Conditions of Approval: Project is subject to all current City of Meridian ordinances and all other conditions
of approval issued for this site. The issuance of this CZC does not release the applicant from any previous
requirements of other permits issued for this site, including but not limited to the annexation (AZ -00-007),
preliminary plat (PP -03-026), conditional use permit (CUP -00-020), and fmal plat (FP -04-011) for Stokesberry
Subdivision No. 2.
Site Plan: The Site Plan prepared by James Gipson Associates, on July 3, 2008, labeled sheet 1, is
approved (stamped "Approved" on September 3, 2008 by the Meridian Planning Department) with the
following changes (see redline changes on site plan):
Install wheel restraints to prevent encroachment onto sidewalk (UDC 11-3C-5133).
Install at least one bicycle rack near the entrance (UDC 11 -3C -6G and UDC 11 -3C -
5C).
This site is not required as part of the Design Review to provide an eight foot,
internal pedestrian walkway from the perimeter sidewalk to the main entrance of
the building because there is no sidewalk on Eagle Road.
Except for the changes mentioned above, the approved site plan is not to be altered without prior written
approval of the Planning Department.
Landscape Plan: The Landscape Plan prepared by James Gipson Associates on July 3, 2008, labeled sheet
6, is approved (stamped "Approved" on September 3, 2008 by the Meridian Planning Department) with
the no changes from the Planning Department.
Elevations: The Elevations prepared by James Gipson Associates, on July 3, 2008, labeled sheet 5, is approved
(stamped "Approved" on September 3, 2008 by the Meridian Planning Department) with no changes from the
Planning Department.
Irrigation: An underground, pressurized irrigation system must be installed to all landscape areas per the
approved specifications and in accordance with UDC 11-3A-15.
Fazkin: The proposed parking areas shall be paved and striped in accordance with UDC 11-313. Project
engineer/architect shall certify that the number and size of handicap -accessible spaces conforms to the
Americans with Disabilities Act (ADA). Handicap accessible stalls must have signage in accordance per ADA
and signed accessible.
Cu�rbin : Per UDC 11-3B-51, all landscape areas adjacent to driveways, parking lots, or other vehicle use
areas, must be protected by curbing, wheel stops, or other approved protective devices. Curbing may be cut to
allow for storm water runoff.
Sidewalks: All sidewalks shall be constructed in accordance with 11-3A-17. Sidewalks shall be constructed
prior to occupancy.
Drainage: Storm water drainage swales shall not have a slope steeper than 3. 1, shall be fully vegetated, and
shall be designed in compliance with UDC 11-3B-11 and UDC 11-3A-18.
Lighting: Lighting shall not cause glare or impact the traveling public or neighboring development and
comply with lighting standards as defined in UDC 11-3A-11.
Signage: No signs are approved with this CZC. All business signs will require a separate sign permit in
compliance with UDC 11-3D.
Trash, Enclosure: All dumpster(s) must be screened in accordance with UDC -11-3A-12. Trash enclosures
must be built in the location and to the size approved by SSC.
Handicap -Accessibility: The structure, site improvements and parking areas must be in compliance with all
federal handicap -accessibility requirements.
ACHD Acceptance: All impact fees, if any, shall be paid prior to the issuance of a building permit. If any
changes must be made to the site plan to accommodate the ACHD requirements, a new site plan shall be
submitted to the City of Meridian Planning staff for approval prior to the issuance of a building permit.
Certificate of Occupancy: All required improvements must be complete prior to obtaining a Certificate of
Occupancy. All changes in occupancy need to comply with the requirements of the Building Department. It is
unlawful to use or occupy any building or structure until the Building Official has issued a certificate of
occupancy. A certificate of occupancy or temporary certificate of occupancy is obtained from the Building
Department (208) 887-2211 after inspections are complete and the field inspection record is returned to the
Building Department.
Plan Modifications: Except for the changes mentioned above, the approved Site/Landscape flan, stamped
"Approved" on September 3, 2008, and Elevations, and are not to be altered without prior written approval of
the Planning Department. No significant field changes to the site or landscape plans are permitted; prior
written approval of all changes is required.

The proposed building is to be used as a small medical facility (for professional
counseling), comparable to other such facilities in the area.
The hours of operation are to be within normal business hours, usually 9 AM to 5 PM,
Monday thru Thursday. No late night or group activities are planned. The planned use as
a small health care facility will be compatible with other uses in the immediate area, and
will not cause excessive traffic, parking difficulties, or other conditions considered
objectionable to neighboring properties. There should be no unusual generation of sound,
odors, vibration.
The existing trash enclosure provided for common use will be used by this facility, and is
of sufficient size to accommodate the waste anticipated. Exterior lighting will be shielded
and/or directed to avoid glare onto neighboring properties or streets. The size, scale, and
general design of the proposed building is similar to other neighboring buildings, and is
intended to be fully in compliance with all applicable codes, ordinances, and policies.

The proposed building is to be used as a small medical facility (for professional
counseling), comparable to other such facilities in the area. This will explain how the
proposed project meets the applicable Design Review standards, as follows :
1. Architectural Character:
a. The facade is minimally visible from any public street because of
the existing landscaping along Eagle Road, and the building is small enough that
significant modulation of the facade is not necessary or feasible. However, ,the East
facade (Eagle Road) incorporates modulation at the sides of the windows and variation of
the roof shape at the gable, providing a logical and appropriate character as viewed from
Eagle Road.
b. The primary entrance (from the existing private drive and parking
area to the West) is very clearly defined by the building shape and accent materials. The
windows and arches total over 30 % of the facade length on both the East and West sides.
C. The roof lines demonstrate overhanging eaves and sloped roofs.
d. The exterior walls have stucco and stone finishes, in appropriately
varying color and proportion.
All exterior mechanical equipment will be screened by
landscaping.
2. Color and Materials : The exterior materials are to be of high quality, stucco
and stone, in appropriately complementing colors.
3. Parking Lots : The parking lots are existing, provided under the planned
development, and are in compliance with applicable standards.
4. Pedestrian Walkways : The walkway system is existing, provided under the
planned development, and is in compliance with applicable standards.
We believe that the proposed project is in full compliance with the intent of the design
review requirements, and will be compatible with other buildings in the area. It will be
the building that was anticipated and intended when the development was originally
designed and approved.
