Pursuant to the
MASTER AGREEMENT FOR PROFESSIONAL SERVICES
BETWEEN
CITY OF MERIDIAN (OWNER) AND JUB ENGINEERS, INC (ENGINEER)
This Task Order is made this St�l_day of July, 2015 and entered into by and between
the City of Meridian, a municipal corporation organized under the laws of the State of
Idaho, hereinafter referred to as "City", and accepted by (JUB ENGINEERS._ INC).
hereinafter referred to as "Engineer" pursuant to the mutual promises, covenant and
conditions contained in the Master Agreement (Category 1c) between the above
mentioned parties dated October 22, 2013. The Project Name for this Task Order
10568 Is as follows:

EMERGENCY RESPONSE PLAN UPDATE
PROJECT UNDERSTANDING
The City requests J -U -B to update their current Water Division Emergency Response
Plan dated September 2009. Work includes updating the Emergency Response Plan
narrative, Continuity of Operation Plan (COOP), organizational chart, contamination
event, incident command trailer, ICS protocol, Radio/Communications, Division
Notifications, Hazard Mitigation Plan, existing contact lists, major equipment inventory,
boil water notification section, and various emergency scenarios. J -U -B will consult with
the City regarding any new policies, procedures, contact lists, or emergency scenarios
that need to be added to the plan.
SCOPE OF WORK
SECTION 1 — OWNER INFORMATION. RESPONSIBILITIES and PROJECT ASSUMPTIONS
The CITY will provide the following:
• On-going review of J -U -B's work and timely considerations of policy issues
within a time acceptable to the CITY and J -U -B.
• Identify any new policies, procedures, contact lists, or emergency scenarios that
need to be added to the plan.

• Current city organizational chart, emergency notifications contact list, and
administration contact list;
• Current list of contract labs and contractors;
• Current list of vehicles and equipment for emergency use
Assumptions:
• J -U -B is able to convert hard copy to MSWord File.
• The City will assist J -U -B in obtaining confidential City information for inclusion
into this plan.
SECTION 2 - SERVICES TO BE PERFORMED BY J -U -B
J -U -B's scope of services is specifically limited to the following:
Task 1- Project Management
J -U -B's Project Manager shall direct, coordinate, and monitor schedule compliance
for all tasks, provide monthly written progress reports to the City, coordinate and
attend workshop and progress meetings with City, and prepare monthly Invoices.
Task 2- Plan Update
1. Kickoff Meeting
J -U -B will conduct an initial kickoff meeting to discuss the existing plan and
what information is needed to be updated. Additionally, the team will discuss
any additional contacts and emergency scenarios the City would like included
in this document.
2. Update 2009 Emergency Response Plan
J -U -B will prepare a draft update to the 2009 Emergency Response Plan for the
CITY'S review in track changes mode. A review meeting will be held to discuss
proposed changes to the document. Upon receiving the CITY's review and
comments, J -U -B will prepare a draft final updated Emergency Response Plan
for final review by the CITY. Final acceptance contingent on final approval.
Deliverables:
• Draft Emergency Response Plan- four (4) hard copies.
• Draft Final Emergency Response Plan -Electronic copy only
• Final Emergency Response Plan- six (6) hard copies, 1MS Word file, and one
PDF electronic file.

TIME OF COMPLETION and OMPENSATION SC LE
The following schedule is based on a Notice to Proceed (NTP) from the City by July
2015 and resulting in Final Update being completed by August 2015. A NTP issued on
a different date will change the schedule accordingly.
The Not -To -Exceed amount to complete all services listed above for this Task Order
No. 10568 is ten thousand dollars ($10,000.00). No compensation will be paid over the
Not -to -Exceed amount without prior written approval by the City in the form of a Change
Order. Monthly invoices shall be billed as a percentage of work completed. Any and all
travel will only be reimbursed if pre -approved by the Project Manager, and only per the
City of Meridian Travel Policy. Reimbursable expenses will be paid at cost and only if
pre -approved by the Project Manager, and accompanied by an itemized receipt. Any
travel and/or reimbursables paid will be paid as part of the Not -To -Exceed Task Order
Total per the Compensation and Completion Schedule above.
