The following additional services have been requested for the above referenced project. 'I~ese changes
were not included in the original estimate and require additional time to complete. As a result we are
asking for an additional $4,SOU.00 (Four Thousand Five Hundred Dollars) to complete the design.
Project changes;
• Relocation of five PRVs.
• Added four additional analog points to be monitored per site, requiring a redesign of the PLC
and radio system.
