I.
The Lot I Block 2 of Touclnnark Living Centers Subdivision FP-02-006 requires a lot line
adjustment prior to issuance of any building pennit or certificate of zoning compliance.

Fire Lanes: Due to the proposed accesses to the buildings, the fire department is requesting
placement of additional hydrants onsite. StaIT notes that the proposed site plan (AI.! dated
12.15.04) shows no hydrants on the site. A hydrant shall be installed along Louise Drive and
another along the access lane in the vicinity of the staff parking lot north of the building as deemed
by Joe Silva on 1.21.05.

Parking: A cross access agreement for access to the north and east of the site shaIl be filed prior to
issuance of any building pennit or certificate of zoning compliance.

Setbacks: The site plan depicts the proposed setbacks for the project. StaITrecommends approval
of the proposed setbacks for the project. See conditions #1 and 2 below.
CONDITIONS OF APPROVAL

Setbacks are approved as depicted on the site plan (AI.! dated 12.15.04). The minimum parking
setback shaIl be 0' feet to the north and the project layout shall remain in substantial compliance
with the approved site plan.
Parking is approved as depicted on the site plan. Minor modifications can be made, if necessary,
during the CZC process as long as the overaIl parking ratios remain in conformance with the
Meridian City Code. rfupon future redesign of Block 2 of Touclnnark Living Center Subdivision
the access to the east of the site becomes a public right of way then the parking stalls along the
east side of the building shaIl be eliminated.
AIl surface parking shall conform to the minimum dimensions per ordinance of9' x 19' with 25'
wide drive aisles. Compact stalls are only aIlowed in the staff parking lot to the north of the
building as depicted on the site plan (AI.! dated 12.15.04).
The landscaping is approved per the landscape plans (sheets LI.O) dated 12.15.04. AIl slreetlrees
shall be deciduous species, as shown; conifers will be allowed internaIly as shown.
This conditional use pennit shall be subject to the expiration provisions set forth in MCC 11-17-
4.B.
Applicant must comply with the conditions of the annexation, plat, planned development, and
development agreement for Touchmark Living Centers.

All exterior lighting, whether attached to the bUilding or located within the parking lot, shaIl be
down-shielded or otherwise altered so that the light does not spill Over onto adjacent properties or
right-of-way. All parking lot lighting shall be in accordance with Ordinance 11-13-4C.

All signage shaIl be in accordance with the standards set forth in Section 11-14 of the City Zoniug
and Development Ordinance. AIl signage is subject to design review and shaIl require separate
pennits. Signs shaIl conform to the requirements of the Planned Sign Program approved for
Meadow Lake Village on 11/27/02 and as submitted with the application.

AIl construction and site improvements shaIl conform to the requirements of the Americans with
Disabilities Act and the adopted building and fire codes.

A drainage plan designed by a State ofIdaho licensed architect or engineer is required and shaIl be
submitted to the City Engineer (Ord. 557, 10-1-91) for all off-slreet parking areas. Storm water
Ireatment and disposal must be designed in accordance with Department of Environmental Quality
1997 publication Catalog of Storm Water Best Management Practices for Idaho Cities and
Counties and City of Meridian standards and policies. Off-site disposal into surface water is
prohibited unless the jurisdiction which has authority over the receiving slream provides written
authorization prior to development plan approval. The applicant is responsible for filing all
necessary applications with the Idaho Department of Water Resources regarding ShaIlow Injection
WeIls.
Certificate of OCcupancy: AIl required improvements must be complete prior to obtaining a
Certificate of OCcupancy for the proposed development. A temporary Certificate of Occupancy
may be obtained by providing surety to the City in the form of a letter of credit or cash in the
amount of 110% of the cost of the required improvements (including paving, striping,
landscaping, and irrigation). A bid must accompany any request for temporary occupancy.
Sanitary sewer and water service shall be via existing service lines in Meadow Lake ViIlage that
wiIl be extended as part of this proposed development. Applicant shall coordinate size and routing
with the Public Works Department.
13. Underground year-round pressurized irtigation must be provided (ftom an existing system) to all
landscape areas on this site.
FIRE DEPARTMENT
I. Acceptance of the water supply for [ITe protection will be by the Meridian Fire Department
and water quality by the Meridian Water Department for bacteria testing.
2. Final Approval of the fire hydrant locations shall be by the Meridian Fire Department.
a. Fire Hydrants shall have the 4 Yo' outJet face the main street or parking lot aisle.
b. The Fire hydrant shall not face a street which does not have addresses on it.
c. Fire hydrant markers shall be provided per Public Works spec.
d. Locations with fire hydrants shall have the curb painted red 10' to each side of
the hydrant location.
e. Fire Hydrants shall be placed on comers when spacing pennits.
f. Fire hydrants shall not have any vertical obstructions to outlets within 10'.
g. Fire hydrants shall be place 18" above finish grade.
3. The phasing plan may require that any roadway greater than 150' in length that is not
provided with an outlet shall be required to have an approved turn around.
4. All entrance and internal roads shall have a turning radius of 28' inside and 48' outside
radius.
5. All Common driveways shall be straight or have a turning radius of 28' inside and 48'
outside and shall have a clear driving surface which is 20' wide.
6. Operational fire hydrants and temporary or permanent street signs are required before
combustible construction begins.
7. Commercial and office occupancies will require a fire-jlow consistent with the
International Fire Code to service the proposed project. Fire hydrants shall be placed
per Appendix D.
8. The medical office lot will have an unlolOwn transient population and will have an unlmown
impact on Meridian Fire Department call volumes. The Meridian Fire Department has
experienced 2397 responses in the year 2003. According to a report completed by Fire &
Emergency Services Consulting Group our requests for service are projected to reach 2800
in the year 2005 and 3800 by the year 2010.
9. Maintain a separation of5' from the building to the dumpster enclosure.
10. Provide a Knoxbox entry system for the complex prior to occupancy.
II. The flIst digit of the Office Suite shall correspond to the floor level.
12. All processes & storage practices shall be required to comply with the International Fire
Code.
13. Provide exterior egress lighting as required by the International Building & Fire Codes.
14. Where a portion of the facility or building hereafter constructed or moved into or within the
jurisdiction is more than 400 feet (122 m) from a hydrant on a fire apparatus access road, as
measured by an approved route around the exterior of the facility or building, on-site fire
hydrants and mains shall be provided where required by the code official. For buildings
equipped throughout with an approved automatic sprinkler system installed in accordance
with Section 903.3.1.1 or 903.3.1.2 the distance requirement shall be 600 feet (183).
a. For Group R-3 and Group U occupancies, the distance requirement shall be 600 feet
(183 m).

15. There shall be a flIe hydrant within 100' of all fire department connections.
POLICE DEPARTMENT COMMENT:
I. The Police Department has no concerns related to the site design submitted with the application.
SANITARY SERVrCES CORP. COMMENT:
I. Please contact Bill Gregory at sse (888-3999) for detailed review of your proposal prior
to the public hearing. There is a concern that the required modifications may significantly
impact your site design and may require a revised site plan. rf the site plan is revised,
contact the planner assigned to the project immediately to discuss the changes and how to
proceed with the revised site plan.
