B GARDEN CITY, ID. 83714 378-8167 FAX 378-9465 September 16, 2008 INTERIM 30 DAY CONTRACT PROPOSAL & AGREEMENT Standard .Janitorial, Floor Maintenance Meridian City Hall 33 East Idaho Ave. Meridian, ID 83642 Building Location: 33 East Broadway Meridian, ID 83642 Standard janitorial consists of nightly, weekly, bi-weekly, and monthly duties. These duties will be performed after working hours. Duties are as follows. JANITORIAL Nishtly Duties Bathrooms; • Dump all trash cans, replace liners clean as required • Clean and sanitize toilets and urinals • Clean and sanitize patrician walls and doors • Spot clean walls • Clean and•sanitize sinks and counters • Polish all chrome fixtures • Re-stock toilet paper, hand towels, soap dispensers, toilet seat covers, feminine hygiene products (to be provided by contractor and billed to the client unless otherwise provided) • Clean mirrors • Spot clean doors and door ways , • Sweep and mop floors with disinfectant Break rooms; • Dump all trash cans, replace liners clean as needed • Clean and disinfect all counters and tables • Clean microwave • Clean sink • Polish all bright work • Spot clean cabinets and refrigerator 1 • Spot clean doors and' door ways • Sweep and mop floors with disinfectant (if applicable) • Vacuum all carpeting (if applicable) • Spot clean carpeting for normal wear up to one foot in diameter and does not include damage or restoration repairs. Offices, interior common areas, and waiting rooms; • Clean entry windows, inside and outside (interior glass) • Dump out side ash cans (if applicable) • Dump all trash replace liners, clean as needed • Clean and sanitize all counters and tables • Wipe and spot clean all desks tops of dust and fingerprints as accessible • Arrange magazines (if applicable) • Vacuum all carpets _ • Spot clean carpets_for.normal wear_up_to_one_foot_in.diameter_and- _ does not include damage or restoration repairs. • Spot dust (as needed) • Spot clean doors and door ways • Spot clean vending machines (if applicable) • Clean water fountains • Spot clean partition windows • Sweep and mop hard floor surfaces with disinfectant Building Entry; • Clean entry glass • Wipe thresholds and door frames • Vacuum entry mats and spot clean for normal wear not to include damage or restoration repair Building Main Floor Lobby; • Vacuum carpet in lobby and spot clean for normal wear not to include damage or restoration repair. • Dust all ledges and fixtures • Dust mop/Wet mop lobby floor Elevators/Stairwells/Basement; • Vacuum all carpets (if applicable) and spot clean for normal wear and does not include damage or restoration repairs • Wipe down elevator doors, control panels, and floor indicator plates. • Clean elevator door tracks and saddles on all floors (as needed) • Spot clean interior elevator walls (as needed) Weekly duties: • Sanitize light switches, phones, and door handles • Clean door kick plates • Spot clean interior walls 2 • Spot clean as needed all unoccupied areas • Wipe, sanitize, and clean all desk surfaces where cleazed on a specified day per week Monthly duties; • Vacuum all edges • High dusting, lights, fans, vent, HVAC registers, and ceilings up to twelve feet, etc. • Dust horizontal surfaces, file cabinets, pictures, etc. • Dust low surfaces, chair legs, etc. • Dust window blinds • Clean baseboards and door kick plates • Thoroughly clean all unoccupied areas ._~_____Pro Power Clean wll_perform the above- duties 5 _nights_per_ week- _ __ Sunday through Thursday, at .08 cents per square foot on approximately 100,000 square feet for a total of: $ 8,000.00 uer month based on an interim 30 day agreement . All product, toilet tissue, hand towels, hand soap, trash can liners, etc., is at an additional cost, billed monthly along with the janitorial billing unless otherwise provided by the client. FLOOR MAINTENANCE A hardJloor surface maintenance schedule will be provided in the "Permanent Contract Proposal" at time of submission. On the interim Pro Power Clean Inc. will provide the following at no additional charge 1. Pro Power Clean will deep scrub and apply three coats of Butchers Iron Stone Sealer and two coats of Butchers Cleaz Reflections Wax to all of the VCT/Vinyl flooring to properly prepaze the surfaces for on going daily maintenance. 2. Pro Power Clean will deep scrub and clean all ceramic the and grout joints on all floors to properly prepare the surfaces for on going daily maintenance (if needed). WINDOW CLEANING Pro Power Clean Inc. will provide a window cleaning service for the above referenced building which is a complete interior and exterior cleaning of the entire building twice a year price to be provided in the "Permanent Contract Proposal" ADDITIONAL SERVICES Pro Power Clean will pressure wash any areas including but not limited to the building(s) exterior walls, floors, sidewalks, and drives for an additional charge upon request or on a scheduled service agreement price to be provided. Pro Power Clean will perform carpet cleaning in any and all areas for an additional charge upon request or scheduled service agreement price to be provided. Other Maintenance and Janitorial services not provided in this Proposal may be available upon request This agreement is an Interim. Contract for 30 days and can 6e extended in 30 day increments for a period up to 90 days with 7 days notice. All Pro Power Clean employees are certified, insured, and bonded. Proof of insurance and coverage's will be provided prior to the commencement of this agreement. Provisions 1. This is a month to month agreement up to 90 days. 2. Any changes, additions, deletions, or modifications to this agreement will require a written addendum signed by both parties. 3. The above referenced services represented the totality of the understood duties to be performed. Any additional services or duties are subject to an additional charge. 4.
