"I hereby certify that I have read and fully understand the rules and regulations
of Ordinance No. 3-101, 3-201 to 3-214 inclusive of 3-1101 to 3-1104 inclusive, City of
Meridian, requiring licenses and regulating the sale of beer. That I am over 21 years
of age, a citizen of the United States, and have been a bonafide resident of the State
of Idaho for the period of 30 days and am a bona -fide resident of the County of Ada at
the present time; and have not been convicted of any law regulating, governing, or
prohibiting the sale of alcoholic beverages or intoxicating liquors and that my place of
business shall not contain screens, boxds, stalls, or any room on the premises where
beer is to be sold. That I am aware of the minimum age for sale of Beer, and penalties
for violation of same.
That as a vender of packaged, bottl.eci, or canned beer, not to be consumed on the
premises, I will supervise and make certain that no beer shall he consumed on these
premises or parking lots under my control.
These premises shall be,o pen to officers of the United States, State of Idaho, or
City of Meridian when -ever occupied and shall not be open for sale of beer between the
hours of 1:00 o'clock A.M. and 7:00 o'clock A.M. and that I will uphold the laws of the
United States, State of Idaho, and of Meridian at all times. That I am familiar with
location limitations as set up by the County, State and City of Meridian, and will
comply with any other special limitations as are in force or as may come about."

Gentlemen:
Storm Sewer - 45° elbow near Eighth Street West is poor. Suggest
extension on a curve until location of line is reached. Suggest
rearrangement of manholes so there is one at junction from inlet
pipe at 11th Street West.
Camellia Street - We no longer use a 45 foot back to back of curb.
This may be changed to 41 foot back to back. If curb radii at
Eighth Street West are changed, they should be made larger.
Roll Curb Section - Our standard width is now 24 inches. The 27
inch may be used but would suggest the new standard on future work.
We will need a 1:300 of plat area.
The Ada County Highway District Engineering Department is to send you
