Dear Stacey:
Enclosed please find a copy of your renewal policy we received from ICRMP for
your October 1, 2005 renewal date. Also enclosed please find the Signature
Page for the renewal confirmation and the Joint Powers Subscriber Agreement.
Please be so kind and sign the attached pages were indicated and return to our
office in the envelope provided.
Please take a moment to review the policy for accuracy. If you find any changes
or corrections that may be necessary, please do not hesitate to contact our
office.

ICRMP Renewal 2005-2006
Enclosed are renewal documents that require your attention for:
Meridian Develo~ment Corporation
1. Renewal Signature Page
2. Joint Powers Agreement
3. Renewal'Certificates of Insurance
4. Pesticide Applicator Certificate, if applicable to your member
5. Summary of Policy Changes
6. Vehicle Proof of Insurance
7. Policy and Declarations Pages
Please meet with the authorized governing board to present the policy, the new Joint
Powers Agreement and all other related documents. Obtain a signature on the Signature
Page for Renewal Confirmation and return the document to our office by October 1, ,
2005. All other documents can be retained by the member. Commissions will be paid
once this fonn is returned and the member has paid their member contribution.
Renewal certificates of insurance for all of your members' propèrty that has a certificate
holder listed on the various schedules are attached. We have mailed the original'to you.
Please make a copy for your office and forward one to the corresponding certificate
holder for their records.
If applicable to your member, we have also included the Idaho Pesticide Applicator
Certificate of Insurance as required by the Idaho Department of Agriculture. We have
mailed the original to the Department for their renewal records. "
Deliver the policy to the member and if you need copies. both the declarations pages and
the policy itself can be downloaded from ICRMP website's eAgent database. A master
vehicle identification card is included with this packet; however, you can also download
this from eAgent.
