INTRODUCTION
This change order modifies the Scope of Work and defines compensation for Engineering
Services provided under the existing contract agreement between Keller Associates
("Consultant") and the City of Meridian, ID ("City") under the original contract agreement dated
October 7, 2014. Any conflicting provisions will be resolved in favor of this addendum.
This change order and agreement include construction administration, start-up, and
programming services throughout construction of Well 27.
SCOPE OF WORK
L Engineering Services
The information below summarizes the engineering services to be completed as part of this
change order. For budgeting purposes the contraction schedule is assumed to be 6 months
from Notice to Proceed to Final Completion.
TASK 11: CONSTRUCTION ADMINISTRATION
The Owner will oversee the construction administration efforts for the project. The Engineer will
furnish support services specifically limited to the following.
A. Preconstruction Conference
The Engineer will attend a pre -construction conference for the project with the
Contractor, Owner, and other parties. The Owner will chair the meeting. The
Engineer will provide a draft agenda for the Owner's review and prepare meeting
minutes.
B. Submittal Reviews
The Engineer will review technical submittals in a timely manner. For budget
purposes it is assumed that each contractor submittal will not require more than two
reviews.
C. Requests for Information
The Engineer will be available during construction on an as -needed basis, as
requested by the Owner, to answer questions or provide clarifications of the plans
and technical specifications. This task does not include time involved due to
contractor errors and/or deviations from the construction documents.

D. Miscellaneous Construction Support
The Engineer will be available during construction, as requested by the Owner, to
review contractor pay applications, field orders, proposal requests, and change
orders. It is understood that the Owner will contract directly with an independent
testing agency. Engineer will be available to review testing results as requested by
Owner.
TASK 12: CONSTRUCTION MEETINGS AND OBSERVATION
E. Construction Meetings and Observation
The Engineer will be available to attend construction meetings and assist the Owner
in making construction observations during construction. For budget purposes six
meetings (monthly construction meetings) and five site visits are assumed.
TASK 13: PROGRAMMING AND STARTUP ASSISTANCE
F. Programming and Startup Assistance
The Engineer's sub -consultant (DC Engineering) will program the PLC and HMI to
meet current city standards, check 1/0 point connectivity, test the control system,
configure communications to the Well 27 site, add alarms for existing Well and new
Filter Buildings, coordinate programming efforts with Filter Supplier, and test remote
control and alarm notifications.
The Engineer will attend the startup and will assist in preparing a punchlist of
outstanding contract items to be completed. The Owner will be responsible for
issuing substantial and final completion.
TASK 14: PROJECT CLOSEOUT
G. Record Drawings and Operations and Maintenance Materials
The Engineer will prepare record drawings from drawings and data received from the
Contractor and Owner's staff. Operation and maintenance materials will also be
reviewed by the Engineer. Upon approval of record drawings and operation and
maintenance materials, Engineer will provide one full size hard copy, one set of
electronic AutoCAD drawings, and one set in PDF format.
The Engineer will prepare a spreadsheet of equipment identification numbers that
correspond to the record drawings. Engineer will coordinate the tags with the City's
Asset Management Department.

Compensation
Tasks shall be completed on a time and materials basis based on the Consultants
current title code billing rates as outlined in the original contract. While individual
subtasks may be exceeded, the total time and material budget will not be exceeded
without a written addendum. A breakdown of these costs is presented in Table 1.
