AMENDMENT No. 2
SCOPE OF SERVICES
CITY OF MERIDIAN PUBLIC WORKS (OWNER)
AND
CAROLLO ENGINEERS, A Professional Corporation (ENGINEER)
This Amendment No. 2 to Task Order No. 4, is issued by the OWNER and accepted by the
ENGINEER pursuant to the mutual promises, covenants and conditions contained in the Agreement
between the above named parties dated the 15th of November, 2004 in connection with:
CITY OF MERIDIAN
BIOSOLIDS IMPROVEMENT PROJECT
PURPOSE
The ENGINEER' scope of services, time of completion and compensation shall be amended as
set forth herein, for extension of time for Construction Period Services, for the City of Meridian,
Wastewater and Biosolids Improvement Projects.
ENGINEER's SERVICES
ENGINEER shall provide additional construction period services as described in Task Order No.
4, which shall include project management, office engineering services, contract administration,
field inspection and resident engineering.
TIME OF COMPLETION
Amendment No. 2 is estimated to extend construction services through the end of April 2009.
Record drawings will be delivered thirty (30) days following the construction period.

COMPENSATION
The engineering fees for the work described in Task Order No. 4 and Amendment No. 1 totaled
a not-to-exceed limit of eight hundred ninety one thousand three hundred dollars ($891,300).
Amendment No. 2 shall increase the total contract amount by $33,116, for a total of nine
hundred twenty four thousand, four hundred sixteen dollars ($924,416)
The ENGINEER will invoice for this monthly on an hourly rate basis in accordance with
ENGINEER' standard fee schedule in effect at the time the work is completed. Outside
services for subconsultant work and other direct costs shall be reimbursed at cost plus 10
percent mark-up. Travel and subsistence will be charged at cost.
The ENGINEER shall not exceed the amount defined for the services of this Amendment No.2
for Task Order No. 4 without written authorization from the OWNER.
EFFECTIVE DATE
IN WITNESS WHEREOF, duly authorized representatives of the OWNER and the ENGINEER
have signed in confirmation of this Task Order No. 4 with the effective date this day of
