sworn, says:
1. I we' am (are_) the owner(s) of the property located at Lot 11 of the Lost
Rapids Subdivision in Meridian, Idaho.
2. My signature below attests that I have reviewed the proposal by Legend
Engineering requesting application review of a building and associated site
improvements at Lot 11 of the Lost Rapids Subdivision and that I consent to
the statements and information provided in the attached plans and exhibits.
