Both background and legend shall be retro-reflective. SITE PLAN NOTES 1.This site plan conforms to an actual survey that was performed on the ground by a licensed land surveyor or in and for the State of Idaho. The more stringent of any of these standards shall be the controlling standards or specifications. 2.The Contractor shall have a copy of the latest City of Meridian Standard Specifications and Drawings on site or readily accessible at all times during construction (available on the website). Failure to have access to a current copy of the Standard Specifications on site could be grounds for a stop work order until the situation is resolved. 3.The Contractor shall have plans stamped “Approved for Construction” by the City of Meridian on site at all times. 4.All Contractors, Subcontractors, and Utility Contractors shall attend a pre-construction conference prior to start of work. 5.Contractors shall notify the appropriate agency when materials are on site or inspection of the work is required. No work may begin on any project without Twenty Four (24) hour prior notice. 6.Contactor shall notify the Public Works Inspector 48 hours prior to the required testing. The Contractor may not open or close water valves. 7.All material furnished on, or for the project must meet the minimum requirements of the approving agencies. At the request of the approving agency or the Design Engineer, Contractors shall furnish proof that all materials installed on this project meet the specification requirements set forth in General Construction Note No. 1. 8.Work subject to approval by any governmental agency must be approved prior to (A) backfilling trenches for pipe; (B) placing of aggregate base; (C) placing of concrete; (D) placing of asphalt paving. 9.Inspection, approval and final acceptance of all water and sewer construction shall be by the Public Works Department, and their decision shall be final. Such inspections shall not relieve the contractor from the responsibility of performing the work in an acceptable manner in accordance with the DEQ/QLPE approved construction plans. 10.Any deviation from the approved plans and specifications must have the applicable agency approval in writing prior to construction. Prior to beginning installation of street lights, the electrical contractor shall obtain an Electrical Permit from the Building Division of the Meridian Public Works Department. Any deviation in street light locations from the approved plans must be approved in writing from the City Engineer. WATER 1.Construction of the water system shall conform to the standards in the "Idaho Rules for Public Drinking Water Systems (IDAPA 58.01.08)" as well as the standards and specifications referred to in General Construction Note No. 1. 2.The horizontal separation of potable water mains and non-potable water mains (sanitary sewer, storm drain, and irrigation) shall be a minimum of ten (10) feet. Where it is necessary for a potable water main and non-potable water main to cross with less than eighteen (18) inches of vertical separation, the crossing shall be constructed in accordance with Section 542.07 of the Idaho Rules for Public Drinking Water Systems (IDAPA 58.01.08) and Section 430.02 of the Wastewater Rules (IDAPA 58.01.16). 3.The horizontal separation of non-potable services and potable water services or potable water mains shall be a minimum of six (6) feet. Where it is necessary for a potable water main and non-potable water main to cross with less than eighteen (18) inches of vertical separation, the crossing shall be constructed in accordance with Section 542.07 of the Idaho Rules for Public Drinking Water Systems (IDAPA 58.01.08) and Section 430.02 of the Wastewater Rules (IDAPA 58.01.16). 4.Place water service lines in a two (2) inch diameter pipe sleeve wherever the service line crosses a storm water treatment facility (i.e. seepage beds, drainage swales). 5.The Contractor shall be responsible for providing continuous water service to all existing water users affected by construction. If water service must be interrupted, the contractor shall coordinate with the Public Works Inspector. SEWER 1.Construction of the sewer system shall conform to the standards in the Wastewater Rules (IDAPA 58.01.16) as well as the standards and specifications referred to in General Construction Note No. 1. 2.The horizontal separation of potable water mains and non-potable water mains (sanitary sewer, storm drain, and irrigation) shall be a minimum of ten (10) feet. ROADWAY 1.All Contractors working within the public road right-of-way are required to secure a right-of-way construction permit from ACHD or ITD at least twenty-four (24) hours prior to any construction. 2.ACHD or ITD will inspect all work within the public rights-of-way to include utility trenches above the pipe zone. PRESSURIZED IRRIGATION 1.The pressurized irrigation system must be built to the standards of the irrigation district in which it is being constructed. 2.Install a reduced pressure backflow preventer in any connection between the potable water system and the pressure irrigation system. The device must be approved by the Idaho Department of Environmental Quality (DEQ) and the City of Meridian Water Department. DRAINAGE For Commercial Subdivisions or Commercial projects with Public Mains that have on-site drainage. 1.For any drainage facilities within a public Right of Way, ACHD or ITD shall be responsible for inspection. For any private drainage facilities the project engineer or architect shall be responsible for inspection and certification of drainage facilities; however any roof drainage and piping to the drainage facilities will be inspected by the Meridian plumbing official under the appropriate permit. CLASS “A” MUNICIPAL WASTEWATER RECLAMATION AND REUSE For Commercial Subdivisions or Commercial projects that have Public Reuse Main or Services. 1.Construction of the Class A effluent distribution system shall conform to the standards in the Idaho Recycled Water Rules (IDAPA 58.01.17), Wastewater Rules (IDAPA 58.01.16), Recommended Standards for Wastewater Facilities - Great Lakes - Upper Mississippi River Board of Sanitary Engineers (10 States Standards), AWWA manual M24 - Chapter 24 as well as the standards and specifications referred to in General Construction Note No. 1. 2.The horizontal separation of Class A effluent distribution mains and potable water mains shall be a minimum of ten (10) feet in accordance with IDAPA 58.01.08, “Idaho Rules for Public Drinking Water Systems,” Subsection 542.07. Where it is necessary for a Class A effluent distribution main and potable water main to cross, a minimum of eighteen (18) inches of vertical separation shall be maintained and the Class A effluent distribution main shall be encased in a continuous pipe sleeve to a distance on each side of the crossing equal to ten (10) feet. Class A effluent distribution mains installed parallel to non-potable water mains (sanitary sewer, storm drain and irrigation) shall be installed at least five (5)feet horizontally from the non-potable main if the non-potable main is located above the Class A effluent main and three (3) feet horizontally from the non-potable main if the non- potable main is located below the Class A effluent main 3.The horizontal separation of Class A effluent distribution services and potable water services or potable water mains shall be a minimum of six (6) feet. Where it is necessary for a Class A effluent distribution service and a potable water main to cross a minimum of eighteen (18) inches of vertical separation shall be maintained, and the crossing shall be constructed in accordance with Section 542.07 of the Idaho Rules for Public Drinking Water Systems (IDAPA 58.01.08) and Section 430.02 of the Wastewater Rules (IDAPA 58.01.16). 4.Label all irrigation risers and faucets with durable tags carrying the warning CAUTION: RECYCLED WATER - DO NOT DRINK, in both English and Spanish. 5.An approved backflow preventer is required if recycled water system is connected to the potable water system. The device must be approved by the Idaho Department of Environmental Quality (DEQ), the City of Meridian Water Department, and listed in the most current version of the List of Approved Backflow Prevention Assemblies Published by the Foundation for Cross-Connection Control and Hydraulic Research, University of Southern California (USC). 6.The Public Works Department shall inspect all recycled water distribution mains and/or services. Forty-eight (48) hours advance notice is required. City of Meridian Standard Notes Residential and Commercial STREET LIGHTING 1.All street light construction shall be done in accordance with the current version of the ISPWC, the Meridian Supplemental Specifications to the ISPWC, and the National Electric Code. 2.A street lighting electrical permit shall be opened prior to starting work. Use the online Citizen Portal at https://citizenportal.meridiancity.org/CitizenAccess/ to open a permit and request inspections. 3.Installation will require inspections for the concrete base reinforcing, bedding, backfill, and for the pole and associated wiring. 4.Street light bases shall be located 10' min. from trees, 5' min. from fire hydrants and 2' min. from water meters. 5.Keep accurate notes of conduit, junction box locations. Attach a redlined as built drawing of this plan sheet to the street lighting electrical permit, or submit a hard copy to the Meridian Public Works Department prior to requesting final inspection. 6.Contact the City Transportation and Utility Coordinator (208) 898-5500 for questions regarding Utility Conflicts, or to access locked meter pedestals. 1. Quality control testing must be performed to ensure that all construction methods and materials are in accordance with ISPWC standards. 2. The testing also aids the Project Engineer and appropriate governing body (i.e. city, county, etc.) in the process of certification and final acceptance of the development. 3. Completion of this form will signify the preliminary acceptance by the Project Engineer and recommendation of the proposed development for final approval.. Ultimately, final approval is at the discretion of the governing body of the proposed development (i.e. city, county, etc.) 4. Failure to perform required testing and/or to receive initialization from Project Engineer will result in non acceptance of the the development. 5. Contractor assumes responsibility to contact Project Engineer 24 hours prior to required testing. Should scheduling conflicts arise due to failure to provide sufficient notice, said Contractor must reschedule as his/her own cost. NOTES: ROADWAY TESTING 1. Approval of clearing and grubbing of native material. Certify removal of all organics. 2. Approval of sub-grade materials. Scarify and compact sub- grade to meet compaction requirements per governing specifications (i.e. contract, Geotech report, ISPWC, etc.) 3. Approval of sub-base materials (pit run). Inspect pipe to verify that type, class, and thickness are uniform as per plans. 4. Thrust blocking provided in all required locations per ISPWC Section 401 and in accordance with Standard Drawing SD-403. 5. Approval of fire hydrant installation and elevation. Conform to ISPWC Section 403 and Standard Drawing SD-404. 6. Inspect installation of service lines, meter boxes, and curb stops. Conform to ISPWC Section 404. 7. Approval of waterline placement, prior to backfill. 8. Perform hydrostatic pressure testing per ISPWC Section 401 in presence of Project Engineer. 9. Provide waterline flushing and disinfection per ISPWC Section 401 and allow Engineer to obtain chlorine sample for testing. 10. Test for coliform bacteria per ISPWC Section 401. 11. Care shall be taken to backfill soft spots or voids with compacted fill material. ·The RSF shall be encapsulated in geotextile reinforcment placed perpendicular to the wall face as shown in the construction plans. ·The first course of GRS wall facing shall be placed on a reinforced soil founation. Great care shall be taken to ensure the first course of facing block is set level and to grade. A thin layer of fine aggregate not exceeding more than 0.5 inches thick may be used to set the facing blocks. Compaction: ·Lift heights shall not exceed the heights shown in the construction plan compacted using vibratory roller compaction equipment. ·Open hand-operated compaction equipment shall be used within 1.5 feet of the wall face to minimize movement of facing element. ·Backfill material will be compacted will be compacted to 95 percent of maximum dry density according to AASHTO T-99. Concrete Masonry Units (CMU): ·All materials shall be installed at the proper elevation, orientation, and alignment as detailed in the construction documents. ·The CMU block shall have a minimum net area compressive strength of 1900 psi according to ASTM C90 and be free of cracks or defects that would inhibit proper placement or impair block strength, durability, or stability. ·The CMU block shall be Split-Face with a minimum face shell thickness of 1.25 inches and a minimum web thickness of 0.75 inches. ·CMU block shall be swept free of debris prior to placement of geosynthetics and subsequent CMU lifts. A string line should be used to check block alignment for each lift, and the plumbness of the face should be checked every other lift to verify proper batter. ·Place dry cement in the cells of the top three rows of block. No. 4 vertical rebar shall be placed in every other cell as shown in detail A.3. Rebar shall have 2" nominal cementitious cover. ·Place open backfill in cells of all remaining block not including the top three rows as specified above. ·Block placement shall be performed such that construction begins at the lowest portion of excavation with each layer placed horizontally as shown on the plans. Geosynthetic(s): ·Each lift of geosynthetics shall be installed perpendicular to the GRS Wall face and pulled taut such that they are free of wrinkles when being covered with fill. ·The geosynthetic reinforcements shall consist of a biaxial woven polypropylene geotextile (US 200 or equivalent). ·Tracks or wheels of equipment shall not be allowed directly on geosynthetic reinforcement. A minimum fill thickness of 6 inches shall be required on top of geosynthetics prior to operating any vehicles or equipment. ·Geosynthetic reinforcement shall be placed between aggregate lifts no greater than the compacted thickness shown on the construction plans. ·Geosynthetic reinforcement shall cover a minimum of 85 percent of the top surface of the CMU block facing. Excess material can be removed by burning with a propane torch or cutting with a razon knife. ·Placement of fill shall be from the wall face backward to prevent the formation of wrinkles in the geosynthetic reinforcement. ·Reinforcement splices shall occur without overlap and should be staggered to avoid a continuous break in reinforcement in the GRS structure. This plan should be revised and updated to address changes in site conditions, new or revised government regulations, and additional on-site storm water pollution. Additional erosion control measures may be required. 2. All revisions to this plan must be documented on the SWPPP Revision Documentation Form. 3. Current versions of the SWPPP, the NOI, and the NOC will be kept on site for the duration of the project. These items will be available for the use of all operators and site personal involved with erosion and sediment controls, and be available to EPA visiting the site. A notice will be posted near the construction entrance during construction, containing the SWPPP, the NOI and the NOC. 4. Fugitive dust blowing from the site shall be controlled by spraying water and dust control polymers as needed on dry areas of the site. 5. The contractor will be responsible for supervision and inspection of all erosion and sedimentation controls and for ensuring the SWPPP is implemented. 6. Prior to beginning earth-moving activities, including clearing and grubbing, all clearing limits, easements, setbacks, sensitive areas and their buffers will be clearly marked to prevent environmental damage both on and off the site. 7. If sediment is accidentally transported on to the street it will be removed from the street surface on a daily basis. 8. All off-site construction shall be stabilized at the end of the working day. 9. All waste material will be collected and stored in a securely lidded dumpster. The dumpster will meet all local and state solid water management regulations. 10. Portable sanitary units will be provided for use by all workers for the entire project. Sanitary waste will be collected regularly for the portable units by an approved sanitary waste management contractor. 11. All exposed soils will be stabilized with vegetation or covered no more than 14 days after the construction activity in that portion of the site has temporarily or permanently ceased. 12. Existing and new vegetation will be maintained to the maximum extent practicable to prevent the contamination of storm water with sediment. 13. The contractor shall be responsible for adjusting the erosion control measure; due to grade changes during the development of the project. 14. Maintain on the site or have readily available sufficient oil and grease absorbing materials to contain and clean up fuel or chemical spills and leaks. 15. Adequate energy dissipation, erosion control, and soil stabilization measures will be provided for all point source discharges of storm water, including run-on discharges and outlets for onsite discharges. 16. Temporary and permanent swells and small detention ponds will be used as necessary to reduce the velocity of runoff and enhance particle settling. 17. Consistent with the general permit requirements, all potential pollutants other than sediment will be handled and disposed of in a manner that does not affect contamination of stormwater. 18. Materials used during construction with the potential to impact storm water, will be stored, managed, used, and disposed of in a manner that minimizes the potential for releases to the environment and especially in the storm water. 19. If a spill of pollutants threatens storm water at the site, the spill response procedures must be implemented in a timley manner to prevent the release of pollutants. 20. All temporary and permanent erosion and sediment control BMPs will be maintained and repaired as needed to assure continued performance of their intended use. 21. All temporary erosion control and sediment control BMPs will be removed within 30 days after final site stabilization is achieved or after the temporary BMPs are no longer needed. 22. Regardless of recommended maintenance schedule, all control measures and inspections shall be performed within 24 hours following any storm of 0.25 inches or greater. An inspection report shall be kept at all times and should be retained for at least three (3) years from the date the site is stabilized. 23. All contractors providing services on the project which may cause storm water pollution will be given a copy of the SWPPP and appropriate training regarding stormwater pollution prevention.
