COMMENTS:
Landscapitl~: All existing landscaping shall be retained on site and shall be continuously maintained.
Lighting: All lighting shall bedown-shielded, and shall not adversely impact the surrounding
properties and right of way. No lighting shall be allowed above 35 feet in height unless
required by the FCC or FAA.
Maintenance: The painted pole shall be continuously maintained in a fully painted manner. All areas
of flaking or discoloration shall be immediately re-painted.
Condition Issuance: Issuance of this permit does not modify any of the conditions of approval as
listed in the original conditional use permit issued for this project. All landscaping, fencing
and building setbacks shall be required as submitted.
Plan Modifications: The approved Site Plan and Landscape Plan are not to be altered without prior
written approval of the Planning & Zoning Department. No field changes to the site or
landscape plans are permitted; prior written approval of all material changes is
required.
The Site Plan & Landscape Plan s~tam~ed 05/30/01 is approved with the comments noted above.

Proposed location: 1776 N. Avest Lane, Meridian, Idaho
Dear Ms. Stiles:
Cricket Communications hereby presents its application for a Certificate of
Zoning Compliance for a proposed Personal Wireless Facility ("PWF")within the
municipal boundaries of the City of Meridian.
Cricket Communications, LLC requests to locate a PWF stealthed as a light pole
at 1776 N. Avest Lane, along with associated ground equipment ("Facility").
The proposed Facility is in accordance with the general policy of the City of
Meridian to locate wireless facilities in commercial zones. The stealthed light
pole will be located within an existing Stor-It ministorage project located in the
area of Fairview Ave. and N. Eagle Road. The Facility itself will be in the middle
of the project, between an existing mini-storage building and a future mini-
storage building. The light pole will be 85' in height. A stealthed light pole will be
in character with the commercial nature of the area. It will be colored graphite
similar to other light poles in the vicinity. The light will face the sidewalk to the
south (which faces the strip mall) at the 35' level of the pole.
Cricket will comply with, and report feedback from, any federal governing
authority as it applies to FAA and FCC regulations. No public costs will be
incurred for facilities or services.
The method of ingress/egress had been included in the site plan drawings.
There are no proposed changes to storm drainage, sidewalks or exterior lighting
as part of this application. There will be no need for refuse disposal or water
services.

1. Completed and signed CZC application form;
2. Last Deed of Record for the subject property, a Grant Deed vesting
title from Roger and Virginia Allen to Avest Limited Partnership;
3. Notarized Affidavit of Legal Interest;
4. Three (3) sets of the Zoning drawings, which include the site plan, a
legal description of the parcel and the leased premises, and elevations
of the proposed stealthed light pole;
5. Two color examples of other PWF's stealthed light poles.
6. Requirements for landscape plan (requirement 5) shall be as the CUP
that was previously approved when the Store It building was approved
(the dead tree will be replaced with like kind healthy tree).
7. A written approval from a.Sanitation Service Company (requirement 6),
a calculations table (requirement 7) and a copy of an ACHD letter
(requirement 9) are not applicable to this type of project application.
The Store It building is planning to expand there building on to what is now is
vacant land to the west sometime later this year. For this reason we will screen
our ground equipment with the like construction of the Store It building materials
so it will blend into the appearance of the existing building.
