INCOMPLETE APPLICATIONS WILL NOT BE PROCESSED Description Document Naming Convention Narrative fully describing the proposed change(s) including the following:  Note the approved project number and project name  The specific requirement(s) that is proposed to be modified  Verify the item(s) was not a topic of discussion at a public hearing  A statement of conformance that the proposed change(s) complies with the approved project’s conditions of approval and all applicable requirements of the UDC Narrative Recorded warranty deed for the subject property Warranty Deed Affidavit of Legal Interest signed and notarized by the property owner (If owner is a corporation, submit a copy of the Articles of Incorporation or other evidence to show that the person signing is an authorized agent) Affidavit of Legal Interest Submit an electronic copy of the approved landscape plan that is proposed to be modified Approved Landscape plan Submit an electronic copy of the revised landscape plan (i.e. fencing and amenities etc.) Proposed Landscape plan
