Broadway Ave, Meridian, ID 83642 Phone: (208) 888-4433 x 417 Fax: (208) 887-4813 TASK ORDER NO. 10018C Between CITY OF MERIDIAN AND CH2M HILL This Task Order is issued by City and accepted by the Engineer pursuant to the mutual promises, covenant and conditions contained in the Master Agreement between the above mentioned parties dated April 13, 2010. Bitter Creek Lift Station and Pipelines Project Contract Extension for Contract Document Revisions and Services During Construction For Construction Re-bidding Task Order 10018C Project Understanding CH2M HILL will provide revisions to the contract documents and perform additional services during construction in support of the Bitter Creek Lift Station and Pipelines project. The revised scope of services described below is in response to the City's re-bidding effort for the remaining construction work associated with this project. PAGE 1 OF 6 The wastewater lift station is located near the intersection of Amity and Linder Roads adjacent to the Bitter Creek development. The pipelines include new dual sanitary sewer force mains (6- and 10-inch diameter) and a new 16-inch potable water main. The dual sanitary sewer force mains originate at the new lift station, run north to Amity Road, thence east along Amity Road to the intersection with W. Ten Mile Road, thence north along W. Ten Mile Road to terminate at the connection to that portion of force main previously constructed by the original contractor (south side of W. Victory Lane) for a total combined length of approximately 2.1-miles. The potable water main originates by the W. Amity Road and S. Nickel Creek Place intersection, run east along Amity Road to the intersection with W. Ten Mile Road, thence north along W. Ten Mile Road to terminate near the new Overland Road re-alignment (new intersection for the Southridge development) for a total length of approximately 2.1-miles. As discussed with the City, construction of the waterline is contingent upon decisions by JLJ Enterprises and is to be treated as a bid alternative. The total estimated contract compensation for the Scope of Work described below will amend, and effectively replace, the remaining compensation value under the original contract (10018), Amendment 1 (10018A), and Amendment 2 (10018B) as previously authorized based on the date of execution of this amendment. Scope of Work CH2M HILL will perform the tasks for contract document revisions and services during construction in support of the Bitter Creek Lift Station and Pipelines project as outlined below. Task 1-Contract Document Revisions Task Description: • Incorporate Requests for Information (RFIs) and Work Change Directives (WCDs) from previous bid and construction period into contract plans and specifications. • Revise plans and specifications to reflect work completed under previous contract. Include materials purchased by City to be installed by contractor. • Revise plans to provide water services for adjacent land owners. Add water service details. City to coordinate with land owners and provide CH2M HILL with locations and sizes of water services. • Collaborate with City to revise specifications to reflect aCity-managed bid and construction project. • Include City-provided storm water specification section. • Modify bid tab to reflect itemized line item bidding. (City to provide format} • Revise plans and specifications to reflect water line construction as a bid alternate. PAGE 2 OF 6 • Submit plans to City of Meridian Development Services for review and address comments. • Prepare estimate of construction cost based on the revised project documents. Deliverables: • Engineer's estimate of construction cost. (PDF digital format) • One (1) original, full-size set of construction plans with engineer's seal for City to use for bid reproduction. • One (1) original copy of project specifications with engineer's seal for City to use for bid reproduction. Assum tions: • Revisions to construction documents will not require re-submittal to DEQ; however, CH2M HILL will obtain permit extensions, as necessary. • City Purchasing Department to provide any City standard specifications for inclusion related to bidding, management, Contractor payment, and execution of the construction contract between the City and the Contractor. Expenses • N/A. Task 2 -Bid Period Services Task Descri tion: • Attend apre-bid meeting with prospective bidders and City. Budget assumes four (4) hours for CH2M HILL PM to attend one (1) meeting. • Respond to questions from bidders and provide City with responses to distribute to bidding contractors. Budget assumes ten (10) RFI's at two (2) hours each. • Assist the City by providing information to prepare bid addendums, as necessary, for City to issue to bidding contractors. Budget assumes four (4) addendums at one (1) hour each. • Assist City in opening of bids, review of bids for completeness, evaluation, and award. Budget assumes four (4) hours. Deliverables: • Responses to bidder questions. • Information for bid document addendums, as necessary. • Bid evaluation assistance (if requested). Assumptions: PAGE 3 OF 6 • City Purchasing Department will be responsible for advertising, leading, and coordinating the bid process. Ex enses • Travel. Task 3 -Services During Construction Task Description: • Attend apre-construction meeting with the Contractor, City, and other agencies (ACHD). Budget assumes four (4) hours for one (1) meeting. • Review and respond to ten (10) technical submittals from the Contractor. Budget assumes four (4) hours per submittal. • Review and respond to requests for information (RFI) from the Contractor. Develop Work Change Directives (WCD), as necessary. Budget assumes ten (10) RFI at three (3) hours each and five (5) WCD at three (3) hours each • Attend weekly project meetings on site. Prepare and issue meeting minutes. Budget assumes twenty-five (25) meetings at three (3) hours per meeting (includes travel time). • Conduct additional site visits for periodic field observations. Budget assumes six (6) trips at three (3) hours each including travel time. • Assist City in the review of Contractor Change Order requests. Budget assumes two (2) change orders at two (2) hours each. • Review as-built notes provided by the Contractor. Prepare record drawings based on contractor as-built notes and supplemental survey data, where applicable. Provide City with hard copies and electronic files of the project record drawings. Deliverables: • Minutes from weekly construction progress meetings. • Technical submittal review comments. • Contractor RFI responses. • Work Change Directives. • Record drawings -three (3) full-size bond copies. One (1) CD with PDF for DEQ. One (1) CD with PDF and CAD for City. Assum tions: • It is assumed that a City inspector will be performing daily inspections during construction and that the City will notify CH2M HILL of any material variation from design. It is also assumed that the City will provide CH2M HILL with written PAGE 4 OF 6 documentation of daily construction inspection and observation activities. CH2M HILL assumes that the set of record drawings being maintained by the Contractor will be available for review when CH2M HILL visits the site during construction, and that the City inspector will be providing periodic reviews to ensure that the Contractor is keeping the record drawings up to date. • The presence or duties of Consultant or Owner personnel at a construction site, whether as onsite representatives or otherwise, do not make Consultant or Owner personnel in any way responsible for (or relieve construction contractor from) those duties, obligations and responsibilities of the construction contractor, including, but not limited to, all construction methods, means, techniques, sequences, and procedures necessary for coordinating and completing all portions of the construction work in accordance with the construction Contract Documents and any health or safety precautions required by such construction work. • The City or the Contractor will provide CH2M HILL with as-built survey data of the constructed infrastructure as necessary to allow CH2M HILL to prepare accurate and complete record drawings. Expenses • Travel. Task 4 -Project Management Task Descri tp ion: • Supervising CH2M HILL project staff. • Project documentation and coordination. • Monitoring project progress, including work completed, work remaining, budget expended, schedule, estimated cost of work remaining, and estimated cost at completion. • Monitoring project activities for potential changes, anticipate changes when possible, and with Owner approval, modify project tasks, budgets, and approach. • Preparing and submitting a monthly billing with backup documentation. • Performing project quality control reviews of deliverables on this project. Deliverables: n/a Assum tions: • Nine (9} month duration for construction contract document revisions and field construction activities by the Contractor. PAGE 5 OF 6 Compensation Level of Effort The task level of effort estimates are summarized in the following table. CH2M HILL has developed this level of effort estimate based on the work plan presented herein and assumptions stated previously. The fees will be billed as time and material, and will not exceed the total amount below without prior approval by the City.
