MASTER AGREEMENT FOR PROFESSIONAL SERVICES
BETWEEN
CITY OF MERIDIAN (OWNER) AND CIVIL SURVEY CONSULTANTS, INC (ENGINEER)
This Task Order is made this ` day of, .b f`�, 2012 and entered into by and between the
City of Meridian, a municipal corporation organized �fnder the laws of the State of Idaho,
hereinafter referred to as "City", and accepted by Civil Survey Consultants, Inc, hereinafter
referred to as "Engineer" pursuant to the mutual promises, covenant and conditions contained in
the Master Agreement between the above mentioned parties dated April 13, 2010. The Project
Name for this Task Order 0659c is as follows:
BLACK CAT ROAD FLUSH LINE
AND
PRV DETAIL REVISIONS
SCOPE OF SERVICES
Per the City's request, the Engineer will provide professional engineering services related to the
design of a potable water system flush line and pressure relief valve on Black Cat Road at Five
Mile Creek. The Engineer will also provide professional engineering services related to the design
of revisions to the City's standard pressure reducing valve station details.
The flush line will be designed with the ability to manually flush the water system and the ability for
the water system to automatically flush to relieve system pressure.
There are two (8" and 10") pressure reducing valve station details that will be revised. The same
revisions will be made to both details with the revisions sized appropriately for each detail. The
stations will be revised per notes provided by the City. The City will provide Engineer an
electronic AutoCad copy of their current station details.
Task 1 —Administration
1.1 This task includes duties associated with setting up the contract and submitting
monthly invoices.
Task 2 — Design Surveying = Flush Line
2.1 This task includes field work associated with performing topographic survey required
to update the topographic survey from the 2007 Black Cat Road Water Main project.
2.2 This task includes office work associated with drafting the topographic survey and
establishing the public right-of-way and property lines. We anticipate that for the
purposes of this project we will be able to define the existing public right-of—way and

property lines from the well 24 design project and from record of surveys and
subdivision plats recorded with Ada County.
Task 3 — Design Flush Line
3.1 This task includes work associated with designing the new flush line. The flush line
will be designed with the ability to manually flush the water system and the ability for
the water system to automatically flush to relieve system pressure.
Task 4 — Construction Document Preparation and Review — Flush Line
4.1 Develop plans and specifications and bid schedule that meet current design
standards and standard specifications of the City and ISPWC.
4.2 Submit 2 full size physical copies of plans and special provisions to City Public
Works Department for Development Services review and approval. Includes one set
of revisions per City comments and then resubmitting to the City for approval.
4.3 Submit 4 full size physical copies of plans and special provisions to City Public
Works Department for QLPE review and approval. Includes one set of revisions per
City comments and then resubmitting to the City for approval.
4.4 Submit one set of plans to ACHD for review and approval prior to construction.
Includes one set of revisions per ACHD comments and then resubmitting to ACHD
for approval.
4.5 Prepare a Traffic Control Plan to be included in the construction plans. The traffic
control plan will not be submitted to ACHD for review and approval. The contractor
may use the plan to assist him in obtaining a permit from ACHD. The contractor will
make all necessary revisions to the traffic control as required by ACHD.
4.6 Prepare an Erosion and Sediment Control Plan (ESCP) to be included in the
construction plans. The ESCP will be submitted to the City and ACHD for review and
approval. The ESCP will be prepared in accordance with the City of Meridian
Construction Stormwater Management Program. All other stormwater management
program requirements shall be completed by the City or the Contractor.
4.7 Engineer will prepare and submit a license agreement application to Nampa Meridian
Irrigation District. City shall pay all fees associated with the agreement.
4.8 Engineer will prepare and submit a Joint Application for Permit to the Army Corps of
Engineers and the Idaho Department of Water Resources. City shall pay all fees
associated with the agreement.
4.9 Prepare final construction documents with inclusion of any addenda from the bidding
phase and provide one physical copy for City's use and 10 physical copies for
contractor's use.

Task 5 — Construction Services — Flush Line
5.1 Provide one set of construction stakes for the new flush line. It is anticipated that
staking will be completed in one visit to the site. Staking will be completed as
requested by the City and Contractor.
5.2 Attend pre-bid meeting.
5.3 Attend pre -construction meeting.
5.4 Answer RFI's from contractor during bidding and construction about the design.
5.5 Review material specifications submitted by contractor.
5.6 Prepare record drawings meeting the City of Meridian Acceptance of
Record/Electronic Drawings, dated August 26, 2009.
Task 6 — PRV Detail Revisions
6.1 This task includes work associated with designing revisions to the City's standard
pressure reducing valve station details. The following revisions will be made:
• Two details will be prepared for each size (8" and 10"). There will be one
detail with manhole cover access for use in the roadway and one detail with
hatch access for use outside the roadway.
• The vault size will be modified as required to allow for revisions.
• Gate valves will be relocated to inside vault.
• Flow Meters will have digital readouts, and the ability to provide total flow and
instantaneous flow through SCADA. Flow meters will include a flow
straightener to reduce required length of pipe run before flow meter. This will
assist with the ability to minimize vault size.
• Pressure reducing valves equipped with valve position indicators and external
y -strainers.
• The pressure gauge and pressure transducer located on the pressure
reducing valve will be eliminated. A pressure gauge and transducer will be
located upstream and downstream of pressure reducing valve.
• Controls for pressure reducing valve and bypass piping will be flipped to other
side of pressure reducing valve.
• A meter and a check valve will be added to the bypass. If possible bypass will
come off pressure reducing valve instead of saddle taps.
• A water supply tap with vacuum breaker will be added to each detail.

a A strainer will be put in front of the pressure reducing valve.
This scope of work does not include revisions to the Pressure Reducing Station SCADA or
electrical systems. The City will coordinate the SCADA or electrical system revisions with
Electrical Engineer.
6.2 Submit 2 full size physical copies of revised details to City Public Works Department
for review and approval. Includes one set of revisions per City comments and then
resubmitting to the City for approval.
TIME OF COMPLETION
Engineer will work with project manager to complete design in a timely manner.
COMPENSATION
The Not -To -Exceed amount for this Task Order No. 0659c is Nineteen Thousand Forty -Eight
Dollars ($19,048.00).
The hourly rates for services and direct expenses are per the Master Agreement and by this
reference made a part hereof. No compensation will be paid over the Not -to -Exceed amount
without prior written approval by the City in the form of a Change Order. Travel and meals are
excluded from this Task Order unless explicitly listed in the Scope of Services AND Payment
Schedule.
