This Task Order is issued by Owner and accepted by the Consultant pursuant to
the mutual promises, covenants and conditions contained in the Master
Agreement between the above named parties dated January 4, 2007.

PURPOSE
The Consultant's scope of services and compensation shall be as set forth herein.
Services shall generally tae described as testing and Inspections consulting services.
TASK 1-Testing and Inspections Consuttling Services
Per the City's request, the Consultant will provide professior~t testing and inspections
consulting senric~s relating to the construction of the Wetl No. 27 Pumping Facilities
Project These services are listed in Terracon's Proposal No. P62090099 dated April
30, 2009 (Attachment A).
COMPENSATION
The Not To-EExxceed amount for Task Order No. 0670d is Two Thousand Flve Hundred
Ffty Twa 80/100 dollars {$2,552.80).

VNe are pleased th submit this proposal to provide construction materials observation and
testing services for the above referenced project.
A. PROJECT UNDERSTANDINt3
It ~ our understanding the proposed Well No. 27 Pumping Facilities Project will be located on
Ten Mile Road between Pine Stree# and Franklin Road in Meridian, Idaho. The project cansis~s
of constnu~ing an approxtrnately 888 square foot building. Construction will cons~t of building
pad development, concrete foundations, slab on grade. CMU masonry walls and parking lot
pavement section devebpment.
B. SCOPE OF SERVICES
Sased on discussions with you and review of tl1e project drawings, project specifications, logl
a~nstruction pract~es and our experience an similar projects, we anticipate the scope of our
services will generally include the following:
1. Observation of contractor proof-roll activities to locate unstable areas of
subgrade.
2. Observation of the footing excavations.
3. Observation and in-place density testing of fills associated with construction of
the building pad, ufilitles, and parking areas.

We anticipate performir~ the materials testing on a ~riodlc (on cad} basis.
C. COMPEN3ATfON
Terra~con's flees for completing the scope of services described above are provided in the
attached Exhibit C. The cysts sf~vn are based upon the estimated quantifies indicated.
Addkiona~y, we have es~mated the total fee to provkle the testing services. We are avaf~ble to
discuss #hese services arxf our total fee ~ltnate with you ~ your cornrenience. Pleafrg,not~e
that you will bo blued only for I~,Ica~ nrovfded.

This proposal may ~ accepted by issuing the appn~xiate task order in accordance witf- our
Master Agresmen# for Professional services. To expedite ktitiation of work, the contract

Bode Office at 2Q&323-9.rs92. This proposal is valid ortily if authorPi~ wlth~ 6Q (sbcty) days
