Conditions of Approval:
Project is subject to all cun-ent Meridian City Unified Development Code (UDC)and conditions of
approval for the Touchmark/Meadowlalœ Subdivision. The issuance of this permit does not release
the applicant from any requirements ofthe approved Meadowlake Subdivision Conceptual Planned
Development, the Wigod Medical Office Conditional Use Permit (CUP-05-043) and the subsequent
Development Agreement. The property is located in a L-O Commercial zone where Clinics
(medical, dental, and optical) are permitted.
Landscaping: The Landscape Plan prepared by South Landscape Architecture PC. on September 14,
2005, Sheet L1.0, is approved (stamped "Approved" on October 19,2005 by the Meridian Planning
& Zoning Department) with no changes. The approved landscape plan is not to be altered without
prior written approval of the Planning & Zoning Department. No field changes to landscape plan
permitted; prior written approval of all material changes is required.
Upon complete build out of the site the developer shall install approved trees within the pro?osed
planters in accordance with UDC 11-3B. The approved landscape plan is not to be altered without

Site Plan: The Site Plan labeled A-I.l, prepared by ZGA Architects on September 14, 2005, is
approved (stamped "Approved" on October 19, 2005 by the Meridian Planning & Zoning
Department): The approved site plan is not to be altered without prior written approval of the

Planning & Zoning Department. NOTE: the required Fire Department approval could require
changes to the approved site plan. If changes are made a new site plan must be approved by the
Planning Department.
lITigation: An underground, pressurized irrigation system must be installed to all landscape areas per
the approved specifications and in accordance with UDC 11-3A-15.
Curbing: Per UDC 11- 3B-51, all landscape areas adjacent to driveways, parking lots, or other vehicle
use areas, must be protected by curbing, wheel stops, or other approved protective devices. Curbing
may be cut to allow for storm water runoff.
Protection of Existing Trees: Any existing trees on site must be protected in accordance with the
Tree Preservation section of the City's Landscape Ordinance. Per UDC 11-3B-10, coordinate with
the Parks Department Arborist (Elroy Huff) for approval of protection measures prior to
construction. Any severely damaged tree must be replaced in compliance with UDC 11-3B-1 0-C.5.
Parking: The parking area shall be paved and striped in accordance with UDC 11-3B. Project
engineer/architect shall certify that the number and size of handicap-accessible spaces conforms to
the Americans with Disabilities Act (ADA). Handicap accessible stalls must have signage in
accordance per ADA and signed "Van Accessible". The site plan indicates 11 parking stalls and 2
handicap stalls along the front building line. UDC 11-3C-2 requires one space per 500 square feet in
commercial districts. Staff is reviewing the building at 4,600 sq/ft of commercial use. 4600/500 =
10 required stalls. The applicant has proposed the entire perimeter parking stalls to be 18'6"x 9'
which are not shown as compact as the front overhang will be in vegetated areas. Also, there are two
parking stalls and the two handicapped stalls which do not overhang the sidewalk accessing the
building and additional sidewalk width is not required.
The applicants engineer shall certifY that all drive aisles meet the minimum standards of the
Meridian Fire Department for turning radiuses. An 8.5 x 11 site plan stamped approved by the
Meridian Fire Department shall be submitted prior to construction.
Sidewalks: All sidewalks shall be constructed in accordance with 11-3A-17. Sidewalks shall be
constructed prior to occupancy.
Drainage: Storm water drainage swales shall not have a slope steeper than 3: 1, shall be fully
vegetated, and shall be designed in compliance with UDC 11-3B-11 and UDC 11-3A-18.
Fencing: Any required or proposed fencing shall be installed with current fencing standards as
defined in UDC 11-3A-7
Lighting: Lighting shall not cause glare or impact the traveling public or neighboring development
and comply with lighting standards as defined in UDC 11-3A-11
Signage: No signs are approved with this CZc. All signs will require a separate sign permit in
compliance with UDC II-3D.
Trash Enclosure: Any dumpster(s) must be screened in accordance with UDC-11-3A-12. Trash
enclosures must be built in the location and to the size approved by SSC.
Handicap-Accessibility: The structure, site improvements and parking must be in compliance with
all federal handicap-accessibility requirements.

ACHD Acceptance: All impact fees, if any, shall be paid prior to the issuance of a building permit.
If any changes must be made to the site plan to accommodate the ACHD requirements, a new site
plan shall be submitted to the City of Meridian Planning and Zoning staff for approval prior to the
issuance of a building permit.
Certificate of Occupancy: All required improvements must be complete prior to obtaining a
Certificate of Occupancy. A Temporary Certificate of Occupancy may be obtained by providing
surety to the City in the form of a Letter of Credit or cash in the amount of 110% of the cost of the
remaining improvements. A bid must accompany any request for Temporary Occupancy.
Plan Modifications: The approved Site Plan and Landscape Plan stamped "Approved" on October
19, 2005 and are not to be altered without prior written approval of the Planning & Zoning
Department. No significant field changes to the site or landscape plans are permitted; prior written
approval of all changes is required.

by Wigod Properties, LLC.
I am a solo Plastic Surgeon. I plan to build a building for the use of my
private practice, as well as any future partners I may have. The building
will be approximately 4500 square feet on approximately 21,000 square
feet of land. Inside will be facilities for a clinic, ambulatory surgical suite,
and aesthetician services. These proposed uses have been reviewed by
Touchmark and St. Lukes Medical Center and neither party has any
objections.
