Conditions of Approval: Project is subject to all current City of Meridian ordinances and the conditions of
approval for the Podiatry Building Annexation (AZ-O1-010, MI-03-004).
Landscaping: The Landscape Plan prepared by C. Caraway Designs, on February 17, 2006, labeled Sheet L.1,
is approved (stamped "Approved" on September 19, 2006 by the Meridian Planning Department) with the
following changes/notes. 1) UDC 11-3B-9 requires all landscape buffers to adjoining uses to result in a
barrier that allows trees to touch at the time of tree maturity. A sufficient number of trees shall be
added to the required buffer on the southern property line to meet this requirement (see redlines). 2)
UDC 11-3B-8 requires at least one tree in all parking lot planters. One tree shall be added to each of the
two planters adjacent to the trash enclosure (see redlines). 3) Per UDC 11-3B-5 mulch shall be used in
all required planting areas. Use of mulch, organic, or rock as the only groundcover in the required
planting areas is prohibited. Required landscape areas shall be at least 70% covered with vegetation at
maturity. The use of pea gravel, drain rock or other similar products is prohibited in planting areas.
(see redlines) The approved landscape plan is not to be altered without prior written approval of the Planning
Deparhnent. No field changes to landscape plan permitted; prior written approval of all material changes is
required.
Site Plan: The Site Plan prepared by C. Caraway Designs, on February,l7, 2006, labeled Sheet C.1, is
approved (stamped "Approved" on September 19, 2006 by the Meridian Planning Department) with the
following changes/notes. 1) All parking lot planters shall be a minimum of five feet in any dimension
measured from the inside of curb (see redlines). The approved site plan is not to be altered without prior
written approval of the Planning Department.
Elevations: The Elevations prepared by C. Caraway Designs, labeled Sheet A5.1, A5.2 are approved with no
changes from the Planning Department.
Irrigation: An underground, pressurized irrigation system must be installed to all landscape areas per the
approved specifications and in accordance with UDC 11-3A-15.
Protection of Existing Trees: Any existing trees on site must be protected or mitigated for in accordance with
the Tree Preservation section of the City's Landscape Ordinance. Per UDC 11-3B-10, coordinate with the
Parks Department Arborist (Elroy Huff, 888-3579) for approval of protection/relocation measures for the
existing trees prior to construction. Any severely damaged tree must be replaced incompliance with UDC 11-
3B-10-C.5.
Parkin : A recorded copy of the proposed cross access easement to the properties east and west
of the subject lot shall be provided to the Planning Department prior to final occupancy.
Proposed parking areas shall be paved and striped in accordance with UDC 11-3B. Project engineer/architect
shall certify that the number and size of handicap-accessible spaces conforms to the Americans with
Disabilities Act (ADA). Handicap accessible stalls must have signage in accordance per ADA and signed
accessible. See redline parking changes/notes on the site plan.
Curbin :Per UDC 11-3B-SI, all landscape areas adjacent to driveways, parking lots, or other vehicle use
areas, must be protected by curbing, wheel stops, or other approved protective devices. Curbing may be cut to
allow for storm water runoff.
Sidewalks: All sidewalks shall be constructed in accordance with 11-3A-17. Sidewalks shall be constructed
prior to occupancy.
Drainage: Storm water drainage swales shall not have a slope steeper than 3:1, shall be fully vegetated, and
shall be designed in compliance with UDC 11-3B-11 and UDC 11-3A-18.
Lim: Lighting shall not cause glare or impact the traveling public or neighboring development and
comply with lighting standards as defined in UDC 11-3A-11.
Si a e: No signs are approved with this CZC. All business signs will require a separate sign permit in
compliance with UDC 11-3D.
Trash Enclosure: All dumpster(s) must be screened in accordance with UDC-11-3A-12. Trash enclosures
must be built in the location and to the size approved by SSC.
Handicap-Accessibility: The structure, site improvements and parking areas must be in compliance with all
federal handicap-accessibility requirements.
ACHD Acceptance: All impact fees, if any, shall be paid prior to the issuance of a building permit. If any
changes must be made to the site plan to accommodate the ACHD requirements, a new site plan shall be
submitted to the City of Meridian Planning staff for approval prior to the issuance of a building permit.
Certificate of Occupancy: All required improvements must be complete prior to obtaining a Certificate of
Occupancy. A written certificate of completion shall be prepared by the landscape architect, landscape designer
or qualified nurseryman responsible for the landscape plan upon completion of the landscape installation. The
Certificate of Completion shall verify that all landscape improvements, including plant materials and sprinkler
installation, are in substantial compliance with the approved landscape plan. A Temporary Certificate of
Occupancy may be obtained by providing surety to the City in the form of a Letter of Credit or cash in the
amount of 110% of the cost of the remaining improvements. A bid must accompany any request for Temporary
Occupancy.
Plan Modifications: Except for the items mentioned above, the approved Site Plan, Landscape Plan and
Elevations stamped "Approved" on September 19, 2006, are not to be altered without prior written approval of
the Planning Department. No significant field changes to the site or landscape plans are permitted; prior
written approval of all changes is required.
*This letter does not indicate compliance with requirements of other departments/agencies, including, but not limited
to, Ada County Highway District, Central District Health Department, affected, irrigation district(s), Meridian Sewer,
Water, Building or Fire Departments, Sanitary Services Co., etc. This letter shall expire one (1) year from the date
of issuance if work has not begun.

The proposed use for the 37339 SF lot will be a professional plaza. This will consist of 2 office buildings
both to be 7000 SF. The office buildings will be constructed of wood framing with stucco, and have an
asphalt compositions roof. Each of the office buildings will be core and shell for tenet improvements.
They will be equipped with bathrooms and basic utility services. The building is 21 % of the lot.
The parking lot will have 30 parking spaces of this 2 will be designated to handicap access. The parking
lot will be in the rear of the building. There will be an enclosed area_for trash. The parking lot and drive
aisle will take up 34% of the lot.
The lot will be fully landscaped and be equipped with basic utility services. This will also include
concrete sidewalks. The landscape will be 35% of the lot. The concrete sidewalk will be using 10% of the
lot.

GF~IERAL DF~CRIPTION
The luminaire consists of two main components, a ballast-
housing and an optical compartrnent, assembled as one u
for shipment
OPTICAL COMPARTMENT
The optical compartrnent consists of a thermal resistant
goros8icate glass refractor mechanically held in a pst
aluminum door frame. Light from a vertical lamp is
distributed by pn;lsely molded refracting prisms to
maximize ub'1¢afion, uniformity and luminaire sparing.
Three unique refractors are available, designed for IES tyl
I II, N and type II - 4 way d'LStribufions. Polycarbonate
refralors are available designed for three distributions. Tt
doorframe is hinged to the ballast housing and secured b!
positive action, spring loaded, self locking latch. The latch
designed to allow operation by a gkrved hand with out the
use of tools. The stainless steel hinge allows the optical
endosure to open for relamping or to be removed for
leaning.
BALLAST HOUSING
The top mounted ballast housing is die cast of 380
aluminum alloy and is threaded to accept 1-1/4NPT pipe.
The housing contains the ballast and other elelrical
components, and is separated from the lamp enclosure by
hinged and gasketed cast aluminum baffle hood with tinge
operated latch.
FINISH
The housing and doorframe are finished with polyester
powder paint applied after a seven stage pretreatment
process to insure maximum durability.
BALLAST
(Refer to Ballast Data Sheet for spelfic operating
ChareCteriStiCS)
120 volt High Pressure Sodium (HPS) ballasts are High
power Factor Reactor type. All other HPS ballasts are Hic
Power Factor Autotransformer type.
175, and 250 watt Metal Halide (MH) ballasts are Peak Le
Autotransfonnec type.
All Mercury Vapor (MV) ballasts are High Power Factor
Constant Wattage Autotransformer (CWA) type.
SOCKET ASSEMBLY
The socket assembty is designed tD place the specified lig
source in the correct position with the refractor to obtain tl
designated distribution.
uucsA usTING
The luminaire is UUCSA listed as suitable for wet location
at a 25 degree C ambient temperature.
INSTALLATION
Refer to the instruction manual provided wRh each luminai
as to the spedfic method of wiring and mounting the
luminaire.
