Verizon Main Street Modifications A-2017-0221-CZCConditions Document 1 Verizon Main Street Modification- A-2017-0221 CERTIFICATE OF ZONING COMPLIANCE REPORT DATE: December 6, 2017 TO: Jodie Knopp, TAEC FROM: Brock Cherry, Assistant City Planner SUBJECT: Verizon Main Street Modifications– A-2017-0221 OWNER: Lawrence Beddes DESCRIPTION OF APPLICANT'S REQUEST The applicant, Jodie Knopp, requests a Certificate of Zoning Compliance (CZC) to modify the existing wireless facility located at 1535 N. Main St. (6) existing panel antennas will be replaced with six (6) new panel antennas. The scope of work includes replacement of six (6) existing panel antennas with six (6) new panel antennas, three (3) existing 60W AWS RRH units with three (3) new 90W AWS RRH units, and one (1) existing MOVP with one (1) new tower top MOVP unit. The following items are proposed to be installed: Six (6) new back to back RRH unit mounts, three (3) new 850 RRH units, three (3) new PCS RRH units, six (6) new pipe mounts, one (1) new hybrid trunk line, one (1) new 4’ microwave antenna (MW), one (1) new pipe mount for the MW, two (2) new tri collar mounts for the MW, and one (1) new line of EW90 for the MW. One (1) existing rack and stack will be installed within the existing shelter. There will be some other minor modifications within the existing ground shelter, but no changes to the ground space or height of the tower. Process Conditions of Approval 1. The applicant shall complete all improvements related to public life, safety, and health as set forth in UDC 11-5C-3B. A surety agreement may be accepted for other improvements in accord with UDC 11-5C-3C. 2. The site plan prepared by J5 Infrastructure Partners, dated August 21 2017, titled “Existing Communications Site 850 LTE and AWS-3 Installation Project”, is approved on December 6, 2017 by the City of Meridian Planning Division, as shown in Exhibit B. 3. The elevations prepared by J5 Infrastructure Partners, dated August 21, 2017, titled “Elevations”, are approved on December 6, 2017 by the City of Meridian Planning Division with no changes, as shown in Exhibit C. 4. The approved site plan and/or elevations may not be altered without prior written approval of the City of Meridian Planning Division. 5. The applicant and/or assigns have the continuing obligation to provide irrigation that meets the standard as set forth in UDC 11-3B-6 and to install and maintain all landscaping as set forth in UDC 11-3B-5, UDC 11-3B-13 and UDC 11-3B-14. 2. The applicant shall comply with the outdoor storage as an accessory use standards as set forth in UDC 11-3A-14. 3. The project is subject to all current City of Meridian ordinances. 4. The issuance of this CZC does not release the applicant from any previous requirements of the other permits issued for the site (CZC-14-060). 5. The applicant and/or property owner shall have an ongoing obligation to prune all trees to a minimum height of six feet above the ground or sidewalk surface to afford greater visibility of the area. 6. The applicant has a continuing obligation to comply with the outdoor lighting provisions as set forth in UDC 11-3A-11. 7. The applicant and/or property owner shall have an ongoing obligation to maintain all landscaping and constructed features within the clear vision triangle consistent with the standards in UDC 11-3A-3. 8. The applicant and/or assigns shall have the continuing obligation to meet the specific use standards for the proposed use as set forth in UDC 11-4-3-43. CITY COUNCIL REVIEW The applicant or a party of record may request City Council review of a decision of the Director. All requests for review shall be filed in writing with the Planning Division on or before December 21, 2017, within fifteen (15) days after the written decision is issued, and contain the information listed in UDC 11-5A-6B. If City Council review of the decision is not requested, the action of the Director represents a final decision on a land use application. You have the right to request a regulatory taking analysis under Idaho Code 67-8003. EXPIRATION Certificates of Zoning Compliance issued in conjunction with a proposed use shall expire if the use has not commenced within one year of the date of issuance of the Certificate of Zoning Compliance. Certificates of Zoning Compliance issued in conjunction with construction or alteration of a structure shall expire if the construction or alteration has not commenced within one year of the date of issuance of the Certificate of Zoning Compliance.
