Annexation, Rezone, and Conditional Use Permit Applications
Dear Commissioners and City Council Members,
Please accept the above referenced applications for the proposed Luna Hospice facility located at 525 E.
Overland Road on behalf of our client, Luna 2 Hospice.We are requesting annexation with R-8 zoning on
0.77 acres and a Conditional Use Permit to operate as a hospice facility with 14 beds.
Annexation and Rezone
The property is currently zoned Estate Residential(R1) in Ada County. It is within the City of Meridian
Impact area and is adjacent to Meridian City limits on the north and south.The Meridian Future Land Use
Map designation for the subject property is"Medium Density Residential". The requested zoning of R-8
conforms to the Future Land Use Map.
The properties to the east and west are currently zoned Estate Residential(R1) in Ada County. The Meridian
Future Land Use Map designates these properties as"Medium Density Residential".
The properties to the south are currently within the Meridian City limits and are zoned R-4.The properties to
the north are also within the Meridian City limits and are zoned C-G.
The subject property is one parcel nearly surrounded by the City of Meridian.Annexation into the City of
Meridian will create continuity in jurisdiction and will support the orderly development of the City.
Additionally, City water and sewer services are currently provided to the property and are readily
accessible.
Conditional Use Permit
This application requests Conditional Use for the development of a hospice (residential care)facility in the
proposed R-8 zoning designation.The subject property is located directly on E. Overland Road and the
proposed use would provide an appropriate transition between the commercial to the north and residential
to the south.

The proposed improvements would be to renovate the existing residential house and construct an addition
that would total 14 hospice care beds.The total square footage of the buildings would be about 4,700
square feet.The development would comply with all regulations and codes in effect, including UDC 11-4-3-
29.
