Zoning for review and approval.
The new building will provide offices and clinical facilities for OB/GYN
Associates with convenient pedestrian access to St. Luke's Meridian
Medical Center. The master plan for the development is compatible with
this proposed project and complimentary in use to adjacent facilities such
as the Touchmark office building, and the Spine Institute of Idaho.
The Touchmark Design Standards Committee approved the project, with
specific provisions such as approval from Meridian Planning and Zoning.
Both Meridian Planning and Zoning requirements and the Touchmark
Co~ru-nercial D2Jign Standards have been utilized to complete uiis
submittal.
The project will be phased to include a complete building shell and partial
interior build-out (1,500 square feet of area shall remain vacant for future
growth).
Parking areas have been separated for employees and visitors to bring the
building closer to East Louise Drive. The entrance to the building is sited
and aligned to the street, while the employee parking area and entrance is
located to the rear of the building. Rain and storm run-off will be
maintained on-site through the use of sub-surface absorption beds located
under the parking lots.

The Design Standards Committee convened on December 6 to review the referenced project for adherence to
the Meadow Lake Village Commercial Design Standards dated January 2004.
The submitted project is APPROVED WITH THE FOLLOWING CONDITIONS:
3.5 Erosion Prevention During Construction
1. Coordinate removal of the berm on the west side of the development with Meadow Lake
Village (MLV).
2. Coordinate construction traffic on private and public streets with Authorities Having
Jurisdiction (AHJ) and MLV.
3.6 Landscaping
1. Provide bermed parking screening on Louise Drive per 3.6.2 #4.
3.7 Buffers and Fences
1. Submit trash enclosure elevations indicating materials and colors.
2. Screen any ground or roof mounted equipment per 3.7.2 #2.
3.8 Setbacks

All landscape areas regulated by the City Landscape Ordinance (12-13) shall be served with an
automatic underground irrigation system. Additional requirements affecting pressurized
irrigation systems can be found in City Ordinance 9-1-28.
Tn-igation Water Srn~r e ,
Use of non-potable irrigation water is required when determined to be available by the City
Public Works Department as regulated by City Ordinance 9-1-28. If city potable water is used, a
separate water meter is recommended so the owner can avoid paying sewer fees for irrigation
water. Potable water shall not be used as a primary irrigation water source on non-residential
lots with more than '/2 acre of landscaping. Year round water availability is also required by
connecting to city potable water or an on-site well as a secondary source.
