Thank you for considering Mountain Power Electrical Contractors for the electrical needs of CM
Company, Inc. I am pleased to present you with the following proposal.
Mountain Power agrees to provide the electrical installation for five street lights at the Chinden
and Linder Crossing Subdivision project per the electrical plans and specifications provided and
the following inclusions for the sum of $25,625.00. -y- $12oopo,Qo t It S7, G25.np
Material only price for pole lights: $950.00 each or $4,750.00 for all five.
This proposal includes:
• Five new 25' light poles and bases with 100 watt HPS lighting fixtures.
• A new meter cabinet located within 100' of the first light pole and within 50' of utility
power.
• Trenching and backfill (approximately 870').
• The miscellaneous tools and materials to complete the project.
• Performing the work during regular business hours.
• Applicable sales tax.
• City of Meridian electrical permit and inspection fees.

EXCLUSIONS:
' All permits, fees, inspections, material testing, surveying, layout, staking and banding.
' All excavation/removal of unsuitable soils, rock hazardous materials and debris.
" SWPPP, weeWy inspections and reporting. All installation and maintenance of BMP's unless noted in our scope of work.

* Any costs incurred for protection or re -work due to adverse weather conditions; temperatures in excess or below specifications,
site moisture conditions, rain, snow and ••mind.
' Light pole bases, signs and striping.
` Overtime due to circumstances beyond Lurre's control will be paid by responsible owner, subcontractor or general contractor.
This is a package proposal, items cannot be separated unless mutually agreed upon prior to acceptance.

* Additional work, (ASI's, CCQ's, PR's, RFPs, and RFI's) directed to be completed prior to architect, owner, or general contractor's
written approval is assumed to be approved. All worklmsts will be paid for as submitted.
* All material & workis per bid plans and specifications and ACI standards. All agreements and warranties expressed or implied are
only as attached in written form. Any alterations or deviations from project specifications involving extra costs, or any additional
quantities, will become an additional charge over and above attached specifications.
* Bonding is available but not included.
` Full payment is due and owing on completion of work. Progress payments will be made for work
completed in stages. Interest will be charged at 1:5% per month or 18%A.P.R. for delayed payments. All expenses Lurre incurs
in the collection of monies due will be reimbursed to Lurre, including attorney & consultant fees.
' No Retention to be withheld by Owner. Full payment due upon completion of work.
* Price Is based on nothing preventing Lurr& from full production. No Standby is included in price.
* Allowing Lurre to begin work on this project will be considered acceptance of this bid proposal in it's entirety.
" This proposal assumes all right-of-way, licenses, permits, fees, etc. are authorized & paid by Owner.
` This proposal is based on a mutually agreed upon schedule and sequence of milestones, including work of other subcontractors
that may impact this schedule.
` Lume Construction Inc. will not be responsible for any liquidated damages
' If this proposal is not use as an agreement it is to, in its entirety, become part of any contract agreement either by reference or inclusion.
' Commercial General Liability insurance aggregate shall not exceed $2,000,000.00. if certificate is required, information regarding
coverage's, additional insured and special wording shall be provided to Lurre prior to commencing work.
' Lurre will not be responsible forwo& disturbed or destroyed due to force ma[eure conditions, earthquakes, floods, riots, vandalism
and any other natural disasters.
" Customer further agrees to bear all costs, Including attorney fees and cost in the event this agreement is placed by Lurre
in the hands of an attorney after default for enforcement of collection before or after a suit for damages or other legal relief.
' This proposal is submitted in good faith based on the understanding that it will be held confidential by Its receiver and all associated
parties involved in its review and/or evaluation in keeping with the intention and ethics of fair bidding practices. A great deal of effort,
time and expense has been made in the evaluation and preparation of this proposal so please dont shop or peddle prices even
if just to meet quotas or budgets.

Thank you Bruce,
I've talked with CM concerning the 3 type 1 lights that were unfortunately not on the prior plans we had bid. Trey
Crookston @ CM believes these will be approximately $4,000 a piece or a total of $12,000. As an FYI we do intend to
install everything necessary per the approved construction drawings and began working on the site today. Is this
estimate of the costs of these 3 lights satisfactory for the city to provide us with a number to bond for?
