The Public Works Departrnent respectfulfy requests the following item be placed on the
6/24/2008 City Council agenda, under Consent Agenda, for Council's consideration:
Chanpe Order No. 1 for Black Cat Road Water Main Extension Phase 1& 2(Construction)
Additional worlc is required for the completion of the Black Cat Road Water Main Extension
Phase 1& 2(Constructionl This change order consists of the following work and amounts to
6% of the original construction contract:
• Additional water service stub asse"mblies, fire hydrants, sewer main, and irrigation
crossings added to the project.
Recommended Council Action: The Public Works Department
recommends that City Council approves Change Order No. 1 for the Black
Cat Water Main Extension Phase 1 8 2(Construction) project with Civil
Brown Construction, Inc. for $44,164.50 and authorize the Mayor to sign it.
