(CONSULTANT) This task Order is issued by Owner and accepted by the Consultant pursuant to the mutual promises, covenants and conditions contained in the Master Services Agreement between the shave named parties, dated January 24, 2007. SCOPE OF SERVICES CITY OF MERIDIAN, IDAHO 8TH STREET, CHERRY TO CAMELLIA UTILITY ADJUSTMENTS AND RELOCATIONS BACKGROUND The purpose of this task order is to design utility adjustments and relocations related to the construction of continuous sidewalk on the east and west sides of W. 8s' Street between Camellia Lane and Cherry Lane in Meridian. Utility adjustments and relocations are desired by the City of Meridian in conjunction with the federally-funded sidewalk improvements. Since this related utility work cannot be fuzanced using federal funds, the City of Meridian desires to issue separate design and construction contracts to implement these utility adjustments and relocations. The sidewalk improvements are needed to improve the safety of students traveling to and from Meridian Middle School and other pedestrians traveling through the area. Improvements anticipated for W. 8a' Street will be compliant with current Americans with Disabilities Act (ADA) guidelines. Proposed sidewalk to be constructed under this project will be located behind existing curb and gutter. All existing curb and gutter will remain in place with the exception of those sections that are severely deteriorated curb and those that do not conform to proposed driveway locations. Currently, neither side of W. 8th Street exhibits continuous sidewalk between Camellia Lane and Cherry Lane. Pedestrian ramps are non-existent at the mid-block crosswalk location and are not ADA compliant at four other locations. Page 1 of 7 1[_T~ I ONE COMPANY J[ jet Mauy Solutloas`° A multi-use pathway intersects W. 8a` Street on the east side. Many students use this pathway to travel to and from Meridian Middle School. Pavement on the existing multi-use pedestrian pathway terminates approximately 50 feet east of W. 8th Street. This project will address this discontinuity by connecting the proposed sidewalk on the east side of W. 8a` Street to the existing pathway. All pedestrian improvements (sidewalk improvements) will be conducted under ITD Project No. A010(489). Utility adjustments and relocations will be conducted under a separate construction contract by the City of Meridian designed under this task order. PROPOSED SCOPE OF SERVICES Several utilities located within the project area belong to the City of Meridian conflict with proposed pedestrian improvements. It is anticipated that adjustments and/or relocations to the following facilities will be required under this project: • Adjust manhole to grade (count total = 3) • Relocate fire hydrant (count total = l) • Relocate luminaire (count total = 2) • Adjust water valvelmeter (approximate count total =12, no more than 14) The following work will be performed under this agreement. TASK 1. UTILITY ADJUSTMENT AND RELOCATION PLANS SUBTASK 1.1-INCORPORATE CITY OF MERIDIANSTANDARD DETAINS This task includes researching, retrieving, and modifying (as necessary) City of Meridian detail sheets and/or standard drawings related to utility adjustment and relocation. This task includes time necessary to coordinate with the City of Meridian. The 2008 ISPWC and the City of Meridian's Supplemental to the 2008 ISPWC will be used far details on utility adjustment and relocation. SUBTASK 1.2 - MODIFICATION OFPLAN/PROFILE SHEETS Plan sheets in the final bid package will be modified to show City of Meridian facilities that conflict with proposed pedestrian improvements to be relocated or adjusted by the City's Contractor before or during construction of the pedestrian improvements. Both existing and proposed locations will be shown and noted at each location. Adjusted manhole and water meter/valve elevations will be approximate and will require field-verification by the Contractor during construction. Deliverables: • Four (4)11 "x 1 T' dn~ft plan view sheets at 1:40 scale showing the existing and proposed utility locations and proposed pedestrian improvements to be constructed under ITD Project No. A010(489). These sheets will be derived from the ITD project sheets that are Page 2 of 7 I ONE COMPANY Many Solutions'" being prepared for the pedestrian improvements project (I1 D Project No. A010(489)) and will follow the symbology set up for that project. Up to four (4) draft detail shuts showing utility adjustment and relocation information. All details will be based upon those found in the 2008 ISPWC or the City of Meridian Supplemental to the 2008 ISPWC. Detail sheets will either be in 8.5"x11" format or 11 "x17" format. TASK 2. SPECIAL PROVISIONS Consultant shall prepare Special Provisions to provide additional information regarding City of Meridian utility adjustments and relocations. Deliverables: • One (1) set of draft special provisions for utility adjustments and relocations. TASK 3. OPIlVION OF PROBABLE CONSTRUCTION COST The opinion of probable construction cost (Estimate) will be prepared for bid items associated with relocating and adjusting Ciry of Meridian utility facilities by the Contractor. The City of Meridian agrees to 100% compensation for all City of Meridian utility modifications by Contractor shown on the Final Plans, Specifications & Estimate (PS&E) bid package. Deliverables: One (1) draft opinion of probable conshuction cost for utility adjustment and relocation items shown on the plans. TASK 4. REVISIONS TO PLANS, SPECIFICATIONS AND ESTIMATE Upon submittal of the draft plans, special provisions and estimate, the City of Meridian will review and provide a single set of written comments to HDR, consolidated to include comments from all City reviewers. HDR will modify the plans, special provisions, and opinion of probable construction cost and resubmit a final version of these documents. Only one revision of the plans, special provisions, and opinion of probable construction cost is included is this agreement. Deliverables: Four (4)11"x17" final plan view sheets at 1:40 sole showing the existing and proposed utility locations and proposed pedestrian improvements to be constructed under 1'TD Project No. A010(489). These sheets will be derived from the ITD project sheets that are being prepared for the pedestrian improvements project (ITD Project No. A010(489)) and will follow the symbology set up for that project. Up to four (4) final detail sheets showing utility adjustment and relocation information. These sheets maybe taken directly from the 2008 ISPWC, the City of Meridian Supplemental to the 2008 ISPWC, or created by HDR. Detail sheets maybe in 8.5"x11" format or 11"x17" format. • One {1) set of final special provisions for utility adjustments and relocations. QUALITY CONTROL This task includes a quality control review of the utility adjustment and relocation plans, specs and estimate. Deliverables: N/A TASK 6. SUPPORT DURING CONSTRUCTION HDR will pmvide technical guidance in the form of responses to Contractor RFI's during bidding and construction. A maximum of 4 hours is budgeted for this task. Deliverables: Responses to Contractor RFI's, as necessary. TASK 7. ADMINISTRATION This task includes administrative duties associated with setting up the contract with the City of Meridian, preparing and submitting invoices for this work. HDR will submit invoices directly to the City of Meridian. Deliverables: • Up to three (3) monthly invoices submitted to the City of Meridian. Invoices will consist of a memo, invoice summary sheet and log of hours spent during that billing period, CITY OF MERIDIAN RESPONSIBILITIES The City of Meridian will provide standard drawings (utility adjustment and relocation details} in CAD format to the Consultant for inclusion in the bid package to the Contractor. The City of Meridian will provide comments on the draft PS&E submittal in written format. The City of Meridian will be responsible for hiring a Contractor to perform the adjustment and relocation of their utilities.
