DESCRIPTION OF APPLICANT'S REQUEST
The applicant, Blue Cross of Idaho, requests Certificate of Zoning Compliance (CZC) approval
of an addition of two new parking lots on the west side of the Blue Cross of Idaho campus
adjoining Rosario St. The new parking lots will accommodate 236 parking spaces.
Improvements include interior parking lot landscaping, modifications to adjoining perimeter
landscaping, new/modified pedestrian paths and connections to existing parking lots, new and
modified existing irrigation system to serve parking lot landscape and adjoining modified areas,
and new LED site lighting at parking lots and pathway connections on 34.687 acres of land in
the I -L zoning district.
The site is located at 3000 E. Pine Ave.
DECISION
The applicant's request for Certificate of Zoning Compliance is approved with the conditions
listed in this report.
Note: This is not a building permit. Please contact Building Services at (208) 887-2211 to
verify if you need a building permit and/or inspection. Ifyou do need a building permit, you
must complete that process before you commence the use or construction. Please contact
Building Services for additional details about building permits and inspections.
Site Conditions of Approval
1. UDC 11-313-1005 requires mitigation for all existing trees four inch (4") caliper or
greater that are removed from the site with equal replacement of the total calipers lost.
Three trees are being removed that meet this requirement. Mitigation is required.
2. UDC l l-3B-l0C3 requires the City Arborist approve protection fences for trees that are
to be protected during construction. Contact the City of Meridian Parks Department to
receive approval.
Process Conditions of Approval
1. No signs are approved with this application. Prior to installing any signs on the property,
the applicant shall submit a sign permit application consistent with the standards in
UDC Chapter 3 Article D and receive approval for such signs.
2. The applicant shall complete all improvements related to public life, safety, and health
as set forth in UDC 11 -5C -3B. A surety agreement may be accepted for other

improvements in accord with UDC 11 -5C -3C.
3. Upon installation of the landscaping and prior to inspection by Planning Division staff,
the applicant shall provide a written certificate of completion as set forth in UDC 11-
3B -14A.
4. The site plan prepared by CSHQA on April 29, 2015, labeled C1.0, is approved as
shown in Exhibit B with the following comments.
• The applicant shall provide stamped civil drawings detailing the drainage of
the parking lot to Public Works for review and approval.
5. The landscape plan prepared by CSHQA on April 29, 2015, labeled L1.0, is approved as
shown in Exhibit C.
6. The approved site plan and landscape plan may not be altered, except for the changes
requested above, without prior written approval of the City of Meridian Planning
Division.
7. The applicant shall pay any applicable impact fees prior to the issuance of a building
permit.
8. If any changes must be made to the site plan to accommodate ACHD requirements, the
applicant shall submit a new site plan to the City of Meridian Planning Division for
approval prior to issuance of the building permit.
9. The applicant shall complete all required improvements prior to issuance of a Certificate
of Occupancy. It is unlawful to use or occupy any building or structure until the
Building Official has issued a Certificate of Occupancy.
Ongoing Conditions of Approval
1. The applicant and/or assigns shall have the continuing obligation to provide irrigation
that meets the standards as set forth in UDC 11-3B-6 and to install and maintain all
landscaping as set forth in UDC 11-3B-5, UDC 11-3B-13 and UDC 11-3B-14.
2. The project is subject to all current City of Meridian ordinances and previous conditions
of approval associated with this site (CZC-01-037; CZC-05-121; Gemstone Center No.
1).
3. The issuance of this CZC does not release the applicant from any previous requirements
of the other permits issued for the site.
4. The applicant and/or property owner shall have an ongoing obligation to prune all trees
to a minimum height of six feet above the ground or sidewalk surface to afford greater
visibility of the area.
5. The applicant shall have an ongoing obligation to maintain all pathways.
6. The applicant has a continuing obligation to comply with the outdoor lighting provisions
as set forth in UDC 11-3A-11.
7. The applicant and/or property owner shall have an ongoing obligation to maintain all
landscaping and constructed features within the clear vision triangle consistent with the
standards in UDC 11-3A-3.
CITY COUNCIL REVIEW
The applicant or a party of record may request City Council review of a decision of the
Director. All requests for review shall be filed in writing with the Planning Division on or

before May 23, 2015, within fifteen (15) days after the written decision is issued, and contain
the information listed in UDC 11 -5A -6B.
If City Council review of the decision is not requested, the action of the Director represents a
final decision on a land use application. You have the right to request a regulatory taking
analysis under Idaho Code 67-8003.
EXPIRATION
Certificates of Zoning Compliance issued in conjunction with a proposed use shall expire if the
use has not commenced within one year of the date of issuance of the Certificate of Zoning
Compliance.
Certificates of Zoning Compliance issued in conjunction with construction or alteration of a
structure shall expire if the construction or alteration has not commenced within one year of the
date of issuance of the Certificate of Zoning Compliance.
In accord with the above provisions, the subject Certificate of Zoning Compliance is valid until
May 8, 2016.

Addition of two new parking lots on the west side of the Blue Cross of Idaho campus adjoining Rosario St.
The new parking lots will accommodate 108 and 124 parking spaces. Improvements include interior parking
lot landscaping, modifications to adjoining perimeter landscaping, new/modified pedestrian paths and
connections to existing parking lots , new and modified existing irrigation system to serve parking lot
landscape and adjoining modified areas, and new LED site lighting at parking lots and pathway connections.
Parking lot landscape has been designed to contain compatible plant/tree species to those in existing

information. Due to the remote location of the lots, we have analyzed the existing accessible spaces
adjoining the building and determined that sufficient existing accessible spaces are provided for the total
number of parking spaces provided on site. A total of 14 new bike parking spaces are being provided in two
new racks adjoining the existing bike parking in two locations near the existing buildings. (10 required for
the proposed new parking lots.) Racks shall match existing bike racks on site. See Sheet C1.) for
additional information regarding parking and bike parking.

To Whom It May Concern:
On behalf of our client, Blue Crass of Idaho, and in accordance with submittal requirements for a CZC
Application, please accept this narrative, describing the proposed development of additional parking on
the West edge of the existing Blue Cross of Idaho Campus to serve employees within the existing
headquarters office buildings.
The area of the proposed lots falls between the existing internal circulation drive and North Rosario Street
to the West. These proposed parking lots have been designed to contain up to 108 and 128 new stalls for
use of the campus occupants and provide connecting paths to the existing lots to the east. Vehicular
access to the proposed parking lots will be provided off of the existing campus entry drive (East State
Street) and a second access will be provided to Parking Lot No. 2 by a new access drive which aligns with
an existing internal drive near the existing central services building. Due to the location of the proposed
lots, the three accessible parking spaces required by these lots are met by existing excess accessible spaces
adjoining the existing headquarters office buildings. (1243 existing spaces, requiring 23 accessible spaces
— 27 existing accessible spaces provided.) Fourteen new bike parking spaces to serve the new parking lot
are proposed to be provide in two racks of seven adjacent to two existing bike parking locations near the
existing buildings (10 required). The proposed parking lots include landscaped planting beds, concrete
curbs, asphalt paving, and new LED parking lot lighting. New landscape planters are to be tied into the
existing irrigation system which will be modified as required to accommodate the new parking lot areas.
Pathways are provided to the adjoining existing parking lots and the existing employee walking paths are
modified as required by the new parking lot construction. Pathways to the parking lots will be lit by new
LED site lighting on 20' pales.
