On Page 18 of Section 100 of The ISPWC, Article 6, Section 6.01, Subparagraph B, Add the Following: The competent resident superintendent's duties and responsibilities shall include enforcement of OSHA regulations regarding excavations, the prevention of accidents, and the maintenance and supervision of construction site safety precautions and programs. H. On Page 26 of Section 100 of The ISPWC, Article 6, Section 6.17, Subparagraph A.l.a, Replace with the Following: a. Shop drawings are required to be submitted through e -Builder Project Management Software. III. On Page 26 of Section 100 of The ISPWC, Article 6, Section 6.17, Subparagraph A.2,a, Replace with the Following: a. Submit four (4) samples IV. On Page 15 of Section 501 of The ISPWC, Subsection 3.4, Add the Following: I. Line and Grade Contractor to submit survey information by a Professional Land Surveyor of the pipe inverts at each manhole within 48 hours of construction of the manholes to the ENGINEER and CITY. Allowable variance from the established line and grade are summarized in the table below Maximum Allowable Variance Description Allowable Variance' Line .03 inches per inch of pipe diameter or a maximum of 0.5 inches Grade .01 inches per inch of pipe diameter or a maximum of 0.25 inches Invert elevation between _01 inches per inch of pipe diameter adjoining ends of pipe, or a maximum of 0.5 inches including fittings Note 1: Variation must not result in a level or reverse -sloping invert 2. END OF SECTION Technical Specifications - NW 8m Street Sewer Trunk Main 13-1478.200 Broadway Avenue to Cherry Lane — Phase 2 500-1 SECTION SP1 SPECIAL PROVISIONS TO MODIFY THE MEASUREMENT AND PAYMENT SECTION OF THE 2012 IDAHO STANDARDS FOR PUBLIC WORKS CONSTRUCTION Measurement and payment will be on a unit price basis in accordance with the prices set forth in the proposal for individual work items. Where work (or materials) are required, but do not appear as a separate item in the proposal, the cost for that work (or materials) shall be included and absorbed in the unit prices named in the proposal. The CONTRACTOR shall make a careful assessment when preparing the bid. ISPWC Pay Items: When referenced in the bid schedule, the Idaho Standards for Public Works Construction (ISPWC), Pay Item definitions shall be used unless revised below. The following are definitions for pay items used on the project that are not defined by the ISPWC. 2010.4.1.A.I Mobilization,: Payment for mobilization will be on a lump sum basis as follows: A. 50% of the amount for mobilization shall be paid on the first monthly pay application. B. 50% (remainder of bid item) of the amount for mobilization shall be paid on the last monthly pay application. SP1 -1 Sewer Service -Plumbing Pp ermit: Payment for City of Meridian Building Department Plumbing Permit. See Section SP2 Part 1.4 Construction Within Public Rights -of -Way. Payment shall be made on per each basis. SP1 -2 6" Sleeve for Sewer Service Line - C900 PVC - Potable/Non Potable Casing: Payment for casing of 4" sanitary sewer pipe shall include casing pipe, caps, miscellaneous material and all incidental work required. Payment shall be made by the linear foot. SP1 -3 8" Sleeve for Sewer Service Line - C900 PVC - Potable/Non Potable Casing: Payment for casing of 6" sanitary sewer pipe shall include casing pipe, caps, miscellaneous material and all incidental work required. Payment shall be made by the linear foot. SP1 -4 8" Sleeve for Water Main - C900 PVC - Potable/Non Potable Casing: Technical Specifications - NW 8`h Street Sewer Trunk Main 13-1478.200 Broadway Avenue to Cherry Lane — Phase 2 SPI -1 SECTION SP2 GENERAL PROJECT REQUIREMENTS The most recent editions of the Idaho Standards for Public Works Construction (ISPWC), and City of Meridian, Ada County, Idaho, Supplemental Specifications and Drawings to the Idaho Standards for Public Works Construction (Meridian Supplement to the ISPWC) apply except as modified herein. In the case of discrepancy, unless noted otherwise herein, the more restrictive provisions shall apply. 1.1 Site Investigation and Physical Data The CONTRACTOR acknowledges that it is satisfied as to the nature and location of the work and the general and local conditions, including but not limited to those bearing upon transportation, disposal, handling and storage of materials, availability of water, roads, groundwater, access to the sites, coordination with other contractors, and conflicts with pipelines, structures and other contractors. Information and data furnished or referred to herein is furnished for information only. Any failure by the CONTRACTOR to become acquainted with the available information and existing conditions will not be a basis for relief from successfully performing the work and will not constitute justification for additional compensation. The CONTRACTOR shall verify the locations and elevations of existing pipelines, structures, grades and utilities, prior to construction. The OWNER assumes no responsibility for any conclusions or interpretations made by the CONTRACTOR on the basis of the information made available. 1.2 Temporary Utilities for Construction Purposes The CONTRACTOR shall make all arrangements necessary to provide all temporary utilities for construction purposes and shall pay all costs associated of those temporary utilities. Water for construction purposes will be furnished by the OWNER at no cost. The CONTRACTOR shall furnish all valves, hoses, connections and other devices as necessary to obtain sufficient water for construction and for filling and testing of water lines as required. Fire hydrant use is allowed only by permission of the utility owner. Backflow protection is required on all connections to potable water systems. 1.3 Field Service by Manufacturer's Representative The CONTRACTOR shall furnish the services of a manufacturer's or material supplier's representative for all major equipment and materials furnished by the CONTRACTOR or OWNER under this contract, to check, place in operation and test the installation, and train operating personnel. The manufacturer's representative shall be qualified and authorized to perform repairs and maintenance on the equipment. The above gives a general scope of the services desired from the manufacturer's representative. It will be the responsibility of the CONTRACTOR and the equipment manufacturer to determine detailed requirements. Costs for services of the manufacturer's representative shall be included in the proposal of the Technical Specifications - NW 8th Street Sewer Trunk Main 13-1478.200 Broadway Avenue to Cherry Lane — Phase 2 SP2-1 1.7 Compaction Testing The CONTRACTOR shall provide the services of a licensed, independent agency to perform compaction testing for this project per Paragraph 13.03.11 of the General Conditions. The agency must be approved by the OWNER. Compaction tests will be required to show that specified densities of compacted backfill and asphaltic concrete surfacing are being achieved by the CONTRACTOR's compaction methods. The CONTRACTOR shall provide the OWNER with copies of recent Proctor tests for the backfill and paving material in addition to copies of compaction tests performed in the field. The ENGINEER may direct testing at a higher frequency upon failure to obtain specified densities or if the CONTRACTOR changes compaction equipment or methods of compaction. 1.8 Dechlorination and Disposal of Chlorinated Water Any proposed discharge of chlorinated water shall be approved by the City of Meridian. 1.9 Limits of the Work and Storage of Spoils The limits of the site, which may be used for construction, storage, materials handling, parking of vehicles and other operations related to the project include public rights-of-way subject to permission of the ACHD. The limits of work also include rights of access obtained by the CONTRACTOR, subject to all public laws and regulations and rights of access by utility companies and other holders of easement rights. Refer to Paragraph 6.11 of the General Conditions in the ISPWC. 1.10 Protection of Existing_ Structures and Work The CONTRACTOR must take all precautions and measures necessary to protect all existing structures and work. Any damage to existing structures and work shall be repaired by removing the damaged structure or work, replacing the work and restoring to original condition satisfactory to the OWNER. 1.11 Salvage and Debris The City of Meridian will determine if they wish to keep all castings, pipe, equipment, demolition debris, spoil or any other discarded material or equipment. The CONTRACTOR shall deliver to the City any materials it chooses to keep at a location to be determined according to the materials chosen. If the City of Meridian does not want the materials the CONTRACTOR shall dispose of them in a manner compliant with applicable federal, state and local laws and regulations governing disposal of such waste products. No burning of debris or any other discarded material will be permitted. 1.12 Utility Properties and Service In areas where the CONTRACTOR's operations are adjacent to, or near a utility and such operations may cause damage which might result in significant expense, loss and inconvenience, the operations shall be suspended until all arrangements necessary for the protection thereof have been made by the CONTRACTOR. Technical Specifications - NW 81h Street Sewer Trunk Main 13-1478.200 Broadway Avenue to Cherry Lane — Phase 2 SP2-3 conducted outside of the above-described days and hours without prior approval of the OWNER AND ACHD. 1.18 Interferences Obstructions and Sewer Crossings At certain places, power, light and telephone poles may interfere with excavation and the operation of the CONTRACTOR's equipment. Necessary arrangements shall be made with utility companies for moving or maintaining such poles. The utility company affected by any such interferences shall be notified thereof so that the necessary moving or proper care of poles and appurtenances may have appropriate attention. All costs resulting from any other interferences and obstructions, or the replacement of such, whether or not herein specifically mentioned, shall be included and absorbed in the unit prices of the CONTRACTOR's bid. 1.19 Noise Limitations The project areas are located within a residential zoned area. All applicable City, County ordinances and State and Federal regulations shall be compiled with. No equipment may be started before 7:30 AM without express written permission from the OWNER and ACHD. 1.20 Storage and Protection of Equipment and Materials A. Materials and equipment stored overnight shall be placed neatly within designated storage areas approved by the OWNER. Unusable materials (i.e., rejected or damaged liner material, old concrete chunks, metal scraps, etc.) shall be expeditiously removed from the project area. Provide appropriate barricades, signs, and traffic control devices in like -new condition where necessary to protect the public from any hazards associated with the storage of materials and equipment used for this project. B. No equipment and/or materials shall be stored outside the immediate work area on public rights-of-way, in the following locations, or in the following manner: 1. In any maintained landscaped or lawn area. 2. In a manner that would totally eliminate an individual residents' street parking. 3. In front of any business. The "immediate work area" is the area where work is taking place or will be taking place within one calendar day. The CONTRACTOR shall immediately move stored material or equipment, which causes a nuisance or creates complaints. 122 Emergency Maintenance Supervisor The CONTRACTOR shall submit to the OWNER, the names, addresses and telephone numbers of at least two employees responsible for performing emergency maintenance and repairs when the CONTRACTOR is not working. These employees shall be designated, in writing by the CONTRACTOR, to act as its representatives and shall have full authority to Technical Specifications - NW 8`" Street Sewer Trunk Main 13-1478.200 Broadway Avenue to Cherry Lane — Phase 2 SP2-5 SECTION SP3 PRIVATE PROPERTY CONDITION DOCUMENTATION AND OWNER APPROVAL PART1 GENERAL This work consists of providing photographic documentation of pre -construction and post - construction conditions on all private properties and obtaining approvals and releases from all private property owners who may be impacted by the construction of the project. A. Photographic Documentation of Pre -Construction and Post -Construction Conditions — The CONTRACTOR shall provide to the OWNER photographic documentation of pre -construction and post -construction conditions on all private properties including work inside residences, if applicable. The CONTRACTOR shall provide to the OWNER digital photos of each area of work on private properties sufficient to document the conditions prior to the start of the CONTRACTOR'S work and immediately after completion of the CONTRACTOR'S work. Submit all photos to the OWNER in an organized manner on compact discs. The photographs must have the date stamp clearly indicated on them or they will be rejected as invalid. B. Property Owner Notification - Notification of planned service disruptions (water main shutdowns, sewer service interruptions, etc.) shall be made to residents and business owners 48 hours in advance and shall include the extent and type of service disruption, Contractor's contact name and phone number, and date and time of disruption. See Attachment Number 3, Property Owner Notification Form. The Property Owner Notification Form shall be sent to all affected property owners and the OWNER. C. Property Owner Approval and Release — Prior to final payment, the CONTRACTOR shall obtain a release from each property owner the form prepared by the ENGINEER confirming that the installation and site restoration work is satisfactory to the property owner. See Attachment Number 4, Property Owner Release Form. All releases shall be submitted to the OWNER. The storm sewer system discharges ultimately make their way to the Boise River. During construction, BMPs for stormwater will be implemented and maintained to treat stormwater before it leaves the site. The Boise River does have a TMDL for sediment and bacteria. The sediment/bacteria subbasin Assessment and Total Maximum Daily Loads can be accessed at httn•//www deq idaho gov/water-quality/surface-water/tmdls/table-of-sbas-tmdls/boise-river- lower-subbasin.asnx (accessed 8/12/2013). Potential sources of sediment to stormwater runoff include site excavation operations, vehicle tracking, construction dewatering, landscaping operations, and backfilling. Attachment 2 Erosion Sediment Control Plan - NW 8th Street Sewer Trunk Main 13-1478.200 Broadway Avenue to Cherry Lane — Phase 2 Page 5 Material Handling and Waste Management All waste materials will be collected and disposed of off-site. No waste construction materials will be buried on-site. All personnel will be instructed, during tailgate training sessions, regarding the correct disposal of trash and construction debris. Notices that state these practices will be posted in the office trailer and the individual who manages day-to-day site operations will be responsible for seeing that these practices are followed. All trash collected during construction will be disposed of at an off-site location for the duration of construction. One temporary sanitary facility (portable toilets) will be provided by contractor at the site throughout the construction phase. The portable toilet will be located away from concentrated flow paths and traffic flow and will have collection pans underneath as secondary containment. The portable toilet will be brought to the site for the duration of construction. All sanitary waste will be collected from the portable facilities a minimum of three times per week. The portable toilet will be inspected weekly for evidence of leaking holding tanks. Toilets with leaking holding tanks will be removed from the site and replaced with new portable toilets. Establish Proper Building Material Staging Areas All building materials will be staged.off-site. Temporary staging, stockpiling of fill and disturbed areas left inactive for more than 14 consecutive days shall be protected with temporary perimeter sediment barrier. Designate Washout Areas All washout will occur off-site. Establish Proper Equipment/Vehicle Fueling and Maintenance Practices Several types of vehicles and equipment will be used on-site throughout the project, including graders, scrapers, excavators, loaders, paving equipment, rollers, trucks and trailers, backhoes, and forklifts. All major equipment/vehicle fueling and maintenance will be performed off-site. Only minor equipment maintenance will occur on-site. All equipment fluids generated from maintenance activities will be disposed of into designated drums stored on spill pallets. Absorbent, spill -cleanup materials and spill kits will be available on site. Drip pans will be placed under all equipment receiving maintenance and vehicles and equipment parked overnight. Leaks will be repaired immediately, or the problem vehicle(s) or equipment will be removed from the project site.
