Conditions ofApproval :Project is subject to all current Meridian City Code (MCC) and conditions
of approval for the El Dorado Business Campus/Bonito Subdivision. The issuance ofthis permit does
not release the applicant from any requirements of the approved Annexation and Rezone (AZ-O1-
O l 8), Conditional Use Permit (CUP-01-037), Preliminary Plat (PP-01-020) and Final Plat (FP03y010)
permits issued for this site. MI-04-006 modified the list of permitted uses to allow skilled nursing
facilities.
Building Height: Per condition #2 of CUP-01-03 7, the maximum building height for this lot is 35 feet,
measured to top of roof.
Landscaping The landscape plan prepared by The Land Group, Inc., labeled L-1, dated 8-19-04, and
stamped "approved" on 8-31-04 by the Meridian Planning and Zoning Department, is approved with
the following. changes:
a) Add 1 tree per 35 lineal feet along the west boundary where adjacent to asphalt.
b) Add 1 tree per 35 lineal feet along the north boundary where adjacent to asphalt.
The landscape plan is not to be altered without prior written approval of the Planning and Zoning
Department. No field changes to the landscape plan permitted; prior written approval of all material
changes is required.
Site Plan: The site plan prepared by Larson Architects, P.A., Sheet SP-1, dated 8-18-04, and
stamped "approved" on 8-31-04 by the Meridian Planning and Zoning Department, is approved with
the following changes:
a) Correct the scale from 1" = 30' to 1" = 20'.
b) Widen the Ridenbaugh Pathway on south boundary to a minimum of 10 feet and extend
the pathway to the west property line, as shown by the redline.
c) Per Meridian Fire Department, `mTo Parking" signage shall be installed at the east planter
where the drive aisle narrows below 25 feet to a fire lane.
The approved site plan is not to be altered without prior written approval of the Flanning and Zoning
Department.
Ridenbaugh. Pathway: Per 'the attached recorded easement (Exhibit A, W.H. Moore Co.), the
Ridenbaugh pathway on the south property line must stub to the west property line to allow for future
extension of the pathway system See redline notes.
Jogg~g; Pathway; The internal jogging path shown on the west property line is a required amenity of
the CUP. A minimum 5-foot wide pedestrian easement in favor of the El Dorado Business Owner's
Association (or similar entity) shall be recorded for the pathway and submitted to the P&Z
Department prior to issuance of a Certificate of Occupancy.
Irnrrigation: An underground, pressurized irrigation system must be installed to all landscape azeas per
the approved specifications and in accordance with MCC 12-13-8 and MCC 9-1-28.
Curbing: Per MCC 1Z-13-7-9, all landscape azeas adjacent to driveways, parking lots, or other
vehicle use areas, must be protected by curbing, wheel stops, or other approved protective devices.
Curbing may be cut to allow for storm water runoff.
Protection of Existing Trees: Any existing trees on site must be protected in accordance with the
Tree Preservation section ofthe City's Landscape Ordinance. Per MCC 12-13-13-4, coordinate with
the Parks Department Arborist (Elroy Hu1fj for approval of protection measures prior to
construction. Any severely damaged tree must be replaced in compliance with MCC 12-13-13-6.
P_arkina: The project engineer/azchitect shall certify that the number and size ofhandicap-accessible
spaces conforms to the Americans with Disabilities Act (ADA). Handicap accessible stalls must have
signage in accordance per ADA. The applicant shall be responsible for meeting the requirements of
ACRD as they pertain to this development
Sidewalks: All sidewalks shall be constructed as submitted and in accordance with MCC 12-5-2.K:
Sidewalks shall be constructed prior to occupancy.
Drainage: Storm water drainage swales shall not have a slope steeper. than 3:1, shall. be fully
vegetated, and shall be designed in compliance with the MCC.
tin :Lighting shall not cause glare or impact the traveling public or neighboring development.
Si Wage: No signs aze approved with this CZC. All signs will require a separate sign permit in
compliance with the sign ordinance.
Trash Enclosure: Any dumpster(s) must be screened in accordance with MCC 11-12-1.C. Trash
enclosures must be built in the location and to the size approved by SSC. NOTE: A portion of the
proposed trash enclosure is located off-site. Said trash enclosure shall be shared by Lots 1, 2 and 3,
Block 3.
Handicap-Accessibility: The structure, site improvements and parking must be incompliance with all
federal handicap-accessibility requirements.
ACHD Acceptance: All impact fees, if any, shall be paid prior to the issuance of a building permit. If
any changes must be made to the site plan to accommodate the ACHD requirements, a new site plan
shall be submitted to the City of Meridian Planning and Zoning stafffor approval prior to the issuance
of a building permit.

Certificate of Occupancy: All required improvements must be complete prior to obtaining a
Certificate of Occupancy. A Temporary Certificate of Occupancy may be obtained by providing
surety to the City in the form of a Letter of Credit or cash in the amount of 110% of the cost of the
remaining improvements. A bid must accompany any request for Temporary Occupancy.
Plan Modifications: The approved Site Plan and Landscape Plan, stamped "approved" on 7-26-04
are not to be altered without prior written approval of the Planning and Zoning Department. No
significant field changes to the site or landscape plans are permitted; prior written approval of all
changes is required.

Ree Ridenbaugh Pathway Construction
A fief visit to EI Dorado Sub was hey today with Jonathan Seel iW.H. Moore Co.),
Doug Strong (Meridian Parks) and myself. The purpose was to review the alignment
of the Ri~nbaugh Canal pathway, an amenity n~uired by the CUP and Final Plat.
There is a depth limitation at the eastern end of the pathway due to high
groundwater discovered at the toe of the slope last month, which limits the pathway
construction behind Lots 1 and 2, Block 1. Until it is determined what the source aril
corrective measures are for the high groundwater, we agreed it wou~ be the best
course of action to delay pathway cwnsbudion on these iwo lots. John Anderson of
Nampa Meridian Irrigation f7istrict (NMID) has said his preference is to have no
more digging in this area until further investigation can be done. Another option (not
preferred) is to re-align the pathway near the ex pump house and connect it to
the sidewalk on the south side of E. Copperpoint Drive and continue to the east.
This would avoid pathway oorrstrtrttion in the high groundwater area but would not
be the best alignment for the public. tt would also require a mod cation to the
recorded pathway easement.
At the end of the 2004 irrigation season (approx. October 15) when Nampa Meridian
Irrigation District can better determine the souro8 of the problem, this issue should
be r8-visited arul a final decision made on tt~e timing of the pathway construction.

Attached, please find attached a Certificate of Zoning Compliance application for the Advance
Health Care Building. This project is located on Lots 6 & 7, Block 1, of the Bonito subdivision,
El Dorado Business Campus, located on the corner of Overland and Eagle Road. The project is
to be used as Skilled Nursing Facility, based on the uses approved under the development
agreement. The proposed building is 25,041 square feet.
