AGREEMENT NO. 04
CITY OF MERIDIAN DOGrLICENSE AGREEMENT
This agreement is entered into this ~ (~ " 21ay of ~/ !il , 1999, by
and between the City of Meridian, State of Idaho, hereinafter "City" and Intermountain Pet
Hospital, hereinafter "Vendor".
I. PERIOD OF AGREEMENT
The period of Agreement shall be from January 1, 2000, through December 31, 2001,
unless terminated earlier by either party as set forth in Paragraph #6.
II. PURPOSE
The purpose of this Agreement is to set forth the rights and obligations of the parties with
reference to this issuance and sale of dog licenses pursuant to Meridian City Code § 6-2-3, in
order to encourage and facilitate the licensing of all dogs within Meridian.
III. TERMS AND CONDITIONS
1. The City shall issue to the Vendor the following items as requested by each Vendor:
license tags, links and registration cards. The City shall also issue a reporting form for sales.
2. License fees shall be as follows:
Non-neutered or non-spayed dog $15.00
Neutered or spayed dog $7.50
Duplicate License Fee $5.00
Senior Citizens (62 and over) $5.00

Meridian City dog licenses maybe purchased only by residents of Meridian.
4. Vendors shall deposit with the Meridian City Clerk receipts from license sales on a
monthly basis and forward a copy of the monthly reporting form to the Meridian City Clerk. The
clerk shall reconcile reports and receipts.
Vendors shall return all unsold licenses, links and registration forms to the Meridian
City Clerk's Office by December 31S` of each year during the contract period.
6. Termination of Agreement may be made by either party upon thirty (30) days
written notice to the non-terminating party. Said notice shall be appropriately addressed as

Upon termination, Vendor shall return all unsold licenses, links and registration forms to
the Meridian City Clerk no later than the expiration of the thirty (30) day notice period.
7. This Agreement and its performance shall be construed in accordance with and
governed by the laws of the State of Idaho. Should any of the terms, provisions, or paragraphs of
this Agreement be held in a court of law to be invalid, it is recognized by the parties herein that
said terms, provisions or paragraphs so held invalid may be stricken and the remainder continue
in effect.
