I've prepared the final construction drawdown request for completion of the project.
Please have Mayor DeWeerd sign all three copies, keep one for your records and return
the two-marked "sage" and "commerce" to Sage for processing.
The only items left that need to be completed in order to close-out the grant in a timely
fashion are the 504 Handicap Accessibility guidelines and the Fair Housing analysis. I
am enclosing a copy of the 504-transition plan that the City needs to fill out in order to
comply with HUD regulations. Please give the transition plan to the head of y..our
building department or parks department and ask them to fill in the timeline portion as
well as the estimated cost portion. Please return this form to me no later than October ~ ~ O ~
I am also enclosing a Grantee Employment form, which needs to be filled out by
someone in your Human Resources Department. This form lists employee classifications
in the City as well as pay grades and numbers for minority employed and other attributes
that HUD also requires. Please also have this form back to me no later than October 3ra
as well. If either department needs assistance, please tell them to contact me directly.
