I, Tahri Molifaa, as an Executive Vice President of BV Executive Management, Inc., an Idaho corporation
which corporation is the Manager of, and for and on behalf of, BVB Ten Mile Crossing Annex, LLC
("BVB"),PO Box 51298, Idaho Falls, Idaho being first duly sworn upon, oath, deposes and says:
1. That BVB is one of three tenant-in-common record owners of the property described on the attached,
and BVB grants permission to:
Thomas Wong on behalf of The Habit, Commercial Development Resources
----to-submit the accompanying application(s)pertaining-to-that property.
2. BVB agrees to indemnify, defend and hold the City of Meridian and its employees harmless from
any claim or liability resulting from any dispute as to the statements contained herein or as to BVB's
tenant in common ownership of the property which is the subject of the application.
3. BVB hereby grants permission to City of Meridian staff to enter the subject property for the purpose
of site inspections related to processing said application(s).

BV Executive Management, Inc., which corporation is the Manager of BVB ten Mile Crossing Annex, LLC,
and the Manager who subscribed said limited liability company name to the foregoing instrument, and
acknowledged to me that such corporation as the Manager executed the same in said limited liability company
name.
