In regards to the above, referenced project, please review the attached design documents.
Hopkins Financial Services,.Inc. is proposing a Commercial and Residential Planned
Unit DevelOpment. The apartment complex will provide a~tmnsition area between the
commercial along FairvieW Avenue andthe single family residential neighborhood to the
north. Traffic will .be designed to take direct access to Fairview Avenue. The proposed
project does not intend on connecting to North Teare Avenue or Clarene Street except :for
emergency vehicle access only. We have submitted a traffic proposal to Ada County
Highway District to install a traffic signal at the main intersection.
The Fairview Lakes apartments will be phase one of a two phase project. The proposed
apartment complex will consist of 192 apartment flats. Each building will contain 12
units and will.be 2¥2 stories.high. The projected mix ofunits will be 132 one bedroom
units. 36 two bedroom one bath and 24 two bedroom: Each apartment will have one
carport and one open parking space. The complex will have a Clubhouse with
management services, exercise room and a swimming pool. The Jackson Drain Will be
relocated through the middle of the complex and'wil!. provide a natural Open amenity.
The developer agrees to pay for any additional sewer, water or trash use associated with
the developement of the apartment complex.

known as 826 Fairview Ave.; Meridian, Idaho 83642 have entered a Purchase and Sales
Agreement with Hopkins Financial.Services, Inc. Thus, I authorize Hopkins Financial
Services, Inc. to perform the following:
1) I, William Curtis, authorize Hopkins Financial services, Inc. to act as my agent
with the City of Meridian and any other governmental agency to submit
preliminary plat for approval for the potential development of this site.
2)
I, William Curtis, authorize the City of Meridian and any other governmental
agency to withdrawal, revoke, and/or cancel the preliminary plat approval on the
stated property as of this date. In addition, I withdraw my original request for
annexatiOn,
3)
I William Curtis, permit Hopkins Financial Services, Inc. to place no more than
two (2) advertising signs on the property for the purposes of marketing the
potential project with their respective realtor. The signs are to be no greater than
12Ft. X 8Ft. in dimensions.
