The owner of the property for which such annexation/rezone is proposed shall file an application for
annexation/rezone with the Planning & Zoning Administrator. The application shall contain the following
information:
1. Completed and signed application form.
2. Proof of title of said owner (warranty deed).
Notarized consem from titled owner of property. (If owner is a corporation, provide a copy of the
Articles of Incorporation or other evidence to show that the person signing is an authorized
representative)
Legal description and map (metes & bounds) of subject property, including all adjoining rights-of-way,
railroads, roadways, highways and easements the full length of the property which is signed and stamped by
a land surveyor registered in the State of Idaho. Map shaH reflect the boundaries as described in the legal
description, including all descriptive call outs.
5. A statement describing the characteristics of subject property which make the zoning amendment desirable.

A statement outlining the necessity or desirability of development pertaining to the zoning amendment and its
harmony with adjacent development.
A statement of how the proposed zoning amendment relates to the City of Meridian Comprehensive Plan
(please cite policy numbers4,-
8. One (1) map at a scale of one (1) inch equals one hundred (100) feet of the property concerning the zoning
amendment.
One (1) copy of a vicinity map at a scale of one inch equals three hundred feet (l"=300'). This map can be
obtained from the Planning & Zoning Department upon request. Please request this map seven days prior to
submitting application.

A list of the mailing addresses of all property owners within three hundred feet (300') of the external
boundaries of the land being considered. This list must be obtained from the City of Meridian Planning &
Zoning Department. Please request list seven days prior to submitting application.
11.
A fee established by the Council:
1 acre or less = $550.00.
Over 1 acre = $550.00 plus $15.00 for each additional acre or portion thereof.
Staff prepared Consensual Annexation Application for properties receiving sewer & water
service = $1,100.00 + $15.00 per additional acre or portion thereof.
12. A signed affidavit stating that the property will be posted I week before the public hearing. Posting must
contain name of applicant, description of zoning amendment, and time and date of public heating. Official
notices are available at City Hall or the Planning & zomg Department.
13. After the property has been posted the applicant shall deliver to the Zoning Administrator a notarized
statement that he has posted the property and the date the posting was placed. (If two public heatings are
required, please submit a notarized statement for each hearing)

BEGINNING at the northeasterly comer of said SE ¼ of the SE ¼;
Thence S 0°16'38" W along the easterly boundary of said SE ¼ of the SE ¼ a distance of
220.44 feet to a point;
Thence leaving said easterly boundary S 89o59'04'' W 186.76 feet to a point;

A Resolution of the Board of Commissioners of the Meridian Rural Fire Protection
District, setting forth certain findings and purposes; authorizing the Chairman and Secretary to
sig~ and enter into the THIRD ADDENDUM TO EXHIBIT "C" OF CITY OF MERIDIAN
RURAL FIRE PROTECTION DISTRICT FIREFIGHTING AND LIFE PRESERVATION
SERVICE CONTRACT AND JOINT EXERCISE OF POWER AGREEMENT, a copy of which
is attached hereto as Exhibit "A," and by this reference made a part hereof, and BARGAIN AND
SALE DEED, a copy of which is attached hereto as Exhibit "B," and by this reference made a
part hereof.
WHEREAS, it is in the best interest of the Meridian Rural Fire Protection District to join
with the City of Meridian, and enter into the THIRD ADDENDUM TO EXHI]3IT "C" OF CITY
OF MERIDIAN RURAL FIRE PROTECTION DISTRICT F[REFIGHTING AND LIFE
PRESERVATION SERVICE CONTRACT AND JOINT EXERCISE OF POWER
AGREEMENT and BARGAIN AND SALE DEED, copies of which are attached hereto, in
order to transfer an undivided 74% interest, as tenant in common, in 'that certain real property
located at 3545 North Locust Grove Road, Meridian, Idaho, and more particularly described
therein, in exchange for payment from the City of Meridian in the amount of 74% of the
purchase price paid by the Meridian Rural Fire Protection District to obtain said real property.
NOW, THEREFORE, be it resolved and the Board of Commissioners of the Meridian
Rural Fire Protection District hereby resolves as follows:
The Chairman and Secretary are hereby authorized to join with the City of Meridian
and to enter into the THIRD ADDENDUM TO EXHIBIT "C" OF CITY OF
MERIDIAN RURAL FIRE PROTECTION DISTRICT FIREFIGHTING AND
LIFE PRESERVATION SERVICE CONTRACT AND JOINT EXERCISE OF
POWER AGREEMENT and BARGAIN AND SALE DEED, copies of which are
attached hereto, with the City of Meridian and to bind the Meridian Rural Fire
Protection District to the terms and conditions therein, in exchange for the payment
referenced herein.

Deed to said real property, more particularly described as follows:
