Landscaping_ The Landscape Plan prepared by Insight Architects on 6/29/09, labeled Sheet L1.0, is approved
(stamped "Approved" on 7/1/09 by~the Meridian Planning Department) with no changes.
The approved landscape plan is not to be altered without prior written approval of the Planning Department. No field
changes to landscape plan permitted; prior written approval of all material changes is required. Prior to final
inspection and sign off far this project, a written certificate of completion shall be submitted to the Planning
Department, prepared by a landscape architect, landscape designer or qualified nurseryman responsible for
the landscape plan upon completion of the landscape installation. The Certificate of Completion shall verify
that all landscape improvements, including plant materials and sprinkler installation, are in substantial
compliance with the approved landscape plan.
Site Plan: The Site Plan prepared by Insight Architects on 6/12/09, revised on 6/29/09, labeled Sheet A0.1, is
approved (stamped "Approved" on 7/1/09 by the Meridian Planning Department} with the following modification:
The 6' wide pedestrian pathway shown on the site plan at the northeast corner of the site that crosses
the parking lot and connects to E. Gala Street needs to be distinguished from the vehicular driving
surface through the use of pavers, colored or scored (stamped or patterned) concrete, or bricks, in
accordance with UDC 11=3A-19C.4b. The concrete walk shown on the site plan scored to match
sidewalks on site is not approved.
The approved site plan is not to be altered without prior written approval of the Planning Department.
Elevations: Elevations prepared by Insight Architects on 6/12/09, labeled Sheets A3.0 and A3.1, are approved with
no changes from the Planning Department.
Irrigation: An underground, pressurized irrigation system must be installed to all landscape areas per the approved
specifications and in accordance with UDC 11-3A-15.
Protection of Existin Trees: Any existing trees on site must be protected or mitigated for in accordance with the
Tree Preservation section of the City's Landscape Ordinance.' Per UDC 11-3B-10, coordinate with the Parks
Department Arborist (Elroy Huff, 888-3579) for approval of protection/relocation measures for the existing trees
prior to construction. Any severely damaged tree must be replaced in compliance with UDC 11-3B-10-C.5.
Parking: The proposed parking areas shall be paved and striped in accordance with UDC 11-3C and 11-3B. Project
engineer/architect shall certify that the number and size ofhandicap-accessible spaces conforms to the Americans
with Disabilities Act (ADA). Handicap accessible stalls must have signage in accordance per ADA and signed
accessible.
Cam: Per UDC 11-3B-SI, all landscape areas adjacent to driveways, parking lots, or other vehicle use areas,
must be protected by curbing, wheel stops, or other approved protective devices. Curbing maybe cut to allow for
storm water runoff.
Sidewalks: All sidewalks shall be constructed in accordance with 11-3A-17. Sidewalks shall be constructed prior to
occupancy.
Dralna~e: Storm water drainage swales shall not have a slope steeper than 3:1, shall be fully vegetated, and shall be
designed in compliance with UDC 11-3B-11 and UDC 11-3A-18.
LiLi~hting: Lighting shall not cause glare or impact the traveling public or neighboring development and
comply with lighting standards as defined in UDC 11-3A-11.
Si riag~e: No signs are approved with this CZC. All business signs will require a separate sign permit in
compliance with UDC 11-3D.
Trash Enclosure: All dumpster(s) must be screened in accordance with UDC-11-3A-12. Trash enclosures must be
built in the location and to the size approved by SSC.
Handicap-Accessibility: The structure, site improvements and parking areas must be incompliance with all federal
handicap-accessibility requirements.
ACHD Acceptance: All impact fees, if any, shall be paid prior to the issuance of a building permit. If any changes
must be made to the site plan to accommodate the ACHD requirements, a new site plan shall be submitted to the
City of Meridian Planning staff for approval prior to the issuance of a building permit.
Certificate of OccupancX: All required improvements must be complete prior to obtaining a Certificate of
Occupancy. All changes in occupancy need to comply with the requirements of the Building Department. It is
unlawful to use or occupy any building or structure until the Building Official has issued a certificate of occupancy
A certificate of occupancy or temporary certificate of occupancy is obtained from the Building Department (208)
887-2211 after inspections are complete and the field inspection record is returned to the Building Department.
Plan Modifications: Except for the changes mentioned above, the approved Site Plan, Landscape Plan, and
Elevations, stamped "Approved" on 7/1/09, are not to be altered without prior written approval of the Planning
Department. No significant field changes to the site or landscape plans are permitted; prior written approval of all
changes is required.
Note: If the plat for the subdivision in which this property lies has not been recorded, all subdivision
improvements shall be installed prior to occupancy of the first structure in the subdivision.

Located in the Gaudry Seegmiller Subdivision (Lot 3)
To Whom It May Concern:
This letter is provided as a narrative letter for the Design Review submittal for the project listed
above.
This development consists of a single story 3,480 s.f. medical outpatient plastic surgery center.
This subdivision has been developed for outpatient medical facilities and has several adjacent
doctors, dentists, and physicians. The site currently has a paved asphalt parking lot, dumpster
enclosure, and parking lot lighting. All utilities have been stubbed in for future use on this site.
Architectural Character:
A) Facades: The facade of this building uses a combination of arches, columns, and window
openings to create architectural character. In combination with the stucco, large fascia,
roof tile, and aluminum clad windows.-
B) Primary Public Entrances: Prominent arches and columns are used at the main
entrance to the building to add architectural style that distinguished the public entrance
from the rest of the building. This makes the public entrance a focal point of the building
and inviting for pedestrians.
C) Roof Lines: A combination of a flat roof and sloped roofs are utilized in the architectural
design.. The sloped roofs and large fascia transition to the surrounding medical park
development while the flat roof adds variations in the facade. The sloped roofs also add
character to the building while supplying protection form the elements for the pedestrians.
D) Pattern Variation: Varied roof lines, walls, and massing contribute to this buildings
aesthetic design. Efforts were made during the design to articulate the building mass into
a pleasing form that identifies the public spaces.
E) Fenestration: A majority of this building utilizes an aluminum clad window openings (see
enclosed color sample of window frame color) that are paired up to create a visually
pleasing appearance. The front entry utilized arched transom windows and sidelights to
accent the front entry.
F) Mechanical Equipment: The mechanical equipment is located on the rooftop and
screened by the parapet. Though it is visible in elevation view, the mechanical equipment
would be hidden per (Section C-2.4.2.3.3) the Urban/Suburban Design Manual. No
mechanical equipment will be located on the ground under this permit.
Color and Materials:
The exterior materials used on this building will be that of high-quality. Materials include:

2) Stone Trim -Chiseled Stone Trimmers -See Sample
3) Concrete Columns -Precast concrete
4) Windows and Doors -Aluminum Clad -See Sample
5) Concrete Roof Tile -Monier Lifetile -California Mission Blend -See Sample
Site Plan and Building Layout
This site has been developed with curb, gutter, parking, and site lighting. All utilities have been
stubbed up into the development.
A) Building Location: This building is set back in the Millennium Medical Park Plaza
and is accessible from Gala Street or Millennium Way. The placement of this building
was largely dictated by the existing site conditions, however efforts were made to
promote an integrated environment between the adjacent buildings with pedestrian
walkways.
B) Parking Areas: This is an existing PAD site that has an existing paved asphalt
parking lot with 20 parking spaces dedicated to the future building. The main drive
entrance is located on South Millennium Way. There are existing parking lot light
poles located on this site. A cut sheet has been provided with this application. There
is a proposed (4) space shaded metal canopy on the east side of the building for
surgeon and staff on duty. A photo of an example is on the site plan. The canopy will
be constructed of steel and painted a light neutral color.
C) Access: Access was again largely dictated by the existing site conditions.
Interconnectivity worked nicely with other buildings within this medical .park plaza.
D) Street Layout and Internal Circulation: This parking area works well with circulation
between pedestrians and vehicles. Interconnectivity of the sidewalks line up and area
handicap accessible. The parking is located internally within the development with a
nice street presence.
E) Pedestrian Walkways and Facilities: Pedestrian walkways are being located in the
original developments master plan. A (3) three space bicycle rack is being provided
per Meridian City Code.
Landscaping:
A) There are no existing trees, shrubs or landscaping on this site except for at the
driveway entrance from Millennium Way. The required landscape buffer was placed
along the street during the development of this medical complex. There is existing
grass in this area, but no trees since they would be located in the site vision triangle.
B) A mix of Class I, Class II, and Evergreen trees will be placed on the site.
C) There is also a large group of drought tolerant ground cover plants and shrubs. No
grass has been placed in the interior of this site as to reduce the amount of water and
maintenance necessary.
D) Pressurized irrigation will be used on this site provided by the Nampa & Meridian
Irrigation District.
E) No proposed fencing will be installed under this project.

Exterior Lighting:
There are existing parking lot light poles located on this site that match all other
surrounding exterior lighting fixtures within this medical subdivision.
Exterior recessed can lighting will be used at the entry points to aid in night time access.
A cut sheet has been provided with this application.
