Re: Flush Line Repairs
Dear Roxanne:
We are requesting a supplement to Task Order No. 0713 for the 2008 Flush Lines, to cover additional work
that was not included in the original scope of services.
1. Work effort to obtain Army Corps of Engineers 404 permit to repair the rip rap at the existing well
24 flush line.
2. Work effort to obtain Army Corps of Engineers 404 permit to repair the rip rap for the existing flush
line at the W WTP entrance.
3. Work effort to obtain Idaho Department of Water Resources stream alteration permit to repair the rip
rap at the existing wcll 24 flush line.
4. Work effort to obtain Idaho Department of Water Resources stream alteration permit to repair the rip
rap for the existing flush line at the WWTP entrance.
5. Work effort to obtain an IDEQ water quality certification for the rip rap repairs at the two above
stated sites.
Our original contract was for a not to exceed amount of $37,bb5. We arc requesting an additional amount of
$1,785.00 to cover the additional work as outlined above.
