RE: Blackstone Subdivision No. 2
Dear Mr. Rich:
Central District Health Department has reviewed and does approve the final plat for this
subdivision for central water and central sewer facilities. Final approval was given
June 6, 2018.
Sanitary Restrictions as required by Idaho Code, Title 50, Chapter 13 have been satisfied
according to this letter to be read on file with the County Recorder or his agent listing the
conditions of approval. Sanitary Restrictions may be re -imposed in accordance with Section
50-1326, Idaho Code, by the issuance of a certificate of disapproval.

RE: SUBDIVISION NAME CHANGE
FROM BLACKSTONE SUBDIVISION NO. 2 TO SLOE SUBDIVISION
Dear Ms. Badigian:
The Blackstone Subdivision No. 2 final plat was presented to CDHD for signature recently.
The name `Blackstone 2' was reserved by the previous owner of the subject site and reservations for
subdivision names do not run with the land. A new name has been reserved for the plat and the
reservation documentation is attached.
The Ada County Surveyor requires documentation from CDHD regarding the name change before
the plat can be signed. Attached is a copy of the revised plat for your records.
