ACRD Acceptance: Applicant shall be responsible for meeting the requirements of ACHD as they pertain to
this development. All impact fees, if any, shall be paid prior to the issuance of a building permit. If any
changes must be made to the site plan to accommodate the ACHD requirements, a new site plan shall
be submitted to the City of Meridian Planning and Zoning Staff for approval prior to the issuance of a
building permit.
Certificate of Occupancy: All required improvements must be complete prior to obtaining a
Certificate of Occupancy. A Temporary Certificate of Occupancy may be obtained by
Providing surety to the City in the form of a Letter of Credit or cash in the amount of 110%
of the cost of the remaining improvements (including paving, striping, landscaping, and/or
irrigation). A bid must accompany any request for Temporary Occupancy.

This CZC Application is for a new Ambulatory Care Center (approximately 6,800 sf.) and two story office
building (approximately 31,000 sf). The building and site has been through the C.U.P. process and has
been submitted to DEQ for approvals. The parking lots, landscaping and site lighting have been developed
along the guidelines of the C.U.P. Approval. The plans submitted are the full set of landscape and civil
drawings and reflect the entire scope of the project.
The building is composed of a rock finish on the 1st story and an E.I.F.S. finish on the 2nd story. Metal
roofing is used on the upper mansard areas. This scheme has been reviewed by Shari Stiles at Meridian
Planning and Zoning and a neighborhood meeting was held on February 8th, 2002 to review plans with the
surrounding community.

ambulatory care/medical office building portion. The bank parcel,
in no way, shall jeopardize the ability to proceed with the balance
of the development.
b. The construction materials for the proposed bank must include
metal roofing and either brick or brick veneer. The material color
palette presented are conceptual at this time. Applicant has
freedom to modify colors in the future, subject to staffs review.
C. All required parking stalls must be located within the boundaries of
the future bank parcel. As proposed, the row of nine (9) stalls
along the east boundary of the bank parcel have no access to the
bank. A pedestrian crossing through the landscape planter must be
constructed to connect this row of parking with the bank if these
stalls are required as part of the future bank. Applicant has
updated the Master Site Development Plan providing an access
point through the landscape berm along the east property line of
the bank parcel to allow more direct access from these (9) parking
stalls to the bank.
12.11 The 25 -foot perimeter landscaping buffer on Ten Mile Road and the 35 -
foot buffer on Cherry Lane abutting the proposed bank pad must be
wanted as apart of the medical center hase and either be completed or
bonded for completion prior to Certificate of Occupancy on the medical
center buildings.
12.12 The proposed boulevard monument sign in the main entry landscape
median must be constructed outside the clear vision triangle and should
be setback a minimum of five (5) feet from the property line.
12.13 As a PUD, a minimum of 10% of the gross land area must be either open
space or landscaped area. The Site Plan shows a total of 111,397 s.f. of
landscaped area on the site, which exceeds the City's minimum area
requirement.
12.14 Off-street parking shall be provided in accordance with Section 11-13 of
the City of Meridian Zoning and Development Ordinance and/or as
detailed in site-specific requirements.

It was a pleasure meeting with you and Bruce on Tuesday morning to go over a few Change -of -Scope issues
pertaining to the Meridian site of the SaintAlphoneus Ambulatory Cam Center.As per our discussion during ttw
meeting, the folowing changes will be made to the she civil and landscape plans before the package is submitted for
O.E.Q. and permitting. First of all, aince Washington Mutual has chosen not to build on the comer, that parcel will
remain undeveloped In anticipation of a new buyer. Also, on the Saint Alftrisus portion, the two right-in/right-out
�--� curb cuts will be de!eW from this project for this first phase but will be Included in the second phase when the
medical facility Is expanded to the west or when a buyer is found for the comer parcel. A new en-eits circulation
drive will extend north- uth at the western must edge of the site development and include a gravel drive for access
to a manhole adjacent to the wuth boundary. The underground utilities will still stub -out at the oomer parcel as
previously planned, but as already stated, no paved access will be providod, The landsme buffer will remain sing

The District has reviewed the plans for the above referenced project and they are accepted for public street
construction. lay stamping and signing the improvement plans, the Registered Engineer ensures the District that the
Plans conform to all District policies and standards. Varianccs or waivers must be specifically and previously
approved by the District in writing. Acceptance of the improvement plans by the District does not relieve the
