AGREEMENT
FOR
LODGING SERVICES
THIS AGREEMENT FOR TEMPORARY LODGING SERVICES is made this
8th day of December ,2006, and entered into by and between the City of
Meridian, a municipal corporation organized under the laws of the State of Idaho,
hereinafter referred to as "CITY", 33 East Idaho Avenue, Meridian, Idaho 83642, and
Mr. Sandman Motel, hereinafter referred to as "MOTEL", whose business address is
1575 South Meridian Road, Meridian, Idaho 83642.
INTRODUCTION
Whereas, the CITY has offered to pay on behalf of certain individuals, a
maximum 5 consecutive night stay at the motel listed above; and
WHEREAS, the MOTEL has agreed to accept the City's Hotel Vouchers
and has agreed to provide such temporary housing services;
NOW, THEREFORE, in consideration of the mutual promises, covenants,
terms and conditions hereinafter contained, the parties agree as follows:
TERMS AND CONDITIONS
1. Scope of Services:
1.1 MOTEL shall perform and furnish to the Voucher Recipients, upon
execution of this Agreement and receipt of the City's Motel Voucher, a
maximum of 5 nights stay. A sample of the voucher agreement is
attached to this Agreement as exhibit A.
1.2 The MOTEL shall provide services under this Agreement consistent
with the requirements and standards established by applicable federal,
state and city laws, ordinances, regulations and resolutions.
2. Consideration, Time of Performance
2.1 The MOTEL shall be compensated on a per night basis as
provided below. This agreement shall be for one (1) room per voucher,
Five (5) nights per voucher. The maximum number of rooms that may be
provided under this agreement is five (5) but the exact number of rooms to
actually be requested is unknown at the time of execution of this
agreement.

2.2 The expected time of performance shall be for a five night
consecutive period beginning on December 8,2006 (check-in date), and
ending on the morning of December 13, 2006 (check-out date).
2.3 The MOTEL shall provide the City with a consolidated billing
statement for all rooms provided under this agreement along with a
signed copy of each executed voucher. . Vouchers and this Agreement
shall expire on December 13, 2005. Any additional services rendered
including but not limited to Long Distance Phone service, and Pay Per
View movies not included in the room fee listed below will be at the
occupant's expense. Any night's stay beyond the check-out date of
December 13, 2005 shall be at the occupant's expense.

3. Indemnification:
MOTEL shall indemnify and save and hold harmless CITY from and for
any and all losses, claims, actions, judgments for damages, or injury to
persons or property and losses and expenses and other costs including
litigation costs and attorney's fees, arising out of, resulting from, or in
connection with the performance of this Agreement by the MOTEL, its
servants, agents, officers, employees, and guests.

Date of issue: December 8, 2006
This voucher entitles to one (1) room for a maximum five (5)
consecutive nights stay at the Mr. Sandman Motel located at 1575 South Meridian Rd., Meridian, 208-887-2062.
This voucher is effective on the date of issue and expires at 11a.m. on December 13,2006. ANY STAY
EXCEEDING THE 5 CONSECUTIVE NIGHTS WILL BE AT THE EXPENSE OF THE INDIVIDUAL LISTED
ABOVE. The individual listed above shall also be responsible for any additional charges above and beyond the
standard room rate including but not limited to room service, long distance phone charges and pay-per-view
movies. The City does not guarantee any standard of service nor insure any loss of personal items during your
stay. This voucher does not create a "tenancy" or any other rights beyond the payment for the motel services
stated herein.
By signing below you are agreeing to all the terms and conditions of this Motel Voucher, you are verifying that you
were a resident of 1535 N. Main Street Meridian as of December 8, 2006 and you are verifying that the
identification and proof of residency are true and correct.
