The following routline details the history and facts surrounding the
inconsistency of street name usage for the section of Meridian/Kuna
Meridian Road which extends south of Overland Road to King Road:
* Since 1972 several plats have been recorded with boundaries
adjacent to Hwy 69,
* Twelve were recorded with the name of Meridian Road
0 Thirteen with the name of Kuna Meridian Road
* Most properties do not have frontage/access on Highway 69, Of the
ones that do, the majority recognize their address as Meridian
Road, according to Dispatch research,
ty Over the years, the Street Name Committee has approved
evaluations using both Meridian Road and Ku Meridian Road.
All street signs, with the exception of two private lane signs, read
Meridian Road,
Agri 02 20 - ACSO Dispatch requested a discussion be put on the

street name committee agenda regarding Meridian Road/Kura
Meridian Road, due to recent emergency calls where callers gave
street names that were not consistent with the 911 database (see
dispatch memo dated 5118109).
AxLL-012,-2-0-09 - Street Name Committee discussed the history as
outlined above. The Committee recommended that the name be
officially changed to Meridian Road with proper notification to all
property owners.

I am sending you a typed Memo tOday in reference to some of my concern in reference to
Meridian/Kuna Meridian Rd.
I will state at U% point on behalf of the 91 -a center we are requesting that the Rd in question be
changed back to Meridian Rd not Kuna Meridian Rd.
I a I drove Meridian'Kuna Meridian mad today, May IS "009 at approximately I prn to check out
the street signs along this stretch of the road between Overland Rd clear into Kana at East Kuna
Rd (The big curve into Kona).
Going south bound sarong at the Interstate, I saw signs pointing to Kuna and pointing to
Meridian, as well as the sign that pointed the way to Highway 69 along with Meridian Rd signs.
From that point on going south bound all major intersections had Meridian Rd signs on them
(including at the inters—mbon of Kana and Meridian Rd), All of the smaller roads that entered into
sub-divisioris also had Meridian Rd signs on, them.
There was 2 locations that I observed a Kana Meridian Rdsign and that was at, two private
th e
staeets just before getting to the big curve in Kuna, on I e w S ;ree
t side of the stmt only. Those
private streets were E bac re Ln and E Kok-an-ee Ln,
Tra,veling back north bound all street signs read Meridian Rd.

ever name) Rd,
3. We do not get a lot of phone calls from residence along this stretch of road, but they do call in
once in a while and when they do it does create a problem,

A. March 22, 2009, 911 medical emergency carne in from a resident who advised their address
was 20-00 block of N Meridian Rd. When the call was entered the call went immediatelV to the
2000 block of N Meridian Rd in Meridian. Meridian was dispatched to the call, could not find the
address, dispatcher called back found out they were in Kana, 11(caller still advised they were N
Meridian Rd) turns out this is now Kona Meridian Rd.
The rest of the examples listed below have been with in approximately the last month to month
and a halt
B. Southern Springs Dental 1910 s Meridian Rd, came up with an address in Eagle, again turns
out is was S Kuna Meridian Rd in Meridian.
C Walgreens 1625 S Meridian Rd, would not come up except as N Meridian Rd by Cherry Ln,
again turns out it Is S Kuna Meridian Rd.
D. Citizen calls in loose cows at Meridian Rd and Columbia, not Kuna Meridian and Columbia,

I had one of the ladies from worv, run a report from the cornpany who provides data into the 911
center (Enhanced 911) that shows where. a calier is calling from if they are, in fact calling from a
home phone, not a cell phone, That data is provided to them from, Q -West and Qwmt receives
that data from the resident4""busiriess or if address changes are, made they get a change cd
address notice usually from the pe -sop maildnrj the change.
When the report was -an we were able to det;rmirie from the best of our knowledge) the
