The applicant shall be responsible for application and compliance with any Section 404 Permifting that may be required by
the Army Corps of Engineers.
DRAWINGS OF RECORD AND CONSTRUCTION COSTS:
The Engineeris responsible to keep a record of changes to the approved construction drawings. These changes shall be
noted on the plans and labeled, "Record Drawings", by the Project Design Engineer. These "RECORD DRAWINGS"
should be delivered to the Public Works Department within 30 days after construction is completed and pior to
Certificates of Occupancy being issued.
A PROPOSED set of RECORD DRAWINGS shou/d be submifted to the Engineeing Depaftment pior to running all
required documents. The possibility exists that changes may be needed for them to be accepted.
Projects will not be accepted as final until Record Drawings and Construction Cosfs are submitted.

GENERAL CONSTRUCTION COMMENTS
Contractor shall notify the Meridian Public Works Dept. (898-5500) when materials are on site. Call must be made a
minimum of 24 hours prior to construction.
4. The Contractor pouring sidewalks shall stamp the water and sewer service locations
5. The Contractor installing sewer or water pipe shall repair any damages incurred during the installation of Joint
Trench utilities.
6. The Contractor installing sanitary sewer and water shall install sleeves across the roadways to carry pressurized
irrigation piping. The Meridian Public Works Department and the Ada County Highway District shall inspect the
sleeve installation.
7. Waterand Sewer Service lineending locations shall be marked with finder wire and a 2" x 4" or 3" wooden post. The
locater post shall be set a minimum of4 ft. above ground and painted green for sewer and blue for water.
8. Potable . and Eighteen Non-Potable (18) inches service of vertical lines are separation now subject must to be all maintained DEQ crossing/between separation all main issues. and
service lines.
SEWER CONSTRUCTION
1. A minimum of 12" of clean chips required over pipe, 4" under pipe.
2. Maximum number of grade rings allowed on sewer manholes is 12 inches.
3. Maximum depth of sewer service at property line is 5 feet. Service lines shall be extended a minimum of 10 feet
past the property line or back of sidewalk.
4. An air pressure test and TV inspection is required after all underground utilities have been installed. Please schedule
testing a minimum of 24 hours in advance. A visual inspection of the sewer manholes and lines will be made after
final paving is completed to confirm alldebris has been removed. The Contractor is requested to do a walk through
(after paving) before calling for an inspection.
WATER CONSTRUCTION
'l . A minimum of 12" of clean sand required over pipe, 6" under pipe.
2. No one is to operate any water valves except the l\4eridian Public Works Depa(ment. Notify your assigned lnspector
when the operation of any water valve is required.
3. Continuity testing on the locating wire is now required by the Contractor. lt must be done in the presence of the
Public Works lnspector.
4. The City of l\4eridian now requires a minimum 2 hour pressure test on the water system. Please schedule testing a
minimum ofz{ 'qt hours in advance.
5. The Contractor shall disinfect, flush and obtain an appropriate number of water samples from the system for
bacteriological testing. Disinfection is limited to 5 days maximum. Sampling and testing shall follow immediately.
Samples must be taken in the presence of the Public Works lnspector, who willthen take possession of the samples
and let you know what the results are.
6. ln paved areas, blow offs shall have a maximum of (1) 4" and ('1) 2" grade
ring and shall have a concrete collar
installed.
7. Contractor will obtain permission from or notify appropriate entities (such as irrigation district and ACHD) prior to
any flushing. Contractor must make a reasonable effort to prevent damage to other projects by their flushing and
to be aware of the potential for flushing by others that will affect their project and plan
for such an event.

MISCELLANEOUS COMMENTS . PRECONSTRUCTION MEETING

ACCEPTANCE OF RECORD / ELECTRONIC DRAWNGS
Within thirty (30) days of completion of constructlon, before Building Occupancy Permits are issued,
or prior to finat payment for Capital Proiects, the Design Engineer shall submit a complete, updated set
of Record / Electronic Drawings of the project to the Public Works Deparlment / Engineering Division.
The Community Development and Public Works Depaftments encourage Consultants to submit a sel of
proposed Record Drawings to ensure accurate compliance prior to
generating copies. The Record Drawings
shall be reviewed for accuracy and may be rejected if changes to the original construction plans have not been
addressed.
MINIMUM
Following are the City of Meridian's minimum requirements for approval of Record / Electronic

Record Drawing submiftals shall show the following surveyed and adiusted data:
1. Sewer manhole rims and t-type cleanout rims and inverts are to be located horizontally and vertically.
Design elevations (rims and inverts) in the profile shall be crossed through and the revised (invert and
rim) elevations added. Design grades shall be recalculated to show installed grade. These revised
grades shall be shown with revision clouds. Changes in types and lengths of pipe must also be shown.
2. Domestic water fire hydrants, water valves, air vac stations, prv stations and blowoff locations are to be
located horizontally and vertically. Known changes in types and lengths of pipe or any other changes to
the domestic water system must also be shown.
3. Recycled water fire hydrants, water valves, air vac stations and blowoff locations are to be located
horizontally and vertically. Known changes in types and lengths of pipe or any other changes to the
recycled water system must also be shown.
4. Any other changes to the construction drawings should be noted and shown with a revision cloud.
5. All surveyed points (as noted above) are to be shown on a separate layer in the electronic AutoCAD file.
Record Drawing submittals shall be as follows: (See specifics on back of page)
1 . Three (3) complete copies (Zerox or Blueline).
2. Two (2) electronic copies on CD of the above plans in the following formats.
a. City of Meridian - Record Drawing Submiftal:
One (1) AutoCAD drawing format. Coordinate the formatting with Public Works.
One (1) complete set of drawings in PDF format.

Per the provisions in Idaho Code $
39-118(2Xd), the City of Meridian has retained the services of
CENTRA Consulting, Inc. as its Qualified Licensed Professional Engineer (QLPE), to verifo
that the
plans and specifications proposed for the above-referenced project comply with the IDEQ Facility and
Design Standards (IDAPA 58.01.16 and 58.01.08) and the applicable Meridian standards, and to
approve the project for construction. By virtue of their attached letter, they have reviewed and
approved the above referenced plans and specifications as complying with the Facility and Design
Standards, and Engineering Standards of Care as it relates to the reviewed elements.
The above referenced project complies with the City of Meridian's current Wastewater Facility Plan,
and Sewer Master Plan, and our system has adequate capacity to accept sewage wastes for treatment.
These determinations have been made by use of computer models developed for our Sewer Master
Plan. This project is also within the City's urban service planning boundary. Our Domestic Water
distribution system can provide water service and the minimum required fire flow (1000 GPM) and the
minimum required residual pressure of 20 PSI. These determinations have been made by use of
computer models developed for our Water Master Plan.
The City of Meridian will own and operate the wastewater collection and domestic water distribution
systems within this project upon final inspection and acknowledgement by formal acceptance letter. In
essence, the City of Meridian "Will Serve" the project.
Attached for your records is a complete and accurate copy of the plans and specifications reviewed in
connection herewith, stamped "Approved for Construction."

allowed under ldaho Code 39-1'18(2Xd). The completed "Qualified Licensed Professional Enganeer (QLPE) Review
and Approval Checklist" (QLPE Checklist) is attached. The QLPE Checklist was prepared to
meet the transmittal
letter requirements specified in IDAPA 58.01.08.504.03(b) and 58.01 .16.400.03(b). The planswere marked as
"Approved for Construction".
Please note that the certifications required for Sections ll(B) and (C) of the QLPE Checklist must be satisfled with a
will-serve letter and declining balance statement from the City.

o This checklist is optional to use and is guidance from DEQ to qualified licensed professional
engineers (QLPE). This checklist describes the required elements of a QLPE

V. NOTE ON RULES AND STANDARDS
This checklist addresses the majority of common items from the Idaho Rules for
Public Drinking Water Systems (IDAPA 58.01.08), the Wastewater Rules (IDAPA
58.01.16, for QLPE approval of projects. However, this checklist is not all-inclusive,
and users are expected to fully understand the rules and standards, apply them
where necessary, and request interpretations from DEQ if there are any questions.
DEQ regional offices may have additional written information that will assist in the
design/approval process.
