PROFESSIONAL SERVICES
THIS AMENDMENT TO THE AGREEMENT FOR PROFESSIONAL SERVICES is made this
day of A , 2009, and entered into by and between the City of
Meridian, a municipallcorporation organized under the laws of the State of Idaho, hereinafter
referred to as "CITY", 33 East Broadway Avenue, Meridian, Idaho 83642, and Harvest Design,
P.C., hereinafter referred to as "CONSULTANT", whose business address is 2971 E. Copper
Point Dr. Ste. 150, Meridian, Idaho 83642.
INTRODUCTION
The referenced Agreement for Professional services was entered into by and between the City
and Consultant on the 16th day of December, 2008.
TERMS AND CONDITIONS
All terms and conditions of the Agreement for Professional Services apply.
SCOPE OF AMENDMENT NO.1
Task 1 — Expand Interpretive Scope
1.1 Consultant shall perform work requested by the City to expand the scope of the
interpretive elements relating to the use of reclaimed water at the park. This scope
includes the addition of 5 raised planter beds, researching and specifying native "water
wise" plants that are drought tolerant, additional planter beds for interpretive information,
landscape plan modifications, and irrigation plan modifications. Compensation shall not
exceed $9,500.00.
Task 2 — Extend Sewer Mainline and Sidewalk at Ten Mile Rd
2.1 Consultant shall perform work requested by the City relating to the extension of the
sewer mainline and sidewalk on Ten Mile Rd. to the north property boundary of the
improvements associated with the Agreement For Professional Services. This scope
includes potholing to locate existing utilities, surveying, contract document modifications,
sidewalk design, ACHD license agreement, easement legal descriptions, property line
adjustment with north property owner, coordination with the Silverleaf Subdivision, and
coordination with Settlers Irrigation. The extension of the sewer will allow the public
restroom facility (in the proposed new maintenance facility) to gravity flow to the
mainline. Compensation shall not exceed $9,317.00.
Task 3 — Provide Monitoring and Control Schematics for the Reclaimed Water System
3.1 Consultant shall perform work requested by the City relating to providing monitoring and
control schematics for the reclaimed water system. This information will provide
parameters for construction to assure the quality of the finished product meets the City's
standards and expectations. Compensation shall not exceed $1,573.00

Task 4 — Print Documents for Bidding
4.1 Consultant shall print all project documents for the use in bidding. Compensation shall
not exceed $6,151.75.
