'You Wiil recall that you previously requested connection to the City of Meridian :
utilities, and that a written agreement was part of that connection process..One of the
requirements is that you apply for, and pursue, annexation. Therefore, you will need to pay
an additional $435.40 to cover the costs for the annexation application. Please deliver your
check to Meridian City Hall at 33 East Idaho Street within 30 days of this letter.

RE: House Connection to City Sewer
Dear Mr. Werth:
The Meridian City Council, at their meeting held on October 3, 2000, approved your request to
connect your residence to the existing City sewer llne in Locust Grove Road.
As you know, City Council approval was necessary because your property is presently outside
the Meridian City limits. This approval was conditioned on you signing an agreement with the
City that in part will require you to make application for annexation of your property through the
City Planning and Zoning Department, located at 200 E. Carlton Street, Suite 201. The City
Attorney will prepare this Agreement for City Services and will send it to you for your signature.
In accordance with the City Council action last night, a part of this Agreement will require you to
connect to City water by October 30, 2002.

Please find attached the following two original hookup agreements:
1. Jerald S. Frank and Jane E. Frank; and
2. Marvin A. Werth and Violet Werth.
These two matters were under the Department Reports at the October
3, 2000 City Coundl meeting. I have prepared such agreements per action from that
meeting. They are now ready for execution by the parties, and additionally, I have
attached the originals of the Resolutions and Certificate of Clerks for both
agreements.

Last week the ownms of two separate county parcels on S. Locust Grove Road contacted the City
about connecting to the exialing city se~er that is in Locust Grove Road. Both parries have
experienced d~n field failures. I talked with the Mayor and each City Council member about the
