In regards to the above referenced retirement center, Grace at Fairview Lakes is requesting a
modification to the original conditional use. The original application proposed developing a
three phase retirement facility consisting of a 30 unit memory care, a 60 unit assisted living and
a separate congregate care facility. Grace would like to modify the proposed development to
remodel the existing assisted living facility and add 27 units of assisted living to the existing
facility. Upon completion of the addition it will complete the development of the retirement
phase of Fairview Lakes.

Per Unified Development Code (UDC) 11 -5A -5D, the applicant for all applications requiring a
